Purring alone does not tell you whether a cat is content or hurting, so look for a change from your cat’s usual behavior and check for other signs. If unusual purring comes with changes in movement, appetite, social behavior, or litter-box habits, contact a veterinarian for advice.
Why a cat may purr when they are in pain
Purring happens in more than one situation. VCA Animal Hospitals and Cats Protection both list purring at unexpected times as a possible sign of pain; neither says that purring by itself proves a cat is unwell. Cats Protection’s 2025 behaviour guide notes that purring linked with contentment and purring linked with pain can sound very similar. The sound alone cannot distinguish them.

Instead, consider the setting and what else has changed. A purr during a familiar, relaxed interaction may fit your cat’s normal pattern. A new or unusually frequent purr alongside withdrawal, reduced movement, or another change deserves closer attention. These observations can guide a conversation with a veterinary team, but they cannot identify the cause at home.
What other changes should you look for?
Cats may show pain through subtle changes rather than crying out or limping. Compare your cat’s current behavior with their normal routine, paying attention to several areas:

- Movement and posture: reluctance to jump or climb, hesitation when moving, reduced activity, or a hunched posture.
- Social behavior and handling: hiding, becoming withdrawn, restlessness, or reacting differently to being touched or picked up.
- Daily habits: changes in sleep, appetite, drinking, grooming, or litter-box behavior.
These changes can have causes other than pain, and no single sign confirms a diagnosis. VCA advises contacting a veterinarian when increased purring occurs with other behavior changes, such as hiding, altered energy, or reluctance to jump onto furniture or climb stairs. Cats Protection also recommends a veterinary health check when behavior changes or pain signs appear.
When to seek veterinary care
Contact your veterinarian about a new pattern
Call your veterinary practice if purring is new or unusual for your cat, especially when it appears with any of the changes above. Tell the team what you have noticed and when it began—for example, whether your cat is eating differently, avoiding a jump they used to make, or spending more time hiding. The change from your individual cat’s normal routine is useful context.

Straining to urinate is an emergency
Repeated attempts to urinate that produce little or no urine, painful straining, or increasing distress may indicate a urethral obstruction. Cornell University’s Feline Health Center describes suspected obstruction as an emergency requiring immediate veterinary treatment. Do not wait to see whether it passes; seek veterinary care immediately.

Describe your observations to the veterinary team and follow their advice about assessment and treatment. Do not give your cat an over-the-counter nonsteroidal anti-inflammatory drug (NSAID): VCA warns that some can be very dangerous for cats. Ask a veterinarian which pain relief, if any, is appropriate.
