Blood in a Cat’s Urine: Can Stress Be the Cause?

Stress can be linked to feline idiopathic cystitis, but it is not the only cause of blood in a cat’s urine. Learn when to seek emergency care and how vets investigate.

Stress may contribute to feline idiopathic cystitis (FIC), a bladder condition that can cause blood in a cat’s urine. But blood alone cannot show that stress is the cause: stones, infection, urethral plugs, obstruction and other diseases can produce similar signs. A veterinarian must assess the cat and rule out other causes.

If your cat is repeatedly straining or visiting the litter box but passing little or no urine, seek emergency veterinary care now. A urinary blockage can be life-threatening, and you should not wait to see whether it clears on its own.

When blood in a cat’s urine is an emergency

Repeated attempts to urinate with little or no output may mean the urethra is blocked. This is more common in male cats because their urethra is longer and narrower. A blocked cat may cry out or appear distressed; as the problem progresses, lethargy, poor appetite, reluctance to move, vomiting or dehydration may occur.

Suspected obstruction requires immediate veterinary attention. Do not wait for a routine appointment or try to treat a blockage at home. Even if your cat is passing some urine, contact a veterinarian promptly about new blood, painful urination or unusually frequent attempts. The appearance of the urine cannot identify the cause.

How stress can be related to blood in the urine

Blood in the urine is called hematuria. It is one possible sign of feline lower urinary tract disease (FLUTD), an umbrella term for signs associated with bladder or urethral problems—not a single diagnosis. Other signs can include painful or frequent urination, straining, crying while urinating and urinating outside the litter box.

One possible cause is feline idiopathic cystitis (FIC), a bladder condition in which stress may play a role. Its underlying cause is not fully understood; veterinary guidance describes an interplay involving the lower urinary tract, stress and neurohormonal factors. Potential stressors include moving, changes in household animals, environmental changes and altered feeding schedules. These associations do not prove that stress caused a particular cat’s bleeding.

Other possible causes include urinary stones, bacterial infection, urethral plugs, obstruction and, less commonly, tumors. Because these conditions can look alike to an owner but need different care, blood in the urine should be treated as a sign to investigate rather than a diagnosis.

How a veterinarian finds the cause

A veterinarian typically takes a history, examines the cat and tests a urine sample. If these steps do not establish the cause, further evaluation may include blood work, X-rays, ultrasound or a urine culture. Which tests are appropriate depends on the cat’s signs and initial findings.

What published figures do—and do not—show

Cornell’s 2022 Spanish-language owner brochure says FIC accounts for up to 70% of cats with lower urinary tract disease. “Up to” matters: this figure is not a universal estimate for every cat population or every case of blood in the urine.

In a separate owner Q&A, Cornell gives bacterial causes as 1–3% of lower urinary tract disease cases in young-to-middle-aged cats and about 10% in cats over 10. Those figures refer to the Q&A’s context; they do not mean infection can be ruled out in an individual cat.
