Ascites is fluid buildup in a cat’s abdomen, but a swollen belly alone cannot confirm it. The swelling can also come from enlarged organs, a mass, pregnancy, obesity, or parasites. A veterinarian needs to determine whether fluid is present and identify its cause, because treatment and outlook depend on that diagnosis.
What ascites means—and what a swollen belly can indicate
Ascites is a clinical sign, not a disease on its own. The fluid may be blood, urine, or an effusion linked to changes in pressure, low blood protein, inflammation, infection, or another illness. A veterinarian must assess the cat to distinguish free abdominal fluid from other causes of distension. Merck Veterinary Manual defines ascites as abdominal fluid accumulation; VCA Animal Hospitals also describes other causes of abdominal enlargement.

Signs to watch for and when to seek care
A cat with ascites may have a rounder or enlarged abdomen, low energy, reduced appetite, or discomfort. Fluid can press against the diaphragm and make breathing difficult. If your cat has abdominal swelling and trouble breathing, seek veterinary evaluation promptly. Sudden or progressive swelling also warrants contacting a veterinarian. Do not try to drain the abdomen or give human medication at home. Cummings School of Veterinary Medicine at Tufts University advises veterinary assessment for abdominal swelling.
Possible causes and how veterinarians narrow them down
Important causes in cats include congestive heart failure, cancer (neoplasia), liver disease, and feline infectious peritonitis (FIP). Trauma, urinary leakage, and peritonitis are also possible. These conditions can produce fluid through different mechanisms, so the appearance of the belly cannot identify the cause.

What to expect during diagnosis
The veterinarian will take a history and examine your cat. Depending on the findings, the workup may include imaging, blood and urine tests, disease-directed testing, and sampling abdominal fluid. The specific tests are tailored to the individual cat.

- Blood and urine tests: A complete blood count, serum chemistry, and urinalysis may help assess organ function and other clues.
- Fluid analysis: A veterinarian may evaluate a sample for cell counts, protein, specific gravity, and microscopic findings. The results can help narrow the possible causes.

Treatment and outlook depend on the diagnosis
Care is directed at the underlying cause. Depending on the diagnosis, treatment may involve condition-specific medication, surgery, cancer treatment, antibiotics for bacterial infection, or supportive care. A veterinarian may drain fluid when clinically indicated to relieve pressure or discomfort; drainage alone does not resolve the disease causing the fluid.

Prognosis and the chance of recurrence vary with the cause and its severity. Merck Veterinary Manual notes that ascites associated with liver disease and severe hypoalbuminemia generally carries a poor long-term prognosis; that assessment should not be applied to every cat with ascites.
