Yes—general anesthesia is recommended for a complete veterinary dental cleaning. It lets the veterinary team examine the whole mouth, take dental X-rays, check beneath the gums, clean hard-to-reach areas, and treat disease found during the procedure. Scaling visible teeth while a dog is awake is not an equivalent substitute: it cannot provide the same examination or care below the gumline.
What a complete dental cleaning involves
A professional dental procedure is more than scraping visible tartar. The veterinarian needs to assess each tooth and the surrounding tissues, including areas hidden below the gumline. AAHA recommends general anesthesia with endotracheal intubation, monitoring, and physiologic support for dental cleaning and scaling. The breathing tube helps protect the airway during the procedure.

With the dog anesthetized, the team can perform a tooth-by-tooth examination, probe around teeth, assess mobility, chart findings, and take dental radiographs. AAHA strongly recommends full-mouth intraoral radiographs for dental patients because disease or other findings may not be apparent from the visible surfaces alone. The results may change which treatment is needed; discuss in advance how the clinic will contact you and obtain consent if the examination reveals a need for additional care, such as an extraction.

Why awake scaling is not the same procedure
Nonanesthetic scaling removes calculus from visible tooth surfaces, but it does not allow the same complete assessment or treatment below the gums. Cleaning those areas can be uncomfortable or painful, and an awake dog cannot reliably remain still for a thorough examination and radiographs. Without general anesthesia, the veterinary team also cannot provide the same pain management or airway protection.
AAHA warns that nonanesthetic dentistry can involve patient stress, injury, and aspiration risk, and lacks the diagnostic capability needed for a complete dental evaluation. Removing visible tartar alone can make teeth look cleaner without addressing disease underneath. General anesthesia, sedation, and limited examinations
Sedation is not routinely interchangeable with general anesthesia for a complete cleaning. AAHA describes heavy sedation as a possible option in selected situations when cleaning, polishing, and extractions are not anticipated—for example, to gather limited baseline information. Sedation-only procedures have airway and monitoring limitations, so the veterinarian should explain what a proposed procedure can and cannot accomplish.
Ask whether the plan is for a complete dental procedure or a limited examination, and what the team expects to be able to assess and treat. AAHA’s sedation guidance discusses these limited uses.

How to think about anesthesia risk for your dog
Anesthesia is not risk-free, and risk is individual. Your veterinarian should consider your dog’s overall health and any comorbidities when planning the procedure. Older age deserves attention, but AAHA says chronological age alone should not rule out anesthesia for a medically important, indicated procedure.
AAHA’s 2020 anesthesia guideline estimates anesthetic-related death risk at 0.05% (1 in 1,849; 95% confidence interval 0.04–0.07%) in healthy dogs. This is a population estimate for healthy dogs, not a prediction for an individual pet; it may not describe a dog with health problems. The guideline also reports that deaths occurred most frequently in the first three postoperative hours. Read the AAHA client education guidance and discuss how your dog’s health affects the plan.

Questions to ask before the procedure
- What pre-anesthetic evaluation do you recommend for my dog, given their health and medical history?
- Who will monitor my dog during anesthesia, and which vital signs will they track?
- How will you protect my dog’s airway, and how will recovery be supervised?
- Will the procedure include a full-mouth examination, periodontal probing, charting, and dental radiographs?
- If the examination or X-rays reveal disease requiring additional treatment, how will you contact me and obtain consent?

Home care does not replace a veterinary dental examination, radiographs when indicated, or treatment for established periodontal disease.
