FIV in Cats: Symptoms, Testing, Transmission and Care

FIV is not an automatic short-term prognosis for cats. Learn how it spreads, why symptoms cannot confirm it, what test results mean, and how to support an FIV-positive cat.

An FIV diagnosis is not an automatic short-term prognosis. Many cats with feline immunodeficiency virus remain outwardly healthy for years; others develop infections or other illness as their immune defenses weaken. Symptoms alone cannot tell you whether a cat has FIV, so diagnosis and follow-up belong with a veterinarian.

What is FIV?

Feline immunodeficiency virus (FIV) is a virus that affects cats and can weaken immune defenses over time. It is related to HIV in broad viral-family terms, but it is species-specific: Cornell University’s Feline Health Center says there is no evidence FIV can infect or cause disease in people.

Cornell’s overview, last updated in 2021, reports that about 2.5–5% of healthy cats in North America are infected, with rates of 15% or more among sick or higher-risk cats. These are estimates reported by that source, not a current measurement of every region or population.

What are the symptoms of FIV in cats?

Some infected cats have no visible signs for months or years. When illness develops, it may result from secondary infections or immune-related disease. Cornell describes three broad phases, but not every cat follows an identical course.

Acute phase

The acute phase generally occurs one to three months after infection. Temporary swollen lymph nodes, fever, low energy, and poor appetite may occur. Signs can be mild and go unnoticed.

Asymptomatic phase

The cat may appear healthy during this period, which can last months to multiple years. The absence of symptoms does not establish whether a cat is infected.

Progressive immune compromise

As immune defenses are affected, problems can include recurrent or chronic skin, eye, urinary, or upper respiratory infections; severe gum and mouth inflammation (gingivostomatitis); weight loss; cancer; immune-mediated blood disorders; seizures; behavior changes; or other neurological problems. None of these signs is unique to FIV. A veterinarian needs to assess the cat rather than infer FIV from symptoms.

How do cats get FIV?

The primary route is a bite wound from an infected cat. Unneutered outdoor males that fight are at greater risk because they are more likely to be bitten.

  • Shared bowls, mutual grooming, and other peaceful, non-aggressive contact are not considered efficient transmission routes in Cornell’s overview.
  • Stable groups of cats that do not fight have a low transmission risk, although a veterinarian can advise on the particular cats and household.
  • Sexual transmission is not considered significant.
  • Transmission from mother to kitten is rare; risk increases if the mother becomes infected during pregnancy.

Keeping cats indoors and reducing fighting through spaying or neutering are practical prevention measures. Testing household cats can help a veterinarian assess exposure and care needs.

Can FIV-positive and FIV-negative cats live together?

In a peaceful, established household where cats do not fight, the transmission risk is low according to Cornell’s overview. Because bite wounds are the principal concern, introductions and any history of aggression matter. Ask your veterinarian to evaluate the individual cats and advise on a safe household plan.

If a new cat is moving into a home where an FIV-positive cat lived, Cornell recommends thorough cleaning and disinfection, or replacing dishes, bedding, litter pans, and toys. This precaution also addresses other infectious agents that may be present; it does not mean ordinary shared objects are an efficient FIV transmission route.

How is FIV diagnosed?

FIV is usually diagnosed with a blood test that looks for antibodies, often an in-clinic ELISA screening test. The result needs to be considered alongside exposure timing, age, vaccination history, and the cat’s health. A positive result may need confirmation, particularly for a healthy cat.

Why a recent exposure can test negative

After infection, it can take two to six months for antibodies to reach detectable levels. A negative antibody result soon after a possible bite therefore may not rule out infection. Cornell recommends retesting at least 60 days after possible exposure; your veterinarian can advise on timing based on the exposure and test used.

Why a positive result may need context

A positive screen in a healthy cat may warrant confirmation with another method, such as Western blot or immunofluorescence (IFA), as described by Cornell. Kittens can test positive because they carry maternal antibodies without being infected. Cornell advises retesting positive kittens every 60 days until they are at least six months old.

FIV vaccination can also produce antibodies and complicate interpretation. Cornell reports that no FIV vaccine has been commercially available in North America since 2016, while noting availability in Australia, New Zealand, and Japan. Availability may change and varies by location; tell the veterinarian about any vaccination history and confirm current local guidance.

Antibody tests and PCR are not interchangeable

Antibody tests look for the cat’s immune response. PCR looks for viral DNA, but Cornell notes that PCR can produce false-positive and false-negative results and does not recommend it as the preferred screening method. The veterinarian should interpret positive, negative, or conflicting results in context rather than treating any one result as infallible.

Can cats with FIV live a normal life?

Yes. Cornell says appropriately managed FIV-positive cats can live normal, healthy lives for many years, although some develop secondary illness. The diagnosis alone does not set an individual cat’s lifespan or predict when symptoms will appear.

Care focuses on reducing infection risk, noticing health changes early, and monitoring with a veterinarian:

  • Keep the cat indoors and spay or neuter to reduce roaming, fighting, and reproductive transmission.
  • Feed a complete, balanced diet. Avoid raw meat, raw eggs, and unpasteurized dairy because of foodborne infection risks.
  • Arrange wellness visits at least every six months. Cornell recommends annual complete blood count (CBC), serum biochemistry, and urinalysis.
  • Record weight accurately at home. Cornell identifies weight loss as a possible early sign of deterioration; a digital pet scale for cats can help track weight, but it cannot diagnose FIV or replace veterinary exams and laboratory tests.
  • Contact a veterinarian promptly when the cat seems ill rather than waiting for a routine appointment.

Is there a cure for FIV?

Cornell’s overview describes no definitive cure. Treatment is directed at the cat’s health problems and may include care for secondary infections. Zidovudine (AZT) may help some cats with severe stomatitis or neurological disease, but it can have serious side effects and has not been shown to prolong survival in FIV-infected cats. It should only be considered under veterinary direction.

Can I catch FIV from my cat?

No evidence shows that FIV infects or causes disease in people, according to Cornell’s Feline Health Center. FIV is a feline virus, not human HIV.
