Furosemide for Cats: Overview, Dosage & Side Effects

Furosemide is a prescription loop diuretic used mainly for fluid linked to feline congestive heart failure. Here is what it does, how reference doses differ, what side effects and warning signs matter, and how monitoring works.

Furosemide is a prescription loop diuretic, often called a “water pill,” that helps a cat’s kidneys remove excess sodium and water. In cats it is most often prescribed for fluid buildup linked to congestive heart failure, including fluid in the lungs or chest cavity. Your veterinarian decides whether your cat needs it, which form to use, and how much to give and how often. The dose figures in this article are published veterinary reference context to help you follow that plan. They are not instructions for your cat, and you should never start, stop, or change the dose on the basis of a general article.

What furosemide does and why vets prescribe it

Furosemide works in the kidneys. It blocks a transporter in the thick ascending loop of Henle, a segment of the nephron, so the kidney excretes more sodium and chloride and, with them, more water. The Merck Veterinary Manual describes it as the most widely used loop diuretic in veterinary medicine.

In cats, the main reason for prescribing it is congestive heart failure. When the heart cannot move blood efficiently, fluid can leak into the lungs (pulmonary edema) or collect in the chest (pleural effusion). Reducing that fluid load helps the cat breathe more comfortably. VCA Animal Hospitals also lists lung fluid retention and certain kidney diseases among its uses. Furosemide may be supplied as a tablet or a liquid for oral use.

Long-term oral treatment and hospital treatment are different

Furosemide is used in two very different ways, and the numbers in published references apply to one or the other, not both. Doses are expressed as milligrams per kilogram of body weight (mg/kg). The table below summarizes the reference context from Merck’s feline guidance and its cardiac medication table.

Hospital protocols can include infusions, which Merck describes as settings-specific. Those protocols are not home instructions, and a cat’s home dose should never be calculated from them.

Why the dose is individualized

Merck notes that cats are more sensitive to furosemide than dogs. Severe heart failure or reduced blood flow to the kidneys can also change how much a cat needs. That is why the same reference range can mean very different things for two cats, and why your veterinarian may adjust the plan over time based on examinations, blood work, and how your cat responds.

If you think the dose needs to change, call your veterinarian rather than adjusting it yourself. If your cat has missed doses or you are unsure how much to give, ask the clinic before the next scheduled dose.

Tablet or liquid: practical points

Your cat may receive furosemide as a tablet or a liquid. Both are given by mouth. If the prescription is a liquid, measure it with the device your veterinarian or pharmacist provides, and ask them to show you how to draw up the exact volume. Do not substitute household spoons or other containers, because they do not measure reliably.

What to expect: more urination

The most common effect is increased urination. This is the medicine doing its job. Expect more trips to the litter box and larger urine volumes. Make sure fresh water is always available, and watch whether your cat keeps eating and drinking normally. Increased urination is expected; a sudden change in how much your cat drinks or urinates, or a drop in appetite, should be reported to your veterinarian.

Side effects

Side effects follow from the same action that makes furosemide useful. Merck says the most important adverse effects relate to volume depletion and dehydration, reduced cardiac output and glomerular filtration, and electrolyte or acid-base abnormalities. Cats with pre-existing kidney disease have a higher risk.

  • Common: increased urination (VCA).
  • Less common: vomiting and pancreatitis (Merck).
  • Uncommon: temporary or permanent deafness linked to rapid intravenous administration (Merck).
  • Consequences of over-diuresis: dehydration, changes in kidney blood flow, and abnormal potassium, sodium, or acid-base levels, which are found on blood tests before they are always obvious at home.

Warning signs that need an immediate call

VCA’s owner guidance says to stop giving the medicine and contact your veterinarian immediately if your cat shows any of the following:

  • Weakness or collapse
  • Head tilt or problems with balance
  • Signs of electrolyte imbalance
  • No urine production
  • A racing heart rate

VCA also advises calling your veterinary office at once if you suspect an overdose or an adverse reaction. That sentence comes from VCA’s owner guide, not from a named author. If you cannot reach your own clinic, contact an emergency veterinary hospital without delay.

Monitoring: blood tests and home follow-up

Furosemide needs veterinary monitoring because kidney function, electrolytes, and hydration all change with treatment. Merck’s feline reference recommends the following schedule for kidney values:

  • When the diuretic is started
  • At least one week later
  • At least every 3–6 months during chronic use

Merck also notes that mild to moderate prerenal azotemia (a rise in kidney waste markers caused by reduced blood flow to the kidneys) may occur and is often tolerated when a cat is eating and drinking well. That is a judgment your veterinarian makes from the full picture. Do not decide an abnormal result is unimportant on your own.

VCA lists electrolytes, kidney values, blood sugar, and hydration among the items a veterinarian checks. Blood pressure, weight, and hearing may also be assessed when indicated.

Sleeping respiratory rate log for cats with heart failure

For cats with diagnosed heart failure, Merck recommends that owners count the cat’s breathing rate daily while it is asleep, record each result in a log, and bring that log to rechecks. Merck’s reference describes a healthy cat’s sleeping respiratory rate as under 40 breaths per minute, and under 30 in most healthy cats. Use these figures only as background. A single reading is not a diagnosis, and your veterinarian should tell you what changes in your cat’s own numbers warrant a call. Ask them to show you how to count so your readings are consistent.

Precautions and drug interactions

VCA states that furosemide should not be given to pets that cannot make urine or that have worsening kidney disease. Caution is advised with dehydration, electrolyte imbalance, liver or kidney disease, diabetes, active vomiting or diarrhea, pregnancy or lactation, and possible sulfonamide sensitivity.

VCA lists ACE inhibitors, aspirin, corticosteroids, digoxin, insulin, and theophylline as medicines that need caution when combined with furosemide. Furosemide can also increase the potential for kidney or hearing toxicity from other drugs. Merck highlights a specific concern with aminoglycoside antibiotics, which can add to kidney and ear damage. Give your veterinarian a complete list of every medicine, supplement, and herbal product your cat receives, including those bought without a prescription.

Questions to take to your veterinarian

  • What is the exact dose in milligrams, how often, and in which form?
  • When should the next kidney and electrolyte blood tests be done?
  • How much urination is expected for my cat, and what change should prompt a call?
  • Should I count my cat’s sleeping breathing rate, and how do I record it?
  • What should I do if a dose is missed or vomited?
