Diabetes in Cats: Signs, Diagnosis, Treatment, and When to Seek Help

Increased thirst, urination, appetite, and weight loss can be signs of feline diabetes, but veterinary testing is needed to confirm it. Treatment is tailored to the cat and may involve insulin, nutrition, monitoring, or—only for selected cats—an oral medicine.

Diabetes mellitus is a condition in which a cat’s body does not make enough insulin or cannot respond to insulin effectively, leaving blood sugar persistently high. Increased thirst and urination, increased appetite, and weight loss are common warning signs, but only a veterinarian can confirm the diagnosis. Many cats can be managed with individualized treatment and monitoring; some go into remission, but remission is not guaranteed.

What diabetes means for a cat

Insulin helps regulate glucose in the blood. When insulin production or the body’s response to it is insufficient, glucose can remain high and spill into the urine. That can lead to increased urination and thirst, while the cat may lose weight despite eating more. Weakness or difficulty jumping may also occur. These signs can have other causes, so they are reasons to arrange a veterinary assessment rather than a basis for diagnosing diabetes at home.

Some early changes can be subtle: an owner may notice that a cat “seems a little off” or is “less interactive lately.” Mention such changes, along with changes in appetite, drinking, urination, activity, or weight, when speaking with the veterinarian.

How veterinarians confirm diabetes

A single high blood-glucose result may not be enough to diagnose a cat. Stress during a veterinary visit can temporarily raise a cat’s glucose, so the result must be considered alongside the cat’s history, clinical signs, and urine findings. Merck Veterinary Manual describes diagnosis as persistent hyperglycemia together with glucose in the urine; repeat blood and urine samples may be needed.

A veterinary evaluation commonly includes blood testing and urinalysis. The veterinarian may also use fructosamine, which can help distinguish persistent diabetes from a stress-related glucose rise, or recommend additional tests. Because affected cats may have other health conditions, the clinician may evaluate for concurrent disease as part of deciding what care is appropriate.

How feline diabetes is managed

Treatment is individualized. Insulin is a common treatment, alongside veterinary-guided nutrition and maintaining an appropriate body condition. The plan depends on factors such as whether the cat needs insulin, overall health and concurrent disease, how treatment can be given at home, monitoring needs, and the veterinarian’s assessment of likely response.

Some cats may enter diabetic remission after diet and insulin management. This is not assured, and remission does not mean an owner should change or stop treatment without veterinary direction. Do not start, stop, or adjust insulin or other diabetes medication on your own.

Insulin, nutrition, and weight management

The veterinarian will determine whether insulin is appropriate and explain how to administer it if prescribed. Diet changes and weight management should also fit the cat’s clinical needs and treatment plan; a food marketed for diabetes is not a substitute for veterinary care.

Monitoring and home testing

Monitoring may involve clinical signs, serial blood-glucose measurements, glucose curves, or fructosamine testing. For some cats, a veterinarian may recommend home monitoring and teach the owner how to use the equipment. VCA names AlphaTRAK 2 meters and FreeStyle Libre skin-mounted sensors as examples. A reading needs veterinary interpretation, and a home device does not replace the clinician’s overall assessment.

When oral SGLT2 medicines may be considered

In the United States, the FDA has approved two SGLT2 inhibitors for glycemic control in selected cats: bexagliflozin (Bexacat) and velagliflozin (Senvelgo). These medicines lower blood glucose by increasing glucose excretion in urine. They are not insulin and are not suitable for every cat.

The FDA states: “An SGLT2 inhibitor is not insulin and is not for use in cats with the type of diabetes mellitus that requires insulin treatment.” FDA labeling and communications contraindicate these drugs for cats that have previously received insulin, are currently receiving insulin, or have insulin-dependent diabetes. A veterinarian must determine whether a cat is an appropriate candidate.

FDA effectiveness figures are specific to the studies and periods reported, and should not be read as cure or remission rates. The agency reported Bexacat was over 80 percent effective in improving glycemic control in two six-month field studies and an extended-use field study (2022). For Senvelgo, it reported over 80 percent effectiveness in improving glycemic control in one six-month field study (2024). The underlying studies differed, so these figures are not a direct comparison between the medicines.

Important safety signs during SGLT2 treatment

The FDA warns that SGLT2 inhibitors can be associated with diabetic ketoacidosis (DKA), including euglycemic DKA. In euglycemic DKA, a cat can have this serious condition without the expected high glucose reading; a seemingly normal result does not rule it out. FDA-listed concerning signs in treated cats include loss of appetite, lethargy, dehydration, weight loss, vomiting, diarrhea, weakness, and changes in drinking or urination.

If a cat taking one of these medicines becomes ill or stops eating, contact the veterinarian promptly and follow the specific instructions provided for that cat. Do not wait for glucose to rise or treat a reassuring reading as proof that the cat is safe.

When diabetes signs may be an emergency

Weakness, depression, refusal to eat, vomiting, collapse, or a severely unwell state in a diabetic cat can signal DKA and requires emergency veterinary care. Seek urgent veterinary help if these signs occur. For a cat on an SGLT2 medicine, sudden appetite loss, lethargy, dehydration, or weight loss also warrants immediate veterinary assessment, regardless of the glucose reading.
