Other illness, pain, stress, or medication effects can also reduce appetite. Licking alone does not identify the cause, and food interest does not rule out illness. Contact a veterinarian promptly if your cat is eating less, refusing food, or seems uncomfortable while eating.
When should you contact a veterinarian?
Call your veterinarian promptly about a noticeable drop in food intake or apparent pain while eating. VCA Urgent Care lists more than 12 hours without food as a reason to seek urgent care, and advises that a diabetic animal refusing even one meal should be seen. VCA’s general guidance says to seek immediate veterinary attention if a cat has not eaten properly for 24 hours. These are guidance thresholds, not a guarantee that waiting is safe; contact a veterinarian sooner if your cat seems unwell or has other concerning signs.

Be especially cautious if your cat is diabetic, overweight, very young, or older. Cats that eat too little can develop serious complications, including hepatic lipidosis; overweight cats are at greater risk. Early veterinary advice matters rather than waiting to see whether the behavior passes. VCA Urgent Care: Loss of Appetite; VCA: My Cat Won’t Eat.

Why might a cat lick food and then refuse it?
Approaching, sniffing, or licking food and then walking away is a sign to investigate, not a diagnosis. A cat may seem interested but feel sick, or may want food yet find eating uncomfortable or physically difficult. International Cat Care describes loss of appetite as a possible sign of underlying illness or pain that needs veterinary attention. International Cat Care: Managing the cat that won’t eat.
Nausea or feeling unwell
A nauseated cat may approach food and then turn away. Lip-smacking, repeated or exaggerated swallowing, and drooling can accompany nausea, with or without vomiting. These signs can help describe what you see, but licking food does not prove nausea.

Dental, mouth, or swallowing pain
Tooth or gum disease, mouth inflammation, jaw pain, or difficulty swallowing can make picking up, chewing, or swallowing food uncomfortable. Watch for dropping food, chewing on one side, turning the head while chewing, drooling, or irritability. A mouth that looks normal at home does not rule out dental disease, since problems may be hidden below the gumline.
Cornell University College of Veterinary Medicine estimates that 50–90% of cats older than four have some form of dental disease, and that 30–70% show signs of tooth resorption. Those broad prevalence estimates do not establish the cause of an individual cat’s eating behavior. Cornell notes that tooth resorption can be painful and may make cats reluctant or unwilling to eat. Illness, pain elsewhere, or difficulty reaching the bowl
Kidney, liver, pancreatic, gastrointestinal, infectious, and other illnesses can reduce appetite. Pain elsewhere in the body, including arthritis, may make eating or reaching a dish harder. Dehydration or an illness that affects smell may also interfere with eating.
Stress, changes, or medication effects
A change in the home or feeding environment, stress, or a medication effect can reduce food intake. These possibilities do not make a persistent change mere pickiness: the same behavior can have medical causes, so discuss ongoing reduced intake with a veterinarian.

What will the veterinarian check?
The veterinarian will ask about the eating change and may distinguish between a cat that seems hungry but cannot eat comfortably and one that has lost interest in food. They will take a history and examine your cat. Depending on the findings, the work-up may include an oral examination, dental X-rays, blood tests, urinalysis, fecal testing, or imaging such as X-rays or ultrasound. Not every cat needs every test; the choice depends on the individual case.
