Carbon Monoxide Poisoning in Cats: Warning Signs and Emergency Care

Carbon monoxide exposure is an emergency for cats. Learn the warning signs, how to get your cat to safety, and what veterinary evaluation and treatment may involve.

Carbon monoxide (CO) exposure is an emergency for cats. If you suspect a leak or exposure, get yourself and your cat into fresh air without entering or re-entering a hazardous area, then seek urgent veterinary care. Do not wait for symptoms or try to treat your cat at home.

How cats are exposed to carbon monoxide

CO forms when organic materials burn incompletely. Fires are one source, but a visible flame is not required: ordinary domestic gas appliances can also release CO. Once inhaled, CO binds to hemoglobin, reducing the blood’s ability to carry oxygen and depriving tissues of it. Merck Veterinary Manual describes this mechanism in animals.

A cat rescued from a fire may have more than CO exposure. Smoke can also cause heat injury, airway burns, and exposure to particulates, cyanide, or other gases.

Symptoms of carbon monoxide or smoke exposure in cats

Signs can affect breathing, the eyes, skin, and nervous system. Watch for:

  • Weakness, poor coordination, seizures, or loss of consciousness

Cherry-red gums have been described in cats with CO inhalation, but this is not a reliable sign to wait for. Any suspected exposure, breathing difficulty, or neurologic change warrants emergency care. A cat may also have significant inhalation injury without visible external burns, and airway problems can worsen after the initial event. VCA’s cat-focused guidance describes common signs; Merck Veterinary Manual notes that injury may evolve over time.

What to do if your cat may have been exposed

  1. Get to fresh air safely. Leave the building with your cat only if you can do so without putting yourself at risk. Do not re-enter to search for the source or retrieve belongings.
  2. Call emergency services if a CO alarm sounds or a leak is suspected. The U.S. Consumer Product Safety Commission advises people to move outside and not re-enter until responders say it is safe. Follow their instructions.
  3. Contact an emergency veterinarian. Report the suspected CO or smoke exposure, when and where it happened, and any symptoms. A cat that appears normal still needs prompt veterinary advice after a credible exposure.

This is not a situation for home remedies or owner-administered oxygen. Focus on getting out safely and arranging professional care. CPSC’s carbon monoxide guidance covers the immediate response to an alarm.

What a veterinarian may check

The veterinary team will first assess airway, breathing, and circulation, then consider the exposure history and possible burns, respiratory distress, neurologic effects, or other concurrent injuries. Depending on the cat’s condition, evaluation may include blood tests, chest radiographs, an ECG, and oxygenation assessment. These tests help assess smoke-inhalation injury; no single normal result necessarily rules out evolving harm.

Why a routine oxygen reading may mislead

Standard pulse oximetry can overestimate oxygen saturation when carboxyhemoglobin is present. Pulse CO-oximetry, when available, is more useful for distinguishing CO-bound hemoglobin. A PaO₂ measurement alone can also be misleading: it reflects oxygen dissolved in the blood, not the blood’s overall oxygen-carrying capacity.

Why monitoring may continue

Smoke-related airway compromise can peak 12–24 hours after the initial injury, and lung changes on radiographs may appear later. An initially normal chest X-ray does not exclude significant inhalation injury. The veterinarian may recommend continued observation based on the exposure and examination. Merck Veterinary Manual describes these diagnostic limitations and delayed effects.

How carbon monoxide poisoning is treated

For suspected CO poisoning, the mainstay of treatment is 100% oxygen administered by veterinary staff. The team also works to keep the airway open and may consider intubation if obstruction or reduced consciousness threatens breathing. Respiratory support and other supportive care depend on the cat’s findings. If smoke exposure has caused burns or other lung injury, those problems also need assessment and treatment.

Hospitalization and monitoring, IV fluids when indicated, and pain management for burns may be part of care. The appropriate plan and duration depend on the individual cat. Do not attempt to set up oxygen or give an antidote at home in place of veterinary stabilization. Merck Veterinary Manual and VCA describe oxygen-based and supportive treatment.

Prognosis and what is known about outcomes

Outcome depends on the severity of CO exposure and any accompanying injuries. Merck Veterinary Manual notes that severe burns combined with severe CO poisoning generally carry a poor outcome. The available sources do not establish a feline-specific survival rate for isolated CO poisoning, so a general smoke-inhalation statistic should not be used to predict an individual cat’s chances.

Reduce the risk at home

The CPSC recommends CO alarms on every level of the home and outside sleeping areas. Choose battery-operated alarms or models with battery backup, and look for alarms meeting the current UL 2034 standard. Install and replace alarms according to the manufacturer’s instructions and local requirements. Alarms do not replace proper use and maintenance of fuel-burning appliances: as the CPSC explains, “A CO alarm can provide some added protection, but it is no substitute for proper use and upkeep of appliances that can produce CO.”
