Food that is offered too fast or in too large an amount can cause a dangerous metabolic reaction in a cat that has gone without eating for a long time. The six steps below are a sequence for getting the cat assessed, keeping it stable while you wait, and following a clinician’s written plan. They are not a protocol for rehabilitating a cat at home.
Why a starving cat cannot be fixed at the kitchen counter
Most people who search for how to nurse a starving cat want to know what to feed it and how much. The honest answer is that the amount, the type of food and the speed of reintroduction all depend on the cat’s condition, and some of those are only knowable through an examination and blood work.
The main danger is refeeding syndrome. The 2022 International Society of Feline Medicine (ISFM) consensus guideline on managing the inappetent hospitalised cat states: “Occasionally, severely debilitated cats can develop a life-threatening metabolic condition termed ‘refeeding syndrome’ if fed in excess of what they can process.” The guideline’s authors are Samantha Taylor and coauthors, and it was published in the Journal of Feline Medicine and Surgery in 2022. According to the same guideline, refeeding syndrome can involve severe electrolyte and glucose abnormalities, weakness and neurologic signs. These changes can appear after food is reintroduced, which is why a cat that finally starts eating still needs monitoring.

The ISFM guideline lists cats at higher risk as those with prolonged lack of food, severe debilitation, substantial body-condition loss, or pre-existing electrolyte abnormalities. If your cat fits any of these descriptions, do not wait to see whether it starts eating on its own.
The six steps, in order
1. Call a veterinarian or an emergency clinic
Call the closest veterinary practice. If it is closed, or if the cat is weak, collapsed, vomiting or unable to stand, go to an emergency clinic. Have this information ready:
- How thin the cat looks, including visible ribs, spine or hip bones, and any recent weight change you know about
- How long it has likely gone without food, and whether it has eaten anything at all
- Whether it is still drinking water and passing urine
- Any other symptoms, such as vomiting, diarrhoea, hiding, breathing changes or unusual sleepiness
- Any medicines, supplements or diet changes already made
Not eating is a symptom in its own right. The Cornell Feline Health Center’s anorexia guidance advises prompt veterinary consultation, because an underlying medical problem may be behind the loss of appetite. The same guidance notes that some nutritional support requires professional expertise.

2. Arrange an examination and transport
Do not delay the visit to try a home feeding routine. Severe debilitation, a long period without food and major body-condition loss are the factors that make refeeding risky, and a veterinarian needs to assess them directly. Expect the clinic to check hydration, temperature, heart and breathing rate, and to run blood tests that look at electrolytes and glucose before deciding how food should be reintroduced.
Carry the cat in a secure carrier with a towel for warmth. Keep the trip short and calm, and avoid handling the cat more than necessary.
3. Offer fresh water while you wait
Keep fresh, clean water available in a shallow dish near the cat. The ASPCA’s general nutrition guidance says fresh clean water should always be available. Do not substitute cow’s milk for water, because it is not a suitable replacement for a cat that has been deprived of food.

If the cat is not interested in water, do not force it. Tell the veterinarian at the examination.
4. Do not force food or liquid
Do not syringe food, slurry or liquid into the cat’s mouth, and do not push food toward its face, unless a veterinarian has specifically instructed you to do so and has shown you how. Forced feeding can cause aspiration, which is inhalation of food or liquid into the lungs, and it does not address the reason the cat stopped eating.

5. Follow the veterinarian’s written diet and schedule
The veterinarian will set the diet, the starting amount and the rate of increase. Ask for these in writing, including the feeding times, the amount per meal, what to do if the cat refuses food, and when the next recheck is due. Do not apply a generic calorie formula from a website or a pet food label to a severely starved cat.
The ISFM guideline describes a deliberately slow approach for at-risk cats, with monitoring at each stage, and the details are covered in the next section. Merck’s general critical-care guidance for animals starved for an extended period describes a similar low start and gradual increase.
If the veterinarian approves a commercial diet, a complete and balanced canned cat food is generally designed to meet the nutritional needs of cats without special needs. A veterinarian should still guide the choice of food and the feeding plan during illness or recovery, because the right diet depends on the cat’s condition.

6. Watch for changes and attend every recheck
Keep a simple log of each meal and each change you observe. Report any of the following to the clinic promptly, even if they seem minor:
- Vomiting, diarrhoea or a sudden refusal of food
- Weakness, trembling, unsteadiness or collapse
- Unusual confusion, seizures or changes in behaviour
- Drinking much less water, or passing much less urine
Because refeeding syndrome can develop after eating has resumed, do not assume that a successful first meal means the danger has passed. Attend every planned recheck, even if the cat seems to be doing well.

Two published clinical protocols give concrete numbers. They describe how veterinarians approach these cases in a clinic. They are not feeding instructions for a caregiver, and they are not interchangeable.

The ISFM figures are protocol values for cats the guideline considers at risk. The Merck figures are general small-animal values. Neither is a calorie target for your cat. The correct starting amount depends on the cat’s weight, its condition, its blood results and how it responds, and only the treating veterinarian can set it.

Treat it as a serious risk for any cat that has gone without food for a long time, not as a rare event you can dismiss.
What not to do
- Do not give a large meal to make up for lost weight.
- Do not offer supplements, vitamins, treats or a named recovery food unless the veterinarian has approved them.
- Do not use the ASPCA’s dog refeeding guidance or any dog protocol for a cat.
- Do not wait several days to see whether the cat eats on its own.
- Do not stop follow-up visits once the cat starts eating.
A cat that is starving has at least one serious underlying problem, and the cause cannot be identified from the outside. Feeding advice for one cat will not automatically suit another.
