A cat may develop irritation or other problems while wearing a calming collar, but the best direct trial evidence did not find more overall adverse-reaction reports with the pheromone collar tested than with a regular collar. That finding applies to one product and a 28-day study—not every calming collar, ingredient, or cat.
Which problems have been reported?
These are reported concerns and practical risks, not a universal list of proven effects caused by pheromones. A change that appears after fitting a collar deserves attention, but it does not by itself establish what caused it.

Skin or coat changes beneath the collar
Local irritation, lesions, or other skin changes can occur where a collar sits. Possible explanations include friction or other mechanical effects, sensitivity, and injury from the cat scratching itself. The available study does not identify one cause for every lesion, so a sore spot should not automatically be called an allergy or poisoning.

Some cats may scratch at a collar, resist wearing it, or otherwise fail to tolerate it. This can be a collar-fit or tolerance problem; it is not, on its own, evidence of a systemic reaction to pheromones.

Behavior changes noticed during use
Caregivers in the trial reported potential adverse reactions, but the category included varied observations. The study does not establish that pheromone exposure caused every reported change, and it does not support a reliable symptom-by-symptom rate for calming collars generally.
Loss or removal
A collar that comes off or is lost cannot keep delivering its contents as intended. In the 28-day trial, 12.1% of collars were lost or definitively removed by the study’s end. Both study collars had breakaway mechanisms; that feature supports physical safety but does not prevent irritation or every injury.

What did the controlled study find?
Endersby, Billy, and De Jaeger’s randomized, controlled study, published in Frontiers in Veterinary Science on December 19, 2024, enrolled 624 cats from 459 households. It compared a polymer collar containing 13% FELIWAY Optimum with a regular silicone control collar for 28 days. Efficacy data were complete for 491 cats. The authors reported potential adverse reactions in 27.2% of cats overall, with no difference in overall reporting frequency between the two collar groups. Read the study.

The 27.2% figure is the proportion reported across both groups during that particular study protocol. It is not a market-wide rate, does not describe any one symptom, and does not show that every event was caused by the pheromone formulation. The study also does not establish long-term safety beyond its 28-day period or equivalent effects for other ingredients and collar designs.

For the behaviors measured, the pheromone collar group improved more than the control group for problem urination (P = 0.0172), scratching (P = 0.0013), and conflict between cats (P = 0.0029). Fear scores improved more, too, but that difference was not statistically significant (P = 0.063). These results are evidence about the tested collar and protocol, not a guarantee that a collar will resolve a behavior problem.
When should you remove the collar and contact a vet?
Remove the collar and contact your veterinarian if your cat develops a concerning reaction, especially a significant skin lesion or marked distress. Seek veterinary guidance rather than trying to diagnose the cause from a list of possible symptoms. A veterinarian can assess the skin, fit, timing, and other potential explanations.

For behavior concerns, an individualized assessment matters: stress-related urination, aggression, fear, and other behaviors can have different causes and may need more than one kind of support. ASPCA advises consulting a veterinarian or qualified behavior professional before medication for behavior problems. See ASPCA guidance.
Would a diffuser or spray be a better fit?
Pheromone products are available as collars, diffusers, and sprays, but evidence for one delivery format does not establish the safety or effectiveness of another. The practical choice depends on the behavior being addressed, whether your cat tolerates the format, and whether evidence applies to that use.
