AAHA’s 2026 guidance outlines how veterinary teams can help caregivers manage cat diabetes with clear instructions, safe treatment routines, and useful home monitoring.

AAHA’s 2026 guidance for veterinary teams treats diabetes care as a shared routine: caregivers need clear instructions, realistic expectations, and ongoing support. For cat owners, the practical message is to follow the treatment plan made with your veterinary team, know what to do when something goes wrong, and use home monitoring to inform—not replace—veterinary care.

What Section 11 covers

Section 11 of the 2026 AAHA Diabetes Management Guidelines for Cats, published April 26, 2026, focuses on educating caregivers. It recommends transparent, supportive, empathetic conversations about the commitment involved, access to trained veterinary team members, and written instructions caregivers can refer to at home.

AAHA emphasizes that successful control is achievable, but it may take weeks or months. Remission is possible and often occurs within 3–6 months after treatment begins, according to the guideline, but it is not guaranteed and is not the only goal. The day-to-day focus is continued care and monitoring.

What to do when your cat will not eat, vomits, or seems unwell

Do not guess whether to give, skip, or change a dose based on a missed meal or vomiting. Instructions differ by treatment, so ask your veterinary team for a written plan that covers these situations before they arise. Contact the clinic promptly if your cat has poor appetite or seems ill; AAHA advises veterinary evaluation when a cat taking an SGLT2 inhibitor has either sign.

If your cat takes an SGLT2 inhibitor

SGLT2 inhibitors are not insulin. They are a different type of diabetes treatment, and their administration, potential complications, monitoring, follow-up, and missed-dose or vomiting instructions must be explained for the specific prescription. AAHA calls for close observation during the first two weeks of therapy because diabetic ketoacidosis is more likely during this period. Follow the veterinary team’s instructions about when to seek help.

If your cat takes insulin

Use the dose and schedule prescribed for your cat. Never increase the dose or give injections more frequently without clear veterinary direction. Ask for explicit instructions about what to do if a dose is missed, your cat vomits, or your cat does not eat; do not apply one cat’s plan to another.

How to give insulin safely

Insulin handling depends on the formulation. Your veterinary team should show you how to store and prepare the specific product, measure the dose, and use its syringe or pen. A syringe that looks suitable is not necessarily the right one.

  • Match the syringe concentration to the insulin: U-40 insulin requires a U-40 syringe, and U-100 insulin requires a U-100 syringe. They are not interchangeable.
  • Some insulin formulations require a pen. Use the delivery device specified for your cat’s prescription.
  • Follow product-specific instructions about refrigeration, mixing, and handling; these are not the same for every insulin.
  • AAHA cites a manufacturer-recommended in-use period of 28–56 days for most insulin types. Ask your veterinary team how that applies to your product, and contact them if you doubt its efficacy or your cat’s clinical signs recur.

Have a trained team member demonstrate the technique, then show it back so any measurement or handling errors can be corrected. Keep the written dose and administration instructions where everyone who helps care for your cat can find them.

Recognizing possible low blood sugar

Possible hypoglycemia can cause lethargy, unusual sleepiness, behavior changes, an abnormal gait, weakness, tremors, or seizures. If you notice these signs, use the individualized response plan your veterinary team provided and follow its urgent-care instructions. If you do not have a plan, contact a veterinarian urgently rather than improvising a dose change.

Home monitoring: what each method can tell you

Home monitoring can help you and your veterinary team understand how your cat is doing, but it does not replace veterinary evaluations. Discuss which method fits your cat and care plan, how to collect readings, and how the team wants the information recorded. Do not use a home reading alone to adjust insulin.

Method What it measures or records How to use it with your veterinary team
Veterinary-calibrated blood glucose meter A blood glucose measurement at the time of testing. AAHA discusses veterinary-calibrated meters, including AlphaTrak 3 as an example. Ask which meter and testing approach suit your cat; the example is not a universal endorsement.
Continuous glucose monitor (CGM) Glucose patterns over time rather than only a single test point. Review the data with your veterinary team. AAHA says even 2–3 days of CGM data can be useful in cats, although human labeling may specify 15 days.
Urine glucose strips Glucose in urine, not a direct blood glucose reading. Ask your veterinary team how urine results fit your cat’s monitoring plan and what they can—and cannot—tell you.
Glucose-detecting litter A change in litter that indicates glucose in urine. Discuss whether this method is compatible with your cat’s care and how to report results.
Water-intake records How much water your cat drinks over a measured period. AAHA identifies recording water consumption over 24 hours as a useful practical measure. Ask how to measure it and share the record with the clinic.

Choose the method with your veterinary team, not by assuming one product is best for every cat. Keep observations and readings in the format the clinic requests so they can interpret them alongside examinations and other clinical information.

Plan for the time and emotional load

Diabetes care affects household schedules and caregiver well-being. AAHA notes that insulin is often administered twice daily and SGLT2 inhibitors once daily; some newer insulins may allow less frequent injections for some patients. These are general examples, not a schedule to substitute for your veterinarian’s prescription.

Insulin needs can change, which may mean more veterinary visits. Ask what follow-up is expected, who to contact with questions, and which tasks are essential if caregiving becomes difficult. AAHA specifically recommends discussing the impact on caregiver quality of life; fatigue and grief are practical concerns worth raising with the veterinary team.
