Most dog ear problems are inflammation of the outer ear canal, called otitis externa. Head shaking, scratching, odor, redness, swelling, discharge, and pain point to a problem in the canal, but they do not reveal why it started. Treatment works only when the underlying cause is found, so an ear that looks better at home may still need veterinary follow-up. Head tilt, loss of balance, or uncontrolled eye movements are a different situation and need prompt veterinary assessment.
What the signs tell you, and what they don’t
Veterinary owner guidance from the Merck Veterinary Manual lists the most common signs of outer ear inflammation in dogs:
- Head shaking
- Scratching at the ear or head
- Foul odor from the ear
- Redness of the ear skin and swelling
- Discharge from the canal
- Scaly skin around the ear
- Pain or sensitivity when the ear is touched
- Itchiness
One ear or both may be affected, and signs can come on suddenly or develop slowly over time (Merck Veterinary Manual, dog-owner guidance on ear infections and otitis externa). Because these signs are shared by many causes, a color or smell alone is not a diagnosis. Owners often assume a yeast infection from a sour smell or a bacterial one from yellow discharge, but the veterinary reference is clear that the discharge cannot reliably separate yeast, mites, bacteria, or a mix of them.
Signs also tell you where the problem is likely to be. Outer ear inflammation and deeper ear disease have different patterns:

Merck’s guidance on middle and inner ear infections notes that some nerve-related effects can remain even after an infection clears, which is why timely evaluation matters (Merck Veterinary Manual, otitis media and interna in dogs).
Common causes of dog ear problems
Ear inflammation usually has a primary trigger, and secondary bacteria or yeast often take advantage once the canal is irritated. Primary causes include allergies, parasites, and foreign objects lodged in the canal. Anatomy and moisture can make a dog more prone to repeated episodes.
Allergies
Allergic skin disease is one of the common primary causes of canal inflammation. The allergy itself is often not visible in the ear, so the ear may be the first place an owner notices a problem. Treating only the ear without addressing the allergy is a frequent reason signs return.
Parasites
Ear parasites can irritate the canal and trigger inflammation. Because mites and other parasites can look similar to other discharge, a veterinarian needs to examine a sample rather than rely on appearance.

Foreign objects
Plant material, debris, or other objects lodged in the canal can cause pain, discharge, and repeated shaking. A foreign object may not be obvious from outside, and a complete examination sometimes requires sedation or anesthesia.
Bacterial and yeast overgrowth
Bacteria and yeast are often secondary. As the canal lining becomes inflamed and the environment changes, these organisms can multiply. Treating the overgrowth without addressing the trigger often leaves the problem unresolved.
Ear shape, moisture, and swimming
Ear shape and canal structure affect airflow and drainage. Excessive moisture, including from swimming and bathing, softens the canal lining and can make infection more likely. Dogs with narrow or heavily folded ear canals may need closer attention to dryness.

- An examination of the dog and the ear canal, usually with an otoscope
- Sampling of discharge for microscopy or culture to check for bacteria, yeast, or parasites
- Sedation or anesthesia when pain or canal blockage prevents a full look
- Further testing or imaging when signs are chronic, affect only one side, or suggest deeper disease
Merck’s dog-owner guidance states that treatment requires finding and correcting underlying causes and relieving pain. The cause, not the discharge, should direct the plan.
Treatment: what a typical plan involves
Treatment is cause-specific. A plan may combine cleaning, topical medication, pain or inflammation control, and antimicrobial treatment where indicated. Medication choice and cleaning method are veterinary decisions, and different products are not interchangeable.
Cleaning
Wax or discharge can block topical medicine from reaching the canal, so the veterinarian may clean the ear before starting treatment. If the ear is very painful, the cleaning may need to be done in clinic under anesthesia.
Medication
Topical medication is chosen according to the diagnosis. Antimicrobial treatment is used where the examination or sampling indicates bacteria or yeast. Owners should not substitute leftover medication or unapproved over-the-counter products.

Pain and inflammation
Controlling pain and swelling is part of treatment, not an afterthought. Pain can make a dog resist examination and medication, so addressing it often helps the rest of the plan work.
Deeper or long-standing disease
When the middle or inner ear is involved, or when the eardrum cannot be evaluated, the plan may include further diagnostics and a different course of care. Veterinarians compare options by the diagnosed cause, the location of the disease, and whether the eardrum can be assessed.
Stopping early is one of the most common mistakes. Merck’s owner guidance says bacterial and yeast infections often need repeated checkups until no infection is seen, and that clearing often takes 2 to 4 weeks. Long-term cases can take months and sometimes need ongoing care. These are duration ranges reported in the guidance, not success rates for any particular treatment.

Merck’s guidance notes that unless all causes are treated, otitis externa often returns. Recurrent external inflammation can also occur alongside middle-ear disease. When signs keep coming back, the next step is usually a fuller look for the cause, such as an allergy workup or a check for parasites, rather than another round of the same treatment. Merck Veterinary Manual, citing its own figures for 2026, also reports that over half of dogs with recurrent external ear inflammation have otitis media.

Home care: what to do and what to avoid
Home care supports veterinary treatment; it does not replace it. Check ears regularly for redness, moisture, odor, or other visible changes. If your dog has recurring problems, ask your veterinarian whether regular cleaning is appropriate and how often to do it.
For cleaning, Merck describes using a veterinarian-recommended cleaning solution with cotton balls. Keep these points in mind:
- Use only the cleaning solution your veterinarian recommends, and use it as directed
- Do not put cotton swabs into the ear canal
- Do not use over-the-counter wax removers unless your veterinarian approves them
- Keep water out of the ears during bathing, and use drying agents only as your veterinarian recommends
Avoid vinegar mixtures and other home remedies. Merck warns that vinegar mixtures can worsen swelling and secretions and increase infection risk. Even products that are normally safe in healthy ears can irritate an inflamed canal, so do not assume a product is harmless because it is common.
When to call the veterinarian
Arrange a veterinary visit promptly if your dog shows any of the following:
- Signs of ear infection, including odor, discharge, redness, or pain
- A change in head position, such as a persistent head tilt
- Loss of balance or trouble rising or walking
- Severe coordination problems
- Uncontrolled eye movements
- Facial weakness
- Signs that continue or return after treatment has been stopped
The middle and inner ear signs in this list warrant a prompt assessment rather than a wait-and-see approach, because they can indicate deeper ear involvement.
