Tramadol for Cats: Overview, Dosage & Side Effects

Tramadol can be prescribed off-label for feline pain, but only under veterinary direction. Learn the reference dosing context, side effects, interactions, and overdose warning signs.

Veterinarians sometimes prescribe tramadol for pain in cats, but it is used off-label in small animals and should be given only as the prescribing veterinarian directs. A published reference dose is not a dose for your cat, and tramadol products that contain acetaminophen are dangerous to cats. If you suspect your cat has taken any tramadol beyond the prescribed amount, or any tramadol-acetaminophen product, contact a veterinarian or emergency clinic right away rather than waiting for symptoms.

What tramadol is and why vets use it in cats

Tramadol is a synthetic opioid analgesic. According to Merck Veterinary Manual’s review “Analgesics Used in Animals” (full review March 2023), veterinarians may use it alone for mild pain or as one part of a multimodal plan for more substantial pain. The drug also inhibits the reuptake of norepinephrine and serotonin, which is part of why its interaction profile is broader than that of many other pain medicines.

Cats handle tramadol differently from dogs. Cats produce substantial amounts of the active metabolite O-desmethyltramadol (M1), and Merck notes that feline oral bioavailability is higher than in dogs. This is one reason dosing directions written for dogs should not be carried over to cats.

In cats, veterinary references describe tramadol for acute and chronic pain. Merck’s discussion of osteoarthritis in dogs and cats describes a long-term oral regimen for feline osteoarthritis. The same discussion notes that evidence for chronic pain medicines in cats is limited, so tramadol’s role should be understood as one option a veterinarian weighs, not a settled standard.

Dosage: reference figures are not a home calculation

Dosing in veterinary references depends on the clinical situation and the source. The two figures most often cited for cats come from Merck Veterinary Manual pages:

These numbers describe what a reference lists. They do not establish what your cat should receive. Do not calculate, raise, lower, skip, or repeat a dose on your own. The exact strength, amount, frequency, and duration are printed on the prescription label and in the veterinarian’s instructions, and they matter most if the medicine was dispensed as a liquid.

Giving tramadol to a cat

According to VCA Animal Hospitals’ medication guide “Tramadol,” the forms that may be used in cats include oral tablets, capsules, and compounded liquid. The drug’s bitter taste can make administration difficult, and liquid forms need careful measurement. Before the first dose, ask the veterinarian or dispensing pharmacy:

  • How much of the prescribed form equals one dose, and how to measure it if the medicine is a liquid.
  • How many doses per day and for how long, and what to do about a missed dose.
  • What to do if the cat refuses the medicine or vomits soon after taking it.

Never substitute a human product. VCA specifically warns against giving cats tramadol combined with acetaminophen, quoting its guide: “Never give tramadol with acetaminophen (Ultracet®) to cats, as acetaminophen is extremely toxic to cats.”

Possible side effects

VCA lists vomiting, constipation, sedation, dilated pupils, and uneasiness as possible effects in cats. Merck adds nausea or vomiting, altered behavior, and a lowered seizure threshold. Many of these effects are mild and may pass as the body adjusts, but you should tell the veterinarian about any change you notice, especially a new or worsening one.

  • Incoordination or stumbling
  • Extreme sleepiness that is hard to rouse
  • Agitation or unusual restlessness
  • A fast heartbeat

Cautions, interactions, and conditions to disclose

Tell the veterinarian about every medication and supplement the cat receives, including anything bought without a prescription. Several factors call for extra caution or a different plan:

  • Seizure disorders or a recent seizure: VCA advises caution with seizure disorders, and Merck advises against use in animals with a recent seizure history.
  • Liver or kidney disease: VCA lists these as conditions that require caution.
  • Monoamine oxidase inhibitors such as selegiline: Merck warns against combining them with tramadol.
  • Selective serotonin reuptake inhibitors: Merck also warns against combining these with tramadol.
  • Other central nervous system depressants and other opioids: these require veterinary review before use together.

Because the list of interacting drugs is long and changes with the cat’s health, the veterinarian should review the full medicine list each time a new drug is considered.

If your cat may have taken too much or a combination product

Do not wait for symptoms after a suspected overdose or after ingestion of a tramadol-acetaminophen combination. Contact a veterinarian or emergency veterinary clinic promptly. Merck reports that cats with tramadol toxicosis may show:

  • Agitation
  • Hypersalivation
  • Hypertension
  • Lethargy
  • Dilated pupils
  • Ataxia
  • Vomiting
  • Tachycardia
  • Tremors

Merck also states there is no specific antidote. Treatment is supportive and is determined by veterinary professionals based on the cat’s condition. Have the medicine packaging or the exact product name, strength, and approximate amount available when you call, because this information helps the clinic respond faster.

What the evidence does and does not establish

Few clinical studies of tramadol in animals have been published, according to Merck. Its reference describes feline pharmacology and use, and its osteoarthritis discussion acknowledges limited evidence for chronic pain-modifying agents in cats. No single study establishes that tramadol works for every cat, and no source ranks it as the best option for feline pain.

A veterinarian compares options based on the type and severity of pain, the cat’s other health conditions, concurrent medications, how easily the cat can be medicated, and how the cat responds. If tramadol is not working or causes problems, the veterinarian can adjust the plan rather than the owner changing the dose at home.

The WSAVA “Recognition, Assessment and Treatment of Pain Guidelines” describe opioid analgesia in broader terms, but that guideline is dated and does not provide drug-specific endorsement of tramadol for cats, so it should not be read as one.
