Yes. Flat-faced cats can be prone to breathing problems because a shortened muzzle may occur alongside changes inside the upper airway that make airflow harder. The risk and severity vary: a flat face does not mean every cat has a breathing disorder, and a breed label alone cannot predict an individual cat’s condition.
Why a flat face can affect breathing
“Flat-faced” describes a shortened facial shape, also called brachycephaly. The concern is not appearance alone: shortened facial conformation can be accompanied by altered upper-airway structures that restrict airflow. Possible contributors include narrow openings at the nostrils, narrow nasal passages, and a relatively long soft palate. A cat may have one or more of these features, and the degree of obstruction can differ between individuals.

When air has to move through a narrowed passage, breathing may become noisy or require greater effort. The UK Animal Welfare Committee notes that a shortened muzzle can be accompanied by internal upper-airway changes that restrict airflow. The University of Illinois College of Veterinary Medicine explains that an obstructed upper airway can increase the negative pressure needed to draw breath. Its veterinary clinical medicine professor, Dr. Heidi Phillips, describes what affected animals experience: “What they feel when they do so is the increased negative pressure required to breathe against an obstruction of the upper airway, something brachycephalic dogs and cats feel with almost every breath every day of their lives,”
That explanation helps distinguish a harmless-sounding snore from a sign that airflow may be restricted. It does not establish that every snoring cat has an airway disorder, nor can an owner determine the anatomy or seriousness of an obstruction from a sound alone. The sound is an observation; whether an individual cat has clinically important obstruction is a question for veterinary assessment.

Which cats may be affected?
Persians and Exotic Shorthairs are among the flat-faced cats discussed in veterinary material. University of Illinois College of Veterinary Medicine commentary also identifies Himalayans as cats in which similar breathing problems occur. These are examples, not a list that establishes how likely an individual cat is to have clinically important obstruction.
Facial shape and signs vary. Some cats may show little or no obvious difficulty, while others have noisy breathing or trouble during activity. An extreme-looking face may prompt attention, but appearance cannot substitute for an assessment of how the cat breathes and whether its airway is restricted. A breed label alone is not a diagnosis, and the evidence does not establish that every cat in a named breed will develop breathing disease.
Signs that breathing may be affected
Possible signs described in veterinary and animal-welfare guidance include:
Do these 3 things before closing this tab:
1Repair Windows errors before they cause bigger problems2Scan for outdated or missing drivers - takes under a minute3Clear out junk files and repair common Windows errors- Snoring or snorting, particularly while asleep.
- Raspy, noisy, or otherwise increased respiratory sounds.
- Breathing difficulty after exercise or during ordinary activity.
- Difficulty breathing during stress or in hot weather.
- Reverse sneezing.
- A sedentary pattern that accompanies the other signs.
These observations are reasons to pay attention, not a home screening score or a diagnosis. Noisy breathing and breathing difficulty are not interchangeable: a sound may be noticeable without proving the extent of obstruction, while visible effort or recurring difficulty deserves veterinary attention. The UK Animal Welfare Committee summarizes survey evidence linking more extreme feline brachycephaly with increased respiratory noise during sleep, difficulty breathing after exercise, and sedentary behavior. The Scottish SPCA describes a range from mild snoring or snorting to severe, potentially life-threatening breathing difficulty.

Context can help you describe what you notice. The signs mentioned in Scottish SPCA guidance include difficulty with ordinary activity or exercise, stress-related difficulty, and more severe problems in hot weather. Pay attention to whether a sound happens during sleep or whether difficulty appears after activity; note whether it recurs or seems effortful. This is useful information to share with a veterinarian, not a way to grade severity at home. The cited guidance does not set a universal owner-facing threshold for how much noise or effort is acceptable.
When not to dismiss a sound as “normal for the breed”
Owners may become accustomed to a cat’s snoring or raspy breathing, especially when other cats of a similar type make sounds too. The University of Illinois College of Veterinary Medicine commentary specifically warns that owners may mistake abnormal breathing for a normal breed trait. A recurring sound, a change in breathing, or difficulty during everyday activity should therefore be described to a veterinarian rather than assumed to be harmless because of the cat’s appearance or breed.
It is also important not to make the opposite assumption: one noisy breath does not, by itself, establish an airway disorder. The useful distinction is between recording an observation and deciding what it means. A veterinarian can assess the cat rather than relying on a breed stereotype or a sound heard in isolation.

The evidence supports an association between shorter facial proportions and respiratory concerns, but the studies used different populations and measurements. Their figures should not be combined into one risk estimate or treated as the prevalence among all flat-faced cats.

| Study | What was examined | What it found | How to interpret it |
|---|---|---|---|
| 2016 PLOS ONE study | A survey of 1,000 cats; respiratory scores included owner-reported noisy breathing while asleep and breathing difficulty after activity. | Shorter measured nose or muzzle proportions were associated with higher respiratory scores. | This is an association involving reported signs, not a clinical diagnosis of every cat and not a universal prevalence estimate. |
| 2021 Journal of Feline Medicine and Surgery study | Researchers examined 64 Persian and 12 Exotic Shorthair show cats and compared facial features with 20 non-purebred domestic shorthair or longhair cats seen at a veterinary hospital. | Among the 76 show cats, owners of 6 reported increased respiratory sounds and/or trouble breathing at least weekly. Moderate-to-severe stenotic nares were found in 86% of the Persian/Exotic show-cat group. | The cats were a selected show-cat sample, and the reported respiratory signs came from owner questionnaires. Neither figure represents all Persian, Exotic Shorthair, or flat-faced cats. |
The two studies measure different things. The 2016 PLOS ONE study connected shorter measured nose or muzzle proportions with higher owner-reported respiratory scores across a survey of 1,000 cats. The score included noisy breathing during sleep and difficulty breathing after activity. That finding points to an association across the surveyed cats; it does not mean that every short-muzzled cat had a clinical diagnosis.
The 2021 Journal of Feline Medicine and Surgery study gives a closer look at a selected group of show cats and their facial features. Owners of 6 of the 76 Persian and Exotic Shorthair show cats reported increased respiratory sounds and/or trouble breathing at least weekly. Separately, researchers found moderate-to-severe stenotic nares in 86% of that Persian/Exotic show-cat group. The 86% figure concerns a physical finding in the study sample, not the proportion of all flat-faced cats with breathing symptoms. Nor does the owner-reported figure provide a general rate for either breed.
The 20 non-purebred domestic shorthair or longhair cats in the 2021 study were comparison cats examined at a veterinary hospital; they should not be treated as a population-wide control group for estimating the risk to all cats. The populations and measures differ from those in the 2016 survey, so the numbers should not be pooled or used to calculate a single likelihood for an individual pet.
The 2016 survey’s association does not by itself show that shortened proportions caused each reported sign. Likewise, the 2021 finding about stenotic nares describes the examined group, not a percentage that can be applied to every cat of those breeds. The UK Animal Welfare Committee’s summary of survey evidence also supports an association between increasingly extreme feline brachycephaly and respiratory noise during sleep, difficulty after exercise, and sedentary behavior. None of these findings supplies a single population-wide prevalence estimate or a universal threshold owners can use to decide whether a cat has a disorder.

What to do if your cat has noisy or difficult breathing
- Notice the pattern. Pay attention to whether sounds occur during sleep, after activity, during ordinary movement, in heat, or when the cat is stressed. Also note whether breathing seems effortful or the signs are recurring. These details help describe the concern without trying to diagnose its cause.
- Contact a veterinarian about recurring signs or apparent effort. A veterinarian can assess the individual cat; facial appearance or a breed name cannot confirm the presence or severity of airway obstruction. Severe breathing difficulty can be potentially life-threatening, so do not treat it as a routine breed characteristic.
- Describe what you observe rather than self-diagnosing. Report the type of sound or difficulty and when it occurs. Terms such as “snoring,” “raspy breathing,” “reverse sneezing,” and “trouble after activity” communicate observations; they do not establish a diagnosis such as BOAS.
- Follow case-specific veterinary advice. The cited guidance does not support treating anatomical obstruction with generic consumer products, supplements, or owner-performed procedures. Ask the veterinarian what findings apply to your cat and what options, if any, are appropriate.
There is no product comparison that answers whether a flat-faced cat has restricted airflow. Cooling products, supplements, or nasal devices should not be presented as treatments for anatomical obstruction on the basis of the guidance summarized here. The practical comparison is between the cat’s individual clinical findings and the severity of its signs, as assessed by a veterinarian—not between generic products or home remedies.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone. Can treatment or surgery help?
Veterinary procedures may alleviate some signs in selected cats, but they are not a guaranteed cure. Scottish SPCA guidance states that surgery cannot cure BOAS and that signs can recur or worsen. Whether any intervention is suitable depends on the individual cat and should be decided with a veterinarian, not inferred from a flat face or a particular breathing sound.
This distinction matters because “can help” and “cures the condition” are different claims. The guidance supports the possibility of symptom relief in some cases, while warning that recurrence or worsening remains possible. It does not establish one procedure or outcome for every flat-faced cat. An owner should not attempt treatment based on appearance alone or assume that an intervention will be necessary or beneficial without veterinary advice.

What owners should take away
Flat-faced cats can be more susceptible to restricted airflow because shortened muzzles may be accompanied by narrow nostrils, narrow nasal passages, or a relatively long soft palate. Studies and welfare guidance associate more extreme facial conformation with respiratory signs, but they do not show that every cat is affected or provide a general prevalence figure.

Snoring, snorting, raspy breathing, reverse sneezing, or difficulty during activity, stress, or heat should not automatically be written off as “normal for the breed.” If signs recur or the cat appears to breathe with effort, seek a veterinary assessment. The meaningful question is not only how flat the cat’s face looks, but what is happening to that individual cat’s airflow and breathing.
Frequently Asked Questions
Are all flat-faced cats going to have breathing problems?
No. Flat facial conformation can be associated with restricted airflow, but severity varies, and the studies do not show that every flat-faced cat is affected. A breed label or appearance alone cannot determine an individual cat’s condition.
Is snoring normal for Persian or other flat-faced cats?
Snoring or snorting can occur, but it should not automatically be dismissed as normal for the breed. The University of Illinois College of Veterinary Medicine warns that owners may mistake abnormal breathing for a breed trait. Recurring sounds or difficulty with ordinary activity are worth discussing with a veterinarian.

Persians and Exotic Shorthairs were included in a 2021 Journal of Feline Medicine and Surgery study. University of Illinois College of Veterinary Medicine commentary also identifies Himalayans. These examples do not predict whether a particular cat has airway obstruction.
Can surgery cure breathing problems in flat-faced cats?
Not necessarily. Scottish SPCA guidance says surgery cannot cure BOAS; procedures may alleviate signs in selected cats, but signs can recur or worsen. A veterinarian should assess whether any intervention is suitable for an individual cat.
Does the 86% figure mean 86% of flat-faced cats have breathing problems?
No. In the 2021 study, moderate-to-severe stenotic nares were found in 86% of the selected Persian and Exotic Shorthair show-cat group. It is not a prevalence estimate for all flat-faced cats, and it is a finding about nostril narrowing rather than a rate of diagnosed breathing disease.
The Bottom Line
Flat-faced cats can be prone to restricted airflow, but not every cat is affected and severity varies. Recurrent snoring, noisy or effortful breathing, or difficulty with activity, stress, or heat should not be written off as “normal for the breed.” A veterinarian can assess the individual cat; appearance alone cannot diagnose an obstruction.
