How Vets Diagnose Diabetes Mellitus in Cats

A single high blood sugar result may reflect clinic stress. Learn how vets confirm sustained diabetes in cats and what the common tests can—and cannot—show.

Vets diagnose diabetes mellitus (DM) in cats by confirming that high blood sugar is sustained—not by relying on one high reading taken at a clinic. Stress can temporarily raise a cat’s glucose, so the result must be interpreted alongside symptoms and evidence from repeat testing, fructosamine, or home monitoring.

What findings make a vet suspect diabetes?

Increased thirst and urination, weight loss, and changes in appetite can prompt a diabetes evaluation. A vet considers these signs alongside the physical examination and laboratory results. In a cat with compatible signs, blood glucose may be high enough for glucose to appear in the urine, but neither finding alone confirms diabetes: stress can cause temporary hyperglycemia, and hyperglycemia or glucosuria is not specific to DM.

How is sustained high blood sugar confirmed?

The 2026 American Animal Hospital Association (AAHA) feline guideline, published April 26, 2026, calls for evidence of sustained hyperglycemia. It identifies increased glycated protein concentrations—especially fructosamine—or hyperglycemia or glucosuria documented on more than one occasion while the cat is nonstressed or at home as evidence that can support diagnosis. The vet interprets these findings in context rather than treating any single test as definitive for every cat.

Fructosamine

Fructosamine measures glycated plasma proteins, including albumin, and reflects average blood glucose over approximately 7–10 days. It can help distinguish ongoing high glucose from a brief clinic-related rise. However, hyperthyroidism and protein-losing diseases, including protein-losing enteropathy or nephropathy, can lower fructosamine by shortening albumin’s half-life. A lower result therefore needs interpretation alongside the cat’s condition and other findings.

Repeat readings at home or when the cat is not stressed

A continuous glucose monitor (CGM) or portable blood glucose monitor (PBGM) can record glucose outside the clinic and help show whether high readings persist. If those approaches are impractical, a veterinarian may suggest checking urine for glucose at home. AAHA advises waiting at least 2–3 days after an in-hospital exam or other stressful event before collecting urine, and aiming for at least two samples voided on different days. Urine glucose strips can be used for this purpose if the veterinarian recommends them; a strip result does not diagnose diabetes by itself, and a negative result does not rule it out.

HbA1c

HbA1c is described as reflecting average glucose over roughly 2–3 months. The AAHA task force reported that it was unaware of feline HbA1c assays validated in diabetic cats, so this longer time window should not be mistaken for an established, definitive feline diagnostic test.

What tests are part of the broader workup?

AAHA recommends laboratory testing to assess the cat’s health and look for concurrent disease or complications. The diagnostic evaluation for a cat with signs suggestive of DM includes:

  • A complete blood count (CBC).
  • A chemistry panel that includes cholesterol, triglycerides, and electrolytes.
  • Urinalysis with sediment examination.
  • Serum total thyroxine (TT4).

When the cat’s condition or disease severity raises concern, the veterinarian may also assess ketones. In a patient meeting DM criteria, ketonuria or blood beta-hydroxybutyrate (BHB) above 1.0 mmol/L suggests a more complicated or advanced disease state and warrants veterinary interpretation.

What if the results do not clearly confirm diabetes?

If a cat has compatible signs but confirmation remains difficult, AAHA says the veterinarian may consider treatment or reassess the cat’s weight and laboratory findings within 1–2 weeks. This is an individualized veterinary decision, not a home treatment plan or a universal retesting schedule. Follow-up also matters when a high clinic reading may be stress-related: a vet can determine whether the cat has persistent diabetes or needs monitoring for risk rather than a confirmed diagnosis.

How to interpret a high glucose result

Method What it can show Important limitation
Clinic blood glucose Blood glucose at the time of the test. Stress can temporarily raise glucose, so one high clinic result does not establish DM.
Fructosamine Average glucose over approximately 7–10 days. Hyperthyroidism and protein-losing disease can lower the result independently of glucose.
CGM or PBGM Glucose patterns collected outside the clinic, including at home. Results require veterinary interpretation alongside symptoms and other findings.
Home urine glucose checks Repeated evidence of glucosuria outside the stressful clinic setting. AAHA advises waiting at least 2–3 days after a stressful visit and aiming for samples from two different days. A result alone does not diagnose or rule out DM.
HbA1c Average glucose over roughly 2–3 months. The AAHA task force was unaware of feline HbA1c assays validated in diabetic cats.
