Eyelid agenesis is a birth defect in which part of a cat’s eyelid—usually the outer portion of the upper lid—does not form. The missing tissue can leave the cornea exposed or allow nearby hairs to rub against it, causing irritation and, in some cats, ulcers or scarring. Some cats have mild defects that need monitoring or veterinarian-directed lubrication; others need surgery. An eye examination determines whether the cornea is at risk and which treatment, if any, is appropriate.
What causes eyelid agenesis in cats?
Eyelid agenesis, also called eyelid coloboma, is congenital: the eyelid does not develop fully before birth. The upper eyelid is most often affected, commonly toward its outer side. Some kittens lack a substantial portion of the lid; the American College of Veterinary Ophthalmologists (ACVO) notes that the outermost one to two thirds may be absent in some cases. The defect is typically present in both eyes, though its severity can differ from one eye to the other.
The developmental defect is established, but the sources cited here do not establish a specific inheritance pattern or a precise cause for an individual cat. A particular breed, parent, or exposure therefore should not be assumed to have caused it. Other congenital eye findings can occur alongside agenesis. In some cats, the tear-producing gland and ducts in the affected region may also be underdeveloped, contributing to dry eye.
What does eyelid agenesis look like?
The eyelid margin may have a visible gap or missing segment, often along the upper outer lid. In a typical lateral defect, the conjunctiva—the moist tissue lining the eyelid and covering the white of the eye—may meet the skin where the lid margin should be. The appearance alone does not show how well the eye is protected: incomplete blinking and hairs near the defect can affect the cornea, the clear surface at the front of the eye.

A veterinarian may assess the eyelid, blinking, tear film, and cornea. There is no validated home screening test in the cited guidance. Arrange an examination if you suspect a missing eyelid segment, even if your cat seems comfortable; seek prompt veterinary advice for squinting, redness, discharge, cloudiness, apparent pain, or hair visibly rubbing the eye.
Can eyelid agenesis damage my cat’s eye?
Yes. A normal eyelid helps spread tears and shield the cornea. When part of the lid is absent, the eye may not close completely, and periocular hairs can contact the corneal surface. These problems can cause irritation or inflammation (keratitis), abrasions, and corneal ulcers. Longer-term or more serious damage can include blood vessels growing into the cornea, pigmentation, scarring, infection, and impaired vision.

Severity varies. ACVO says some kittens appear minimally affected and may not need treatment, while significant pain or ocular-surface damage can require intervention. Merck Veterinary Manual likewise warns that an exposed eye is at risk of inflammation and ulcers. These points are not contradictory: the defect’s name alone does not determine urgency or treatment; the individual cat’s examination findings do.
Does every cat need surgery?
No. A veterinarian may monitor a minimally affected cat or recommend lubrication for comfort and surface protection. Surgery is more likely to be considered when hairs repeatedly irritate the eye, blinking does not protect it adequately, or corneal changes have developed. A veterinary ophthalmologist is particularly useful when there is eye damage or reconstruction is being considered.

The decision is individualized. Relevant factors include the defect’s size and location, existing corneal injury and tear-film problems, the likelihood of repeat treatment or complications, specialist availability and cost, and the owner’s expectations for function and appearance.

Lubrication and monitoring
A veterinarian may recommend artificial-tear gel or ointment for selected cats. Lubrication can improve comfort or provide temporary protection from exposure and hair contact, but it does not replace the missing eyelid. MSPCA-Angell notes that some cats may be irritated by an ointment’s base. Use an eye product only under veterinary direction; do not substitute human eye products or change medication without asking the veterinarian.

Veterinary hair-control procedures
For some small defects or limited circumstances, a veterinarian may manually remove irritating hairs (epilation); regrowth can mean the procedure must be repeated. Cryotherapy is another veterinary option for destroying hairs, but excessive freezing can cause full-thickness tissue damage. These procedures require professional assessment and corneal protection; they are not home-care techniques.
Eyelid reconstruction
When the eye needs better protection or ongoing irritation has damaged the surface, surgery may remove the source of hair contact or create a functional eyelid margin. Reported approaches include lower-lid transposition, skin flaps, and grafts. No one technique fits every defect: the surgeon selects an approach based on anatomy, corneal changes, expected function, and practical considerations. Possible problems vary by procedure and can include persistent hair contact or graft-related complications.

Published outcomes illustrate why results should not be treated as guarantees. A 2021 study of a hyaluronic-acid-assisted soft-tissue expansion followed by a free labial mucocutaneous graft included 24 cats and 39 eyelids with secondary keratitis. The authors reported marked resolution of keratitis and trichiasis (hair contact with the eye) in 27 of 39 eyelids (69.2%) after one procedure, and complications in 6 of 39 (15.4%). Those figures describe that study cohort and technique, not the expected result for every cat or surgery.
A 2023 case report described one cat with about 50% bilateral temporal upper-eyelid coloboma treated with Roberts-Bistner blepharoplasty. Trichiasis was noted in both eyes a few weeks after surgery, although the report described adequate eyelid function and cosmetic outcome in that individual case. A single case does not establish that this procedure is best for other cats.
What should I ask the veterinarian?
- Is the cornea already irritated, ulcerated, scarred, or otherwise damaged?
- Are hairs contacting the eye, and is the tear film adequate?
- Could lubrication or monitoring be sufficient, or is a procedure recommended?
- If surgery is proposed, what technique fits this defect, what outcome is realistic, and what complications or repeat procedures are possible?
- Would referral to a veterinary ophthalmologist help with diagnosis or treatment planning?
These questions help turn a visible eyelid defect into a practical plan based on the eye’s condition, rather than its appearance alone.
