Chlorambucil is an oral anticancer and immunosuppressive drug. Veterinarians use it in cats off label, meaning the use has not been reviewed or approved by the FDA or Health Canada for that veterinary purpose. In cats it is most often discussed for low-grade, small-cell gastrointestinal lymphoma, usually alongside oral prednisolone. The veterinarian chooses the dose, schedule, and monitoring plan for each cat, and any change to those belongs to the treating team.
What chlorambucil is and why it is used off label
VCA Animal Hospitals describes chlorambucil as an immunosuppressive and anticancer medication. Its potential veterinary uses include selected cancers and some immune-mediated diseases. Because feline use is off label, the prescribing veterinarian is working from clinical judgment and published protocols rather than a label written for cats. VCA notes that off-label directions can differ from product labeling, so the instructions your veterinary team gives you are the ones to follow.
When veterinarians consider it for cats
Low-grade, small-cell gastrointestinal lymphoma
The best-documented feline use in the sources reviewed is low-grade lymphoma that involves the gastrointestinal tract. North Carolina State University Veterinary Hospital describes treatment with oral chlorambucil given at home together with oral prednisolone. Low-grade lymphoma is often managed as a long-term condition rather than a short course, and NC State notes that treatment can be chronic, lasting years or lifelong.
Why diagnosis comes first
Many cats with chronic vomiting, diarrhea, or weight loss have inflammatory bowel disease rather than lymphoma, and the two can look alike at home. Telling them apart can require cytology or biopsy plus additional testing. Staging and the grade of the lymphoma shape which treatment is chosen, which is why a cat may be evaluated thoroughly before any chemotherapy starts. If your veterinarian is still working toward a diagnosis, that step matters as much as the drug itself.

Dosage and schedule: what published examples show
There is no universal dose of chlorambucil for cats. Dosing depends on the disease, the protocol being followed, the cat’s own health, the formulation dispensed, and laboratory results. The examples below illustrate how protocols are built. They are not instructions for any individual cat.
- NC State describes one small-cell lymphoma protocol that gives oral chlorambucil every other day at home.
- VCA emphasizes careful dosing and says a missed dose should prompt a call to the veterinarian rather than a self-directed adjustment.
Giving the medication at home
Give only the formulation and amount your veterinarian prescribed. Do not change the interval, do not stop treatment without instructions from the veterinary team, and do not split or crush a dosage form unless the team has told you that the specific product can be handled that way. The sources reviewed do not establish one administration method that applies to every formulation, so the product in your home is the deciding factor.
If a dose is missed
VCA specifically advises contacting the veterinarian for instructions after a missed dose. Do not double the next dose to make up for it. Keep a simple written log of when each dose was given and any dose that was skipped, because the team will ask.

Blood monitoring
Bloodwork is the core safety tool for this drug, because chlorambucil can suppress bone marrow. The table compares what each source states. Where a source does not give a figure, the cell says so.

Keep every recheck appointment, even when your cat seems comfortable. The risk that matters most here is not always visible in day-to-day behavior.

- Signs of infection, such as fever or lethargy with no clear cause.
- Bruising or unusual bleeding.
Serious signs that need immediate action
VCA lists abnormal bleeding, bruising, infection, appetite loss, low energy, vomiting, diarrhea, breathing difficulty, agitation, twitching, and seizures among serious signs. For these, VCA directs owners to stop and contact the veterinarian immediately. If your cat shows any of them, call your clinic or an emergency hospital without waiting for the next scheduled visit.
Delayed low platelet counts
AAHA identifies bone marrow suppression, especially neutropenia, as an important toxicity of chemotherapy in general. It also states that chlorambucil can cause irreversible delayed thrombocytopenia after long-term use. Low platelets can appear without obvious warning, which is why scheduled bloodwork continues even when the cat looks well.

- Known allergy to chlorambucil or to other alkylating medicines.
- Bone marrow disease.
- Active infection.
- Pregnancy or lactation.
Interactions matter too. VCA advises caution with other myelosuppressive medicines and with vaccines, and it advises against vaccination unless the veterinarian recommends it. Give the team a complete list of every prescription drug, over-the-counter product, supplement, and health condition, including ones that seem unrelated.

Chlorambucil is considered a hazardous drug. Ask the veterinary team for handling and disposal instructions before you open the first bottle, and follow the product-specific directions printed by the manufacturer. Store it where children and other pets cannot reach it, and wash your hands after handling tablets or doses.
What the published outcome figures do and do not mean
Two figures are often quoted. They measure different things and apply to different situations, so they should not be combined into a single prediction.

Your cat’s diagnosis, grade, stage, and overall health are what determine the realistic outlook. Ask your veterinarian to explain how those factors apply to your cat specifically.
Treatment goals and quality of life
AAHA’s 2026 oncology guidelines state: “The primary goal of chemotherapy in veterinary oncology is to maintain the best possible quality of life for pets while effectively managing their cancer.” Judge each stage of treatment against that goal. If the drug is controlling the disease but your cat is repeatedly miserable from side effects, bring that to the team so the plan can be reviewed.
