Pyometra is an infected, pus-filled uterus, and it can become life-threatening. It mostly affects unspayed female cats. If an unspayed cat is unwell, especially if she is drinking more, off her food, lethargic, or has a swollen or painful belly, call a veterinarian or emergency clinic right away. Do not wait for discharge to appear, because some cats with pyometra never show any.
What pyometra is and why it happens
Pyometra is a bacterial infection inside the uterus. It typically develops in an intact (unspayed) female after repeated reproductive hormone cycles have changed the uterine lining. The Merck Veterinary Manual describes it as part of the cystic endometrial hyperplasia–pyometra complex in small animals. In practical terms, the lining thickens under hormonal influence, and bacteria then take hold.
Household cleanliness, a particular food, or a single exposure does not cause it. The key preventable risk identified in veterinary owner guidance is simply that the cat has not been spayed.
Open versus closed cervix
- Open cervix: pus can drain out, so you may see discharge from the vulva, often on the fur under the tail or on bedding.
- Closed cervix: infected material stays inside the uterus. There may be no discharge at all, and the cat can become severely ill quickly. VCA Animal Hospitals stresses that a lack of discharge should not be taken as reassurance.
Signs of pyometra in cats
Presentation varies, and the signs overlap with other serious illnesses, such as kidney disease, urinary problems, or pregnancy-related conditions. Veterinary sources list:

Pale gums, weakness, or collapse are severe warning signs and call for immediate emergency care.
Is pyometra an emergency?
Yes. The American College of Veterinary Surgeons states: “Pyometra is a medical emergency that requires rapid intervention to prevent overwhelming infection and death.” It cannot be diagnosed or treated at home. If you suspect it, contact your vet or an emergency service the same day, and mention that your cat is unspayed, when her last heat was, and any discharge you have seen.

How vets diagnose it
Diagnosis combines the cat’s reproductive history, a physical exam, and tests chosen for her individual situation:
- Blood work: can reveal systemic changes caused by infection.
- Urine testing: helps assess hydration and effects on the kidneys.
- Abdominal X-rays: can show an enlarged uterus, especially with a closed cervix. According to VCA’s cat-specific guidance, a normal-looking X-ray does not rule out an open-cervix case.
- Ultrasound: can show a fluid-filled uterus and help tell it apart from pregnancy.
No single symptom or home observation confirms pyometra; the vet must also rule out pregnancy and other abdominal or reproductive disease.

Treatment
Surgery: the usual definitive treatment
Treatment is normally an ovariohysterectomy, surgical removal of the ovaries and the infected uterus. Because the infection can cause dehydration and illness throughout the body, the vet may first stabilize the cat with IV fluids and give antibiotics before and after the operation as needed. The surgery can be more complicated than a routine spay because the uterus may be enlarged and fragile.
Medical management: a narrow exception
For selected breeding cats, a veterinarian may consider hormone-based medication plus antibiotics. This approach takes time to work, can fail, and carries a risk of recurrence. It is a specialist decision, not a home-care option and not a reason to delay an examination.

Prognosis
Untreated pyometra can be fatal. Outcomes are generally better with prompt diagnosis and treatment, but they depend on the cat’s condition and any complications. ACVS reports survival “as high as 80–100%” after ovariohysterectomy when abdominal contamination is avoided and the whole-body infection responds. That guidance covers dogs and cats together, so it is not a cat-only statistic. No cat-specific figure suitable as a general estimate was identified in the veterinary sources reviewed.

Spaying removes the uterus and ovaries and prevents ordinary uterine pyometra. If your cat is healthy and unspayed, ask your vet about the best timing. A cat who is already ill needs assessment first, not a routine scheduling conversation.

Recovery after surgery
- Follow your clinic’s instructions for medication, activity, and incision care.
- ACVS advises restricting vigorous activity for the first two weeks.
- Protect the incision from licking or chewing. A recovery cone or protective collar may help if your vet recommends one and confirms the fit.
A collar protects the wound; it does not treat pyometra. Likewise, supplements, leftover antibiotics, and home remedies are not treatments.
Frequently Asked Questions
Can a cat have pyometra without discharge?
Yes. With a closed cervix, pus stays inside the uterus, so nothing may be visible even though the cat can be seriously ill.
Can pyometra be treated without surgery?
Only in selected cases, usually breeding cats, under veterinary direction with hormone-based medication and antibiotics. It can fail or recur, so surgery remains the usual treatment.
Can a spayed cat get pyometra?
Spaying prevents ordinary uterine pyometra. A spayed cat with similar symptoms still needs a veterinary exam, since other illnesses can look alike.
