The published reference range is 0.25–0.5 mg/kg by mouth every 12–24 hours. That range is a starting point in veterinary references, not a dose for any particular cat. The dose, the interval, and the monitoring schedule belong to your veterinarian.
What enalapril is used for in cats
The Merck Veterinary Manual describes ACE inhibitors as widely used for chronic congestive heart failure in dogs and cats, and lists enalapril as one adjunctive option for feline heart failure. The same review states there is no proof that ACE inhibitors delay the onset of heart failure in animals with subclinical heart disease. In other words, enalapril is a treatment for cats that already have clinical heart failure. It is not established as a way to prevent heart failure in a cat that does not yet have it.
VCA Animal Hospitals, in its owner-facing medication guide, describes enalapril as an ACE inhibitor used for heart failure, high blood pressure, and protein in the urine. It also states that use in cats is off-label or extra-label. Whether the drug helps depends on the specific diagnosis behind the prescription, which is why the same drug can be part of very different treatment plans.
Dosage: the published reference range
Both Merck references give the same figures for cats, and they are worth reading together:

These numbers do not work as a dose calculator. Your veterinarian selects the dose and interval based on the diagnosis, your cat’s body weight, other medicines, hydration, kidney function, blood pressure, and how your cat responds. Do not convert the range into a dose yourself, and do not split or cut tablets to match a calculation unless your veterinarian has shown you how to do it for your cat’s prescription.
Giving enalapril and handling missed doses
VCA says enalapril is given by mouth as a tablet or as a liquid. It notes that the drug may also be given intravenously in a hospital setting. Tablets and oral liquid may be given with or without food, and your cat should have access to clean water.

- Liquid doses: measure carefully. Once your veterinarian has prescribed the liquid form, an oral medication syringe is a practical way to measure the prescribed amount. The syringe does not determine the correct dose; the label and your veterinarian do.
- Do not stop abruptly: VCA advises against stopping enalapril suddenly unless a veterinarian instructs you to.
- Missed dose: VCA’s advice is to give the missed dose when you remember it, then wait the recommended interval before the next dose. Never give two doses at once, and never give an extra dose to make up for a missed one.
- Individual instructions: because schedules can differ between cats, check the prescription label or call your veterinary clinic for the rule that applies to your cat.
Side effects to watch for
VCA lists lethargy and loss of appetite as common side effects in cats, and also lists vomiting and diarrhea among common general effects. It names weakness, collapse, low blood pressure, kidney dysfunction, and elevated potassium as serious possible reactions. Merck adds that possible adverse effects include azotemia (a rise in waste products such as BUN and creatinine in the blood), and, rarely, gastrointestinal effects, fainting (syncope) caused by low blood pressure, weakness, and ataxia. The two sources frame gastrointestinal signs differently, so treat any vomiting or diarrhea as something to report to your veterinarian rather than as a sign that the drug is safe.

Dehydration and pre-existing kidney disease raise the risk of adverse effects, according to Merck. A cat that is not drinking well, or that has vomiting or diarrhea, is at higher risk and should be discussed with your veterinarian before the next dose.

Monitoring: the blood and urine checks
Monitoring is the main way the prescribing team checks whether enalapril is working and whether it is being tolerated. VCA recommends checking electrolytes, kidney values, and urine protein 1–2 weeks after treatment starts, then every 3 months once the cat is stable. VCA also notes that a complete blood count and blood pressure monitoring may be appropriate depending on why the drug was prescribed. The VCA guidance is dated June 2024 in its page metadata, so check with your clinic for any updated protocol.

Medicines and conditions that need extra caution
Enalapril interacts with several common drug classes. Merck and VCA both flag these combinations for review, so share a complete list of everything your cat receives, including supplements and over-the-counter products, with your veterinarian.

- Other vasodilators, antihypertensives, and diuretics: combined use can increase the risk of low blood pressure.
- Potassium-sparing diuretics such as spironolactone, and potassium supplements: possible high potassium (hyperkalemia).
- NSAIDs: combined with ACE inhibitors, they carry a potential risk of acute kidney injury.
- Dehydration and existing kidney disease: both raise the risk of kidney effects and other adverse reactions.
Questions to ask your veterinarian
- What diagnosis is enalapril treating in my cat, and what result would tell us it is working?
- What exact dose and interval are being used for my cat’s body weight, and when will the dose be reviewed?
- When are the next electrolyte, kidney, urine protein, and blood pressure checks due?
- What should I do if a dose is missed, and what should I do if my cat vomits after a dose?
- Do any of my cat’s other medicines, supplements, or diuretics need to change?
Other drugs may be chosen for a cat instead of enalapril depending on the diagnosis, availability, cost, and the drug’s pharmacology. Sources reviewed here do not rank enalapril against specific alternatives for any feline condition, so that comparison belongs in a conversation with your veterinarian.
