Hematomas on Cats’ Ears: Causes, Symptoms & Treatment

Aural hematomas are painful blood pockets in a cat’s ear flap. Learn the signs, possible causes, veterinary treatment options, and why the underlying ear problem matters.

A swollen, soft or spongy cat ear flap may be an aural hematoma: blood collected beneath the skin of the visible ear. It is painful and should be assessed promptly by a veterinarian. Treatment may involve draining the pocket, surgery, or another approach, while also finding and treating the ear irritation that led to scratching or head shaking.

What an ear hematoma is

The pinna—the part of the ear you can see—has skin on both sides with cartilage between them. When blood vessels are damaged, blood can collect in the tissue space, forming a pocket of fresh or clotted blood. The swelling may affect a section of the flap or most of it. This is more than a surface bruise. VCA Animal Hospitals explains the anatomy and formation of aural hematomas in cats.

Causes: why the ear may swell

Often, an ear problem starts a cycle: irritation makes a cat scratch or shake its head vigorously, that movement damages small vessels, and blood collects in the flap. Possible triggers include an ear infection, allergies, other inflammation, mites, or foreign material such as a tick or grass. Direct trauma, including a blunt injury or wound, can also cause a hematoma. Clotting disorders may contribute, even when no injury is apparent. Cornell University College of Veterinary Medicine and VCA describe these possible causes.

Symptoms and when to seek care

  • A thick, soft or spongy swelling on part or all of the ear flap.
  • Scratching at the ear or repeated head shaking, which may signal that the original irritation is still present.
  • In some cases, swelling that narrows or closes off the ear canal.

Arrange veterinary assessment promptly if you suspect a hematoma. VCA describes these swellings as very painful; untreated inflammation can damage nearby tissue and leave the ear permanently thickened or distorted in a cauliflower-like shape, sometimes obstructing the canal. Do not squeeze, puncture, or attempt to drain the swelling, and do not put medication in the ear unless a veterinarian directs you to.

How veterinarians treat aural hematomas

The veterinarian will assess the swelling and examine the ear for a cause of the scratching or head shaking. The treatment plan depends on the individual case. Options include needle aspiration, drainage through an incision or drain, and surgery to empty the pocket and hold the tissue layers together with sutures. A veterinarian may also stabilize the pinna with a bandage or other material. Cornell describes surgical drainage and suturing as a commonly used approach; the American College of Veterinary Surgeons outlines veterinary treatment options.

Approach How it works What to expect
Needle aspiration A veterinarian removes blood from the pocket with a needle. It may be temporary; repeat drainage or visits may be needed.
Incision or drain A veterinarian opens or drains the pocket; a drain may help fluid escape during healing. Follow-up and care depend on the method and the cat’s progress.
Surgical evacuation and sutures After emptying the pocket, the veterinarian uses sutures to press the inner and outer tissue layers together, reducing the space where blood can collect again. The procedure may require anesthesia. Rechecks and aftercare are set by the treating veterinarian.

These approaches are not interchangeable prescriptions. Aspiration may offer less invasive drainage but can be temporary; nonsurgical treatment may take longer and require repeat visits. Surgery is commonly used, but the veterinarian selects an approach based on the cat and the hematoma. Cornell discusses aspiration and surgery, while VCA notes that repeat treatment may be needed.

Treat the cause and follow the aftercare plan

Draining the blood pocket does not address what made the cat scratch or shake its head. If an infection, allergy, mites, or foreign material is responsible, the veterinarian will address that problem too. A hematoma can recur if the underlying irritation—particularly an infection—remains. When infection is present, VCA recommends rechecking the ear canal.

After drainage or surgery, follow the treating veterinarian’s instructions for any bandage, drain, medication, and recheck appointments. Follow-up timing varies with the procedure and healing, so use the schedule provided for your cat rather than a general timetable.
