What side effects should you watch for?

VCA Animal Hospitals lists mild gastrointestinal upset as a possible side effect of carnitine. Its guidance does not give a rate or percentage for dogs receiving typical veterinary doses, so the likelihood cannot be quantified from that source. VCA’s carnitine guidance also advises contacting a veterinarian about anything unusual, because other side effects may occur.
Possible allergic reaction
VCA identifies irregular breathing, rash, fever, and puffiness or swelling around the face as signs of a possible allergic reaction. Stop giving the supplement and contact your veterinarian immediately if any of these occur. A dog may develop a sensitivity after repeated exposure, even if earlier doses seemed to be tolerated.

Why might a veterinarian prescribe L-carnitine?
L-carnitine has veterinary uses, including use alongside other medications for canine dilated cardiomyopathy (DCM). That does not mean every dog with DCM needs it, or that it is a general wellness supplement. Tufts Cummings School of Veterinary Medicine notes that most dogs with DCM do not have carnitine deficiency, though supplementation may sometimes be recommended for Boxers with DCM. The decision depends on an individual dog’s veterinary assessment.

An FDA-hosted veterinary document also discusses the difficulty of assessing carnitine deficiency in dogs and reports involving selected dogs with DCM. Those cases do not establish a general need for supplementation. Reports about diet and canine DCM likewise do not, by themselves, establish that L-carnitine causes or prevents the disease; the FDA cautions that adverse-event report counts alone cannot prove causation.
What do dog studies tell us about side effects?
An FDA-hosted ingredient notice summarizes a chronic study in beagles given L-carnitine chloride at graded doses. Diarrhea was reported in all dogs in the 1,600 mg/kg/day group and in some dogs in the 800 mg/kg/day group. Vomiting was more frequent at doses of at least 800 mg/kg/day and often occurred soon after dosing.

These observations come from specific experimental dose groups and a particular formulation. They are not estimates of side-effect rates during ordinary veterinary use and are not dosing instructions for a pet. The notice also reports that all dogs survived to scheduled termination and that the study found no treatment-related ECG, eye, or ear findings; those observations do not establish a safe dose for an individual dog.
A peer-reviewed study of Labrador Retrievers notes that relatively few studies have assessed L-carnitine’s effects on canine exercise performance and recovery. It is not a study of adverse-effect frequency, so it cannot establish how often dogs experience side effects.

What should you tell your veterinarian before starting it?
- List all medicines, vitamins, herbal therapies, and supplements your dog receives. VCA flags valproic acid for caution and recommends discussing all such products with the veterinarian.
- Tell the veterinarian if your dog is pregnant or may be pregnant. VCA advises caution with use in pregnant pets and says not to use carnitine in pets allergic to an ingredient in the product.
- Use only the veterinarian’s instructions for your dog. Do not derive a home dose from experimental studies; measure liquid forms carefully.
VCA describes oral capsules, tablets, liquids, and powders, and says carnitine may be given with or without food. The right form and directions depend on the veterinarian’s plan for your dog.
