It can be harmless, reflect a heart problem, or result from another condition such as hyperthyroidism. The sound alone cannot tell you which cause applies or how long your cat will live. A veterinary exam, and often an echocardiogram, helps establish what is happening and whether treatment is needed.
What does a heart murmur in a cat mean?
A murmur is a sound made by turbulent blood flow in or near the heart. A veterinarian may hear one during a physical examination, but hearing a murmur does not confirm a particular disease. Some murmurs are physiologic and do not require treatment, although a veterinarian may recommend monitoring. Others are associated with structural heart disease or a condition outside the heart. VCA Animal Hospitals identifies hyperthyroidism as a common secondary cause in cats.

Hypertrophic cardiomyopathy (HCM), the most common feline cardiomyopathy, involves thickening of the heart’s left ventricular muscle. Cornell Feline Health Center reports that cardiomyopathy affects up to 15% of the feline population. That figure describes the population, not the chance that a particular cat with a murmur has HCM.
A murmur is neither a dependable confirmation nor an exclusion test for HCM. According to the Merck Veterinary Manual, at least one-third of cats with HCM do not have a murmur. A normal-sounding heartbeat therefore does not rule out heart disease.

What are the symptoms of a heart murmur in cats?
A murmur itself is something a veterinarian hears; it may not cause visible symptoms. Cats with significant heart disease or an underlying condition can appear well, especially early on, and signs may be subtle until disease advances. Possible signs include:
- Difficulty or increased effort breathing
- Lethargy, weakness, or reduced activity
- Poor appetite or weight loss
- Pale gums
An elevated sleeping respiratory rate can be associated with fluid in the lungs or chest cavity, according to the Merck Veterinary Manual. There is no single home measurement in the sources cited here that should be treated as a universal cutoff. If your cat is struggling to breathe, seek urgent veterinary care rather than waiting to monitor breathing at home.

How is a heart murmur in a cat diagnosed?
The veterinarian uses the cat’s history and physical examination to decide which tests are appropriate. The goal is to determine whether the murmur is physiologic, comes from structural heart disease, or is related to another condition—and to check for complications that affect care.

An echocardiogram is an ultrasound examination of the heart. With Doppler, it can show heart structure and motion and assess the speed and direction of blood flow. VCA describes it as the most useful test for locating turbulence that causes a murmur; Cornell calls echocardiography the gold standard for diagnosing feline cardiomyopathy. It is the principal test for characterizing a suspected cardiomyopathy, but the veterinarian will determine whether and when it is needed.

Other tests
Depending on the examination and suspected cause, the veterinarian may recommend chest radiographs, an electrocardiogram (ECG), or blood tests. Blood pressure and thyroid testing may be relevant: hypertension and hyperthyroidism can cause heart thickening that resembles HCM. NT-proBNP may help screen for heart disease when echocardiography is not immediately available, but Cornell cautions that it cannot definitively diagnose cardiomyopathy. Test selection should be based on the individual cat, not on the murmur grade alone.
How is a heart murmur in a cat treated?
Treatment depends on what is causing the sound and whether the cat has symptoms or complications. A physiologic murmur does not itself need treatment, though a veterinarian may advise follow-up monitoring. If tests identify another disorder, care may focus on that condition. Heart disease management can involve medication, a specialized diet, or supportive care, depending on the diagnosis.

For cardiomyopathy, the plan can depend on findings such as left atrial enlargement and whether congestive heart failure (CHF) is present. In appropriate cases, veterinary care may include diuretics, oxygen, and other cardiac drugs. These treatments are not appropriate for every murmur and should only be selected by the veterinarian managing the cat.

How long can a cat live with a heart murmur?
There is no reliable life-expectancy estimate for “a cat with a murmur” because the sound can have different causes, and the outlook depends on the specific disease and its stage. Some murmurs associated with treatable conditions may resolve; other causes carry a more serious prognosis. A murmur alone cannot predict an individual cat’s lifespan.
For context, Cornell reports that many cats with HCM live normal or near-normal lifespans, and some may go years without needing therapy. Prognosis is worse when complications such as CHF or thromboembolism develop. Its published group estimates are specific to diagnoses and stages:

These figures describe groups of cats, not a countdown for an individual. Ask the treating veterinarian how the confirmed diagnosis, imaging findings, symptoms, and response to care affect your cat’s outlook.
