Canine Osteoarthritis: Why It Can Go Undiagnosed

Canine osteoarthritis may appear as subtle changes in activity, sleep, posture, or willingness to climb stairs—not just a limp. Learn how it is assessed and managed.

Canine osteoarthritis (OA) can develop without an obvious limp or a prior diagnosis. A dog may instead become less active, hesitate at stairs, sleep differently, or change posture—subtle shifts that can be mistaken for normal aging. These signs warrant a veterinary assessment, but no single behavior or X-ray alone establishes that a dog is experiencing painful OA.

Why canine osteoarthritis can go unnoticed

OA is a concern when gradual mobility or behavior changes are attributed to a dog’s age or usual variation. Owners may recognize that something is different yet wait to seek help because the change seems minor or develops slowly. In a 2026 AAHA report, veterinary surgeon Darryl L. Millis said owners may interpret early changes as “old age” or slowing down without recognizing OA as a possible cause. The report’s quotation is reproduced from AAHA’s account.

A dog does not have to limp for an owner to notice a potential problem. Changes to watch for include:

  • Hesitating or refusing to climb stairs or steps.
  • Reduced activity or reluctance to take part in familiar activities.
  • Changes in sleep, posture, or the way a limb is held.
  • Altered movement that appears only in particular situations or comes and goes.

These signs are reasons to discuss mobility with a veterinarian, not proof of OA. Other conditions can also cause movement or behavior changes.

What the screening evidence shows—and what it does not

A May 26, 2026 AAHA article reports on a University of Tennessee Veterinary Medical Center study that screened 30 dogs receiving routine dental prophylaxis. The dogs were 4–10 years old and weighed more than 24 pounds. Dogs with known OA, prior orthopedic surgery, systemic disease, or chondrodystrophy were excluded. Screening included owner questionnaires, an orthopedic examination by a board-certified surgeon, force plate analysis, and sedated radiographs of several joints.

Dogs with OA that had two or more affected joints 16 of 18 dogs (89%)
Dogs with radiographic coxofemoral (hip) OA 37% of the sample

These results show that radiographic OA was found among some dogs without a known OA history in this particular screening group. They do not estimate how common OA is among dogs generally: the sample was small and selected, and the AAHA report is a secondary account rather than the complete primary paper. Most dogs with OA in the reported group also had notable abnormalities on lameness and/or physical examination.

How to record changes before a veterinary visit

Brief, concrete observations can help a veterinarian understand what has changed and when. Note which activities are affected, whether the difficulty is consistent or intermittent, and any changes in sleep, posture, or ordinary movement. If a behavior occurs only at home or in a specific setting, a video may help show it during the appointment.

Structured owner questionnaires can help capture demeanor, mobility, lifestyle, and change over time. The Canine Brief Pain Inventory (CBPI) and Liverpool Osteoarthritis in Dogs (LOAD) are widely used assessment and monitoring aids, but neither is a stand-alone diagnosis.

How veterinarians assess suspected OA

Assessment draws on several kinds of information: the owner’s history, observations of posture and gait, a hands-on orthopedic examination, and imaging when indicated. The COAST consensus approach stages OA using owner and veterinary information, risk factors, clinical signs, examination findings, and radiography.

Radiographs can support confirmation and staging, but an X-ray finding does not by itself show how much pain a dog is experiencing. AAHA notes that pain on joint palpation and radiographic OA may correlate poorly; COAST guidance says treatment decisions should be based on clinical signs rather than radiographic findings alone. A questionnaire, a limp, or an imaging result therefore should not be treated in isolation as proof of painful OA.

How OA management is tailored

A veterinarian can build a plan around the dog’s signs, examination, health, and daily function. Management may combine several approaches rather than rely on one measure.

  • Weight optimization: Merck Veterinary Manual describes weight optimization as important for overweight dogs with OA.
  • Controlled exercise: Regular, controlled activity is a management component. A veterinarian can help determine an appropriate approach for the individual dog.
  • Pain treatment: Merck characterizes NSAIDs as predictably effective for OA, but whether a particular medication is suitable—and how it should be monitored—requires veterinary direction.
  • Rehabilitation: Therapeutic exercise and physical rehabilitation may be part of a multimodal plan, as described in AAHA guidance.

Supplements are not a substitute for assessment

The Merck Veterinary Manual reports that EPA supplementation and EPA-rich diets have demonstrated improved gait and mobility and an NSAID-sparing effect in dogs. Its entry lists EPA supplementation at 50–100 mg/kg by mouth every 24 hours; this is reference information, not an individual dosing recommendation. Ask a veterinarian whether an EPA omega-3 fish oil supplement for dogs or an EPA-rich diet is appropriate. The cited sources do not establish a preferred retail formulation or brand.

The same reference reports that systematic-review evidence did not support glucosamine/chondroitin supplements for OA pain management. Supplements should not replace veterinary evaluation or prescribed treatment.

When to ask for help

Bring gradual or intermittent mobility changes to a veterinarian rather than assuming they are simply part of aging. Share specific examples and any useful video; the veterinarian can decide whether examination, questionnaires, imaging, or another evaluation is appropriate.
