Cisapride is a prescription prokinetic that some veterinarians use to help move stool through a cat’s colon. The published veterinary reference range is 0.1–0.5 mg/kg by mouth every 8–12 hours for mild to moderate idiopathic constipation, with higher doses of up to 1 mg/kg described for more severe cases. These figures explain how veterinarians think about the drug. They are not a dose for your cat. Only the prescribing veterinarian can set an amount, and that amount depends on your cat’s weight, the severity of the constipation, other medications, and the specific compounded product being dispensed.
If your cat has not passed stool for more than a day or two, is straining without producing feces, is vomiting, is lethargic, has a swollen or painful belly, or has had any reaction after a dose, contact your veterinarian now. Do not wait for the next scheduled dose, and do not add laxatives or change the dose at home.
What cisapride does in cats
Cisapride is a gastrointestinal prokinetic, meaning it is intended to improve movement through the digestive tract. The Merck Veterinary Manual describes colonic prokinetic agents such as cisapride as increasing colonic propulsive motility through activity at 5-hydroxytryptamine-2A receptors in colonic smooth muscle. In practical terms, the drug is meant to help the colon contract and propel its contents, rather than to remove a blockage or treat the underlying reason stool stopped moving.
Veterinary references describe its use in cats with idiopathic constipation (constipation with no identified cause) and megacolon, as well as in other gastrointestinal motility disorders. A prokinetic does not diagnose the problem, does not treat every cause of constipation, and does not replace a physical examination, bloodwork, or imaging when those are needed.

Does cisapride help feline megacolon?
The Merck Veterinary Manual states that anecdotal experience suggests cisapride can stimulate colonic propulsive motility in cats with mild to moderate idiopathic constipation. It also cautions that cats with longstanding obstipation (the inability to pass stool at all) and megacolon (a markedly enlarged, poorly functioning colon) are unlikely to improve with cisapride alone. Improvement in a mild case does not mean the drug will reverse advanced disease. Many cats with chronic, advanced constipation need a broader plan that may include enemas, manual removal of stool under sedation or anesthesia, diet and fluid changes, and long-term monitoring.
What dose of cisapride is referenced for cats?
The Merck Veterinary Manual gives an anecdotal oral range of 0.1–0.5 mg/kg every 8–12 hours for cats with mild to moderate idiopathic constipation. It says higher doses, up to 1 mg/kg, may be needed in moderate to severe constipation. These values come from veterinary reference information for clinicians. They are not a home dosing guide, and they are not an individualized prescription for any particular cat.

Why your veterinarian’s directions may differ from an online range
The American Association of Veterinary Pharmacology and Therapeutics (AAVPT) cisapride monograph also presents feline dosing information, including a compounded oral suspension regimen. The same monograph states that cisapride is not specifically approved for veterinary use and that feline clinical efficacy studies are lacking. It also notes that the effective dose may depend on how the medication is compounded, and that oral bioavailability (the fraction of the drug that reaches the bloodstream) may vary among formulations.
This means a number you find online may not match the strength in your cat’s bottle, and the same milligram amount from two different compounding sources may not act the same way. Your veterinarian’s instructions account for the specific product, your cat’s condition, and what they have observed in response to treatment.

Is cisapride safe for cats?
The Merck Veterinary Manual reports no major adverse effects in cats treated at 0.1–1 mg/kg every 8–12 hours. That is a limited report, not a guarantee. It does not come from a controlled feline trial with stated incidence rates, and the references reviewed do not supply a percentage for adverse effects in cats. Do not read “no major adverse effects reported” as “no side effects” or as “proven safe.”
Cardiac concerns in people
The Merck manual notes that cisapride was removed from the human market after cardiac rhythm disorders and deaths were reported in people. It also states that these human problems were highly associated with concurrent drugs or specific underlying conditions. That history is important context for any prokinetic, but it does not establish identical risk in cats. Equally, the veterinary reporting that exists does not prove that cats have no cardiac risk. Tell your veterinarian about every medication, supplement, and health condition your cat has, because those factors are part of the safety assessment.

Signs that warrant a call to your veterinarian
- Vomiting, drooling, or refusing food after a dose
- Unusual lethargy, weakness, collapse, or fainting
- Diarrhea, new bloody stool, or a sudden change in stool consistency
- Worsening straining, crying in the litter box, or repeated attempts without passing stool
- A swollen, firm, or painful abdomen
- Any accidental extra dose, missed dose, or dose given with a different product than prescribed
Why does a cat need compounded cisapride?
In the United States, cisapride is not conventionally manufactured as a finished product for cats. The Merck Veterinary Manual states that it must be compounded. The DailyMed listing for cisapride monohydrate powder is a bulk ingredient intended for animal drug compounding. It is not a finished consumer product, and it is not evidence that the drug has been approved for a particular feline treatment. A compounding pharmacy prepares the medication according to a veterinarian’s prescription, which is why the strength, vehicle, and expiration can differ between sources.
Because bioavailability can vary among formulations, follow the formulation, storage, and administration directions supplied by your veterinarian and dispensing pharmacy. If the product, color, volume, or schedule changes when you refill, ask before giving the next dose.

When constipation needs more than a prokinetic
Constipation in cats can require treatment beyond a prokinetic. The Merck manual describes enemas and/or manual extraction for moderate to severe or recurrent constipation and obstipation. It also notes that stimulant laxatives such as bisacodyl are contraindicated when bowel obstruction is present. For that reason, do not add over-the-counter laxatives, stool softeners, or enemas on your own, and do not assume a blocked cat is simply constipated. Obstruction, megacolon, and other serious causes need a veterinary examination to be ruled in or out.
Ask your veterinarian to explain what they are trying to achieve with cisapride, how long a trial should last before they reassess, and what improvement should look like for your cat. A good plan includes a defined recheck, a clear list of warning signs, and a named contact for after-hours questions.
