Feline aortic thromboembolism (FATE), often called a saddle thrombus, is a painful emergency. If your cat suddenly cannot use one or both back legs, cries out, or has cold, pale, blue-tinged, or pulseless paws, contact an emergency veterinarian and go in now. Do not wait to see whether the signs improve or try medication at home. Trouble breathing may mean heart failure is also present.

What happens in feline aortic thromboembolism?

FATE occurs when a blood clot, usually formed in the left side of the heart, travels through the arteries and blocks blood flow. It often lodges where the aorta branches toward the hind limbs, which is why the condition is also called a saddle thrombus. Reduced blood supply can injure muscles and nerves, causing sudden pain, weakness, or paralysis.

The blockage may affect both back legs or one limb more than the other. The severity and extent of injury vary; a cat’s signs alone cannot establish the size or location of the clot.

What causes FATE in cats?

Cardiomyopathy is the leading underlying cause

Cardiomyopathy—disease of the heart muscle—is the dominant known association. A 2024 review in the Journal of Feline Medicine and Surgery says cardiomyopathy accounts for about 90% of cases in the reports it cites. FATE can be the first sign that a cat has heart disease, so an earlier absence of a diagnosis does not rule it out.

Other reported associations

Neoplasia is the next most commonly described non-cardiac cause, with pulmonary neoplasia prominent in that category. Infection, inflammatory disease, hyperthyroidism, and exposure to corticosteroids or progesterone agonists have also been reported in association with FATE. These associations do not prove that a particular condition or medication caused an individual cat’s clot.

Heart failure can occur at the same time as FATE. The veterinary team needs to assess the cat’s heart and breathing as well as the affected limbs.

What are the signs of a saddle thrombus?

Signs often begin abruptly. Watch for:

  • Sudden weakness, dragging, or paralysis in one or both hind legs.
  • Marked pain, agitation, or crying out, especially when the affected area is touched or the cat tries to move.
  • Back paws or legs that feel unusually cold.
  • Pale or blue-tinged paw pads.
  • Weak or absent pulses in the hind limbs; checking this reliably is a veterinary skill.
  • Rapid or difficult breathing, which can indicate concurrent heart failure.

If your cat suddenly can’t use its back legs or has cold back paws, treat it as an emergency rather than trying to confirm a blood clot yourself. These signs need prompt veterinary assessment.

How veterinarians diagnose FATE

A veterinarian may recognize the syndrome from the sudden history and physical examination, including limb temperature, color, movement, pain, and pulses. Doppler ultrasound or point-of-care ultrasound can help assess blood flow; thermal imaging may be used in some settings.

Assessment also considers whether cardiomyopathy or congestive heart failure is present and whether the cat has complications such as muscle injury or acute kidney injury. The specific tests depend on the examination and the cat’s stability. No home check can confirm FATE or rule out heart failure.

How FATE is treated

Treatment takes place in a veterinary hospital and is tailored to the cat’s condition, how extensively the limbs are affected, concurrent illness, and the owner’s discussion with the veterinary team. Care may involve pain relief and stabilization, evaluation and treatment of heart failure, and antithrombotic treatment intended to limit clot growth or further embolization. Thrombolysis—treatment aimed at dissolving a clot—is an evolving option in selected contexts, not a universal protocol.

Care approach Clinical goal What to know
Analgesia and stabilization Relieve pain and support the cat’s immediate condition. Part of hospital care; the treating team selects and monitors treatment for the individual cat.
Assessment and treatment of heart failure Address concurrent cardiopulmonary disease. Heart failure can coexist with FATE, so it must be considered alongside the limb emergency.
Antithrombotic treatment Limit clot growth or reduce the risk of further embolization. The veterinary team determines the approach and follow-up; no single regimen is established as best for every cat.
Thrombolytic treatment Attempt to dissolve the clot. An evolving option. The 2024 review describes recent advances but notes limited prospective evidence; its use is case-specific.

The available clinical literature does not establish one treatment protocol as best for every cat, and treatment cannot guarantee survival or restored limb function. Ask the veterinarian to explain the aims, uncertainties, monitoring, expected hospitalization and follow-up, risks, and likely costs of the options for your cat. Do not give aspirin, anticoagulants, human painkillers, or other medication unless the treating veterinarian specifically directs you to.

What is the prognosis for a cat with FATE?

The prognosis is guarded and depends on factors such as the extent of the blockage, concurrent heart disease, complications, and response to treatment. A 2024 review reports that recent studies suggest 30–40% survival with supportive care. That is a literature-level estimate, not an individual cat’s odds: studies differ in which cats they include and how they define outcomes, and admission euthanasia has historically been common.

There is no reliable universal recovery deadline. A retrospective 2020 series of 15 cats reported an association between earlier presentation and recovery in that small cohort, but it cannot establish a treatment cutoff or predict an individual outcome. Urgent assessment remains important; it does not guarantee recovery.

Complications and care after the emergency

FATE can involve ongoing loss of blood supply and muscle injury. Reported complications include rhabdomyolysis (muscle breakdown), acute kidney injury, and reperfusion injury, which can occur when blood flow returns to damaged tissue. The veterinary team monitors for complications as part of treatment.

Further clotting is a longer-term concern. After the immediate episode, follow-up may include care for underlying heart disease and veterinarian-directed thromboprophylaxis (treatment intended to reduce future clot risk). The 2024 review describes median survival beyond one year in some post-FATE thromboprophylaxis contexts; that is not a universal expectation or a guarantee for an individual cat.
