Are fish pedicures better than regular pedicures? No: they may temporarily remove dead or thickened surface skin, but evidence does not show superior results, and live fish plus shared tank water create sanitation and injury concerns. Risk appears low for healthy adults, but it is not zero or reliably measurable.

A fish pedicure immerses the feet in water containing live Garra rufa, sometimes called doctor fish. The fish nibble at thickened or dead skin for a cosmetic exfoliation effect. The central trade-off is straightforward: the procedure may smooth the skin temporarily, but the live-animal tank is harder to sanitize and the treatment is unsuitable for several common medical risk groups.

Key takeaways

  • Fish pedicures can remove some dead or thickened surface skin, but current evidence does not show that they are better than conventional callus care.
  • Infection risk appears probably low for the general population in available public-health assessments, but ANSES says the risk cannot be quantified reliably because data are insufficient.
  • Live fish, fish mouths, and shared tank water cannot be sterilized like ordinary salon equipment without harming the fish.
  • People with diabetes, poor circulation, immune suppression, reduced sensation, open wounds, active skin disease, or suspected foot infection should avoid fish pedicures or obtain medical advice first.
  • A foot file or pumice stone can provide more controllable cosmetic callus reduction for appropriate low-risk users.
  • Fish-pedicure rules vary by location; California and New York prohibit the service under their cited regulations, while UK guidance focuses on risk controls.

Are fish pedicures better than regular pedicures?

Fish pedicures are not proven to be better than regular pedicures, a foot file, a pumice stone, or professional callus care. Fish may temporarily exfoliate dead or thickened skin, but the service offers less control over pressure and hygiene than a personal tool, and the live-fish tank creates sanitation issues that ordinary salon equipment does not.

Decision factor Fish pedicure Conventional pedicure Foot file or pumice stone
Evidence of benefit Cosmetic removal of some surface dead or thickened skin; no strong evidence of superior results. Cosmetic nail and skin care; benefit depends on the service and technique. Gradual cosmetic thinning of ordinary calluses is supported for suitable low-risk users.
Hygiene control Live fish and a shared tank cannot be treated with ordinary sterilization methods while keeping the fish alive. Tools and footbaths can be cleaned or disinfected, but poor sanitation can still transmit infection. A personal tool avoids shared tank water and can be cleaned, disinfected, or replaced according to its design.
Possible trauma Nibbling may cause minor abrasions or bleeding; a published case report also described nail shedding. Cutting, filing, or footbath exposure can injure skin when performed aggressively or on damaged skin. Over-filing can make skin tender or cause bleeding, but the user can stop and control pressure immediately.
Medical suitability Not appropriate for people with diabetes, poor circulation, immune suppression, open wounds, active lesions, or suspected infection. People with the same risk factors may need clinician-directed foot care rather than casual salon treatment. Appropriate only for ordinary calluses in people without relevant medical risk factors; diabetes or poor blood flow requires medical advice first.
User control Lower: the treatment depends on animal contact in a communal tank. Moderate: the technician controls much of the procedure. High: the user controls location, pressure, duration, and when to stop.
Regulatory status Varies by jurisdiction; some locations prohibit or restrict the service. Usually regulated under ordinary salon and public-health rules. Generally a personal-care method, although medical conditions still affect whether self-treatment is appropriate.
Appropriate endpoint Cosmetic smoothing only; not treatment for psoriasis, eczema, fungal infection, or diabetic foot disease. Cosmetic care unless a qualified clinician is directing treatment. Cosmetic callus reduction only; persistent or medically complicated foot problems need professional evaluation.

What does a fish pedicure do?

A fish pedicure, also called a fish spa or ichthyotherapy, places the feet in water containing live Garra rufa, sometimes called doctor fish. The fish nibble at thickened or dead surface skin. Providers may also promote fish pedicures for psoriasis, eczema, circulation, or other health benefits, but those broader medical claims are not established by the evidence summarized here.

Rikans Colossal Foot File Foot Rasp Callus Remover for Soft Smooth Feet

  • Large yet light weight design allows you to work with ease and gives quick amazing smoothness
  • Gives equally good results on both dry and wet foot surfaces that allows the user to work according to their own preference and satisfaction
  • Sharp Tool, do not put pressure while using. This Tool is designed to work with gentle motions on dead skin, thick callouses etc.
  • For best results, use it gently and according to the manufacturers directions.

The most defensible benefit is temporary cosmetic exfoliation. Removing loose surface skin can make feet feel smoother, but that outcome does not demonstrate that fish are more effective or safer than a controllable foot file, pumice stone, moisturizer, or professional callus treatment.

Can fish pedicures help psoriasis or eczema?

Fish pedicures should not be used as a substitute for prescribed psoriasis, eczema, fungal-infection, or diabetes-related foot care. Evidence for therapeutic ichthyotherapy is limited, and the available psoriasis study tested a different, combined intervention rather than an ordinary cosmetic fish spa.

According to Grassberger and Hoch (2006), a retrospective pilot study of 67 people with psoriasis reported a 71.7% reduction in PASI score after three weeks of ichthyotherapy combined with short-term UVA exposure; 31 of 67 participants reached PASI-75, and 61 of 67 reached at least PASI-50. The study was not a randomized controlled trial, included UVA as part of the intervention, and took place in a specialized treatment setting, so the results cannot establish that a routine fish pedicure independently treats psoriasis. The study is indexed in the 2006 psoriasis pilot-study record.

“Currently, there is no strong evidence to support the routine use of ichthyotherapy for psoriasis or other cutaneous diseases.”

That conclusion comes from the 2020 dermatology review of fish-pedicure applications. Active eczema or psoriasis plaques can also be vulnerable skin lesions, which is a reason to avoid fish-pedicure exposure rather than deliberately subjecting inflamed skin to nibbling and communal water.

Can fish pedicures cause infections?

Yes. Fish pedicures can expose feet to microorganisms in the fish, the fish mouths, and the tank water, although the exact infection rate for modern fish pedicures is not known. ANSES assessed the overall risk as probably low but higher for susceptible people and stated that the risk could not be quantified because available data were insufficient.

A study of Garra rufa intended for pedicure services found potentially harmful bacteria in the fish and noted that documented human infections had been few. Fish-spa exposure has been associated in reports and reviews with organisms including Aeromonas sobria and Mycobacterium marinum. The pathogen study of fish used for pedicures and a published report of periungual Mycobacterium marinum infection following a fish manicure support treating the hazard as plausible and documented, not as a precisely measurable percentage.

That distinction matters. The available literature includes public-health assessments, laboratory pathogen studies, case reports, and a small pilot study, but it does not provide a large definitive trial showing that fish pedicures are superior to ordinary cosmetic callus care or a reliable general infection percentage.

What does a conventional footbath outbreak show?

A conventional footbath can also become an infection source when sanitation is inadequate or skin is damaged. According to a 2002 New England Journal of Medicine outbreak investigation, 110 customers developed furunculosis after using whirlpool footbaths at one nail salon, and 34 cultures were positive for rapidly growing mycobacteria. Razor shaving before the pedicure was reported by 70% of patients compared with 31% of controls. The 2002 footbath-outbreak investigation involved conventional whirlpool baths, not necessarily fish pedicures, so these figures must not be presented as a direct estimate of fish-pedicure risk. The investigation does show why broken skin and inadequate footbath sanitation matter.

The U.S. Environmental Protection Agency advises consumers not to use foot spas when they have open wounds such as bug bites, bruises, scratches, cuts, scabs, or poison ivy. EPA guidance also advises avoiding shaving, waxing, or hair-removal cream during the 24 hours before a foot-spa treatment. Those precautions are written for foot spas generally, but they are especially relevant to any treatment involving water exposure and potentially damaged skin. See the EPA guidance on preventing pedicure foot-spa infections.

Why is a fish-spa tank harder to sanitize?

The sanitation problem is structural, not simply a question of whether salon staff are careful. Ordinary equipment can be emptied, cleaned, and disinfected between customers. Fish, fish mouths, and the tank environment cannot be sterilized with the same methods while keeping the animals alive, so the tank cannot be managed exactly like a conventional footbath or a reusable salon tool.

UK public-health guidance identifies this difference and recommends risk-management measures covering user screening, hygiene, tank management, fish health, and prevention of transmission rather than ordinary sterilization alone. A business operating a fish spa should be able to explain its local licensing status and its procedures for those controls; a clean-looking tank does not prove that the environment is sterile. The relevant UK fish-pedicure public-health guidance describes the sanitation and management issue.

Can fish pedicures cause bleeding or nail damage?

Fish pedicures can cause minor skin trauma, particularly around damaged or inflamed skin. A dermatology review describes mild temporary bleeding from open crusted lesions and notes that minor trauma may increase infection risk.

Nail injury is also possible, although the evidence does not show how often it occurs. A published case report described onychomadesis after a fish pedicure. Onychomadesis means separation or shedding of the nail plate from the nail bed; one case report cannot establish that the complication is common, but it is enough to show that fish pedicures are not risk-free. The 2018 case report on onychomadesis following a fish pedicure documents this uncommon reported complication.

Who should avoid fish pedicures?

People with conditions that make small wounds, contamination, or delayed healing more consequential should avoid fish pedicures or obtain individualized medical advice before considering one. The higher-risk groups identified in public-health and dermatology guidance include:

  • People with cuts, abrasions, cracked skin, ulcers, blisters, insect bites, scabs, or any other open wound.
  • People with active eczema, psoriasis plaques, unexplained scaling, or another open or inflamed skin lesion.
  • People with a current foot or nail infection, including a suspected fungal or bacterial infection.
  • People with reduced sensation or impaired wound healing from any cause.

ANSES specifically identifies people with diabetes, immunocompromised people, and people with dermal lesions on the feet as higher-risk users. The ANSES fish-pedicure risk assessment explains why the general risk assessment should not be treated as personal clearance for these groups.

People with diabetes should be especially cautious about any service that can create a small wound. The American Academy of Dermatology advises people with diabetes to obtain medical care for foot calluses rather than treating calluses casually at home. A foot problem that looks like an ordinary callus can also reflect pressure, infection, ulceration, or another condition requiring clinical care.

What is a safer alternative to a fish pedicure?

For an otherwise healthy person with intact skin who wants to reduce ordinary callus buildup, a gentle foot file for calluses or pumice stone is a more controllable alternative than placing the feet in a shared tank. Dermatology and clinical guidance supports gradual filing or pumice use for suitable calluses, while warning against excessive removal and self-treatment when diabetes or poor blood flow is present. See the American Academy of Dermatology callus-care guidance and the Mayo Clinic callus-treatment guidance.

How should a low-risk adult use a foot file or pumice stone?

  1. Soak the callused area to soften the skin.
  2. Gently file or use a pumice stone to thin the callus gradually.
  3. Stop if the skin becomes tender or bleeds; do not try to remove all thickened skin in one session.
  4. Moisturize afterward.
  5. Do not use sharp blades, aggressive scraping, or strong chemical callus removers as generic substitutes.

A foot file or pumice stone is a cosmetic callus-care method, not a treatment for psoriasis, eczema, fungal infection, unexplained scaling, or diabetic foot disease. People with diabetes or poor circulation should consult a clinician before treating a callus themselves; Mayo Clinic gives the same caution for people with diabetes or poor blood flow.

Why are fish pedicures banned in some states?

Fish pedicures are banned or restricted in some jurisdictions because live fish and tank water cannot be disinfected using the same methods required for ordinary salon equipment. The legal issue is therefore tied to sanitation and regulatory compliance, not proof that every fish pedicure causes an infection.

Fish-pedicure legality is not uniform across the United States. California’s Board of Barbering and Cosmetology states:

“No. The Board determined that disinfection methods for ‘fish pedicures’ were not consistent with the Board’s regulations.”

That statement appears in the California Board of Barbering and Cosmetology FAQ. New York’s current appearance-enhancement procedural determinations list Fish Pedicure (Garra rufa) as prohibited; the New York procedural-service determination dated March 2025 is the relevant current document supplied for that jurisdiction.

Other jurisdictions use guidance and operating precautions rather than an outright ban. Before booking, check the state cosmetology board, local health department, and the establishment’s licensing status. A service being available in one city, state, or country does not establish that the service is legal or appropriately regulated where you live.

What should you decide before booking?

For ordinary cosmetic smoothing, the balance generally favors a controllable foot file, pumice stone, moisturizer, or qualified professional callus-care service over a fish pedicure. The fish treatment may remove surface skin, but the benefit is limited, the therapeutic claims are weak, and the tank creates a sanitation constraint that cannot be solved by ordinary disinfection.

  • Skip the fish pedicure if you have diabetes, poor circulation, immune suppression, reduced sensation, open skin, active eczema or psoriasis lesions, a fungal or other infection, or impaired healing.
  • Do not book with recently damaged skin: EPA foot-spa guidance advises avoiding the service after shaving, waxing, or hair-removal cream during the preceding 24 hours and avoiding foot spas when open wounds are present.
  • If local law permits the service and you are otherwise low risk, verify the business’s license and ask how it screens customers and manages tank hygiene, fish health, and infection prevention.
  • Seek medical care instead of cosmetic exfoliation when a callus is painful, recurrent, associated with a wound, or accompanied by suspected infection or another unexplained skin change.

The Bottom Line

Bottom line: Fish pedicures can make surface skin feel smoother, but they are not proven to be better than conventional callus care. Because live fish and shared tank water cannot be sterilized like ordinary salon equipment, and because infection, skin trauma, and nail injury are documented concerns, a gentle foot file or pumice stone is the more controllable cosmetic option for appropriate low-risk adults.
