Cat Urinary Blockage: Five Possible Phases and When to Act

Repeated straining, frequent litter-box trips, or little to no urine can signal a life-threatening blockage. The five phases are educational, not a timeline—suspected obstruction needs immediate veterinary care.

If your cat is repeatedly straining to urinate, making frequent litter-box trips that produce little or no urine, seek emergency veterinary care now. A urethral blockage can become life-threatening. The “five stages” below are an educational way to describe possible worsening signs—not a recognized veterinary staging system or a timetable. Signs can overlap, and you should not wait for them to progress.

What the five phases mean—and what they do not

Veterinary references describe symptoms and complications of urinary obstruction, not a standard five-stage scale. The phases below organize those possible signs for clarity; they cannot confirm a blockage, predict what will happen next, or show how long a particular cat has.

Urinary signs can have causes other than obstruction, and they may look similar. Only a veterinarian can assess whether a cat is blocked. Because suspected obstruction is an emergency, repeated unproductive straining or passing only drops warrants immediate care.

Five possible phases of a urinary blockage

1. Early lower urinary signs

A cat may make frequent attempts to urinate, strain or appear uncomfortable, vocalize, pass blood in the urine, or urinate outside the litter box. These signs can occur with several lower urinary tract problems; they do not, by themselves, prove that the urethra is blocked.

2. Repeated straining with little or no urine

Repeated litter-box visits that produce only drops or no visible urine are a major warning pattern. Caregivers sometimes mistake straining to urinate for constipation. Do not wait to see whether another attempt produces more urine. Male cats are at greater risk of obstruction, but suspected blockage in any cat needs urgent veterinary assessment.

3. Increasing pain and distress

With an obstruction, a cat may become more uncomfortable or distressed and may vocalize. A veterinarian may find a painful, distended bladder on examination. Do not press or squeeze your cat’s abdomen to check the bladder or try to relieve the problem.

4. Signs of systemic illness

When urine cannot pass, waste removal and fluid and electrolyte balance are disrupted. A cat may become depressed, lose its appetite, vomit, or become dehydrated. Dangerous electrolyte changes can also develop. These signs indicate serious illness, but their absence does not make suspected obstruction safe to monitor at home.

5. Life-threatening complications

Severe metabolic abnormalities, including high blood potassium (hyperkalemia) and acidosis, can affect the heart and other organs. Untreated complete obstruction can progress to coma and death. This is not a phase to wait for: seek veterinary care as soon as obstruction is suspected.

Why obstruction is dangerous, and why there is no safe countdown

A complete blockage prevents normal urine flow and disrupts kidney filtration and the body’s fluid and electrolyte balance. Deterioration can be rapid, but the course varies and the five-part description is not a clock.

Cornell Feline Health Center warns that death after complete obstruction may occur in less than 24–48 hours; Merck Veterinary Manual reports that uremia can develop within 36–48 hours and death may occur within approximately 72 hours. These are warnings from different veterinary references, not an individual prognosis or a safe waiting period. Do not use them to decide how long to wait.

What to do if you suspect a blockage

  1. Call an emergency veterinary clinic and arrange immediate care. Tell them your cat is straining to urinate and whether any urine is coming out. If your regular veterinarian is unavailable, contact an emergency clinic.
  2. Transport your cat promptly. Keep the cat as calm as practical. Do not delay to try home remedies or to see whether symptoms improve.
  3. Do not try to unblock the cat at home. Do not press the abdomen, attempt to express the bladder, or give medication or urinary products unless a veterinarian directs you.

How veterinarians diagnose and treat an obstruction

The veterinary team assesses the cat’s condition and may perform a physical examination and urinalysis. Depending on findings, evaluation can also include blood work, radiographs, ultrasound, or urine culture. The tests help assess the obstruction and its effects and may help identify a cause.

Treatment focuses on stabilizing the cat and restoring urine flow. Relief commonly involves a urinary catheter under sedation or anesthesia; Merck emphasizes general anesthesia whenever possible for relief in male cats. Intravenous fluids may be used to address dehydration and electrolyte imbalance. The need for hospitalization, monitoring, and follow-up depends on the obstruction’s severity and duration and the cat’s condition.

Aftercare and reducing recurrence risk

Prevention depends on the diagnosed cause and the cat’s history. Plugs or inflammation and a particular type of urinary stone may call for different plans. Ask the veterinarian what the evaluation found, what changes are appropriate, and what signs should prompt another urgent visit.

Diet decisions should be cause-specific. Cornell cautions against using urinary acidifiers without veterinary guidance; a diet suited to one diagnosis may not be appropriate for another. The 2025 iCatCare consensus guidelines note that wet food and increased water intake may help prevent recurrence in some contexts, but studies are not conclusive. A fountain is one optional way to encourage drinking, not a treatment for an active blockage. Follow the veterinarian’s advice rather than starting a diet or supplement on your own.
