Record changes in eating, drinking, hygiene, mobility, social interaction, and favorite activities, then review the pattern with your veterinarian. A quality-of-life scale can organize those observations, but it cannot diagnose your pet or make a decision for you.
Start with your pet’s usual baseline
Compare your pet with their own normal routine, not with another animal or an idea of what every senior pet should be like. Consider what is usual for appetite and drinking, sleep and rest, movement around the home, toileting, social behavior, and activities your pet enjoys. Age alone does not tell you whether a pet is comfortable or having a good life.
Look for changes and patterns. The American Animal Hospital Association (AAHA) notes that appetite and weight changes can be early indicators of illness or discomfort, and points to activity, social interaction, and physical and emotional comfort as relevant parts of quality of life. What to observe each day
A brief, repeatable record is more useful than trying to reconstruct events from memory during a stressful appointment. Note what changed, when it changed, and whether it persists or varies from day to day.

- Comfort: Note changes in resting, movement, or responses that concern you, and describe them to your veterinarian.
- Eating and drinking: Record appetite, drinking, and any noticeable change in weight. Do not dismiss a change as an inevitable part of aging.
- Hygiene and toileting: Observe whether your pet can toilet and stay clean as usual, and whether they need more help.
- Mobility: Track getting up, moving around familiar spaces, and any assistance needed.
- Behavior and connection: Record interaction with family, hiding, play, and interest in familiar routines.
- Good and bad days: Make a simple daily note about how the day went overall. A run of difficult days or a change in the balance can be easier to recognize in a record than in hindsight.
Use a quality-of-life scale as a prompt, not a verdict
One familiar framework is HHHHHMM: hurt, hunger, hydration, hygiene, happiness, mobility, and more good days than bad. Use those categories to help describe what you see; they are not a diagnosis or a universal pass-or-fail threshold. AAHA says, “These tools are designed to support, not replace, conversations with your veterinarian.”
The Ohio State University Veterinary Medical Center’s caregiver assessment offers another set of prompts, including play, interaction, enjoyment, hiding, mobility, hygiene, and overall health. You can use it to compare your pet’s current behavior with their own baseline.

Different tools organize observations in different ways. A number can make notes more consistent, but it cannot capture every detail of an individual pet’s circumstances. Avoid interpreting one total as a universal point at which a particular decision must be made.
Account for differences between cats and dogs
The general areas above can guide observation for either species, but cats and dogs may show illness, discomfort, and behavior changes differently. A tool designed for one species should not be assumed to be validated for the other.

The 2023 AAFP/IAAHPC feline hospice and palliative-care guidelines discuss feline-specific instruments, including a caregiver-completed 20-item health-related quality-of-life tool covering vitality, comfort, and emotional well-being. The guidelines also note that many available tools have not been rigorously validated. Tracking changes over time may still help you and your veterinary team discuss your cat’s care. When you have concerns, share the notes, their timing, and what your pet still enjoys. Specific examples can help the veterinary team understand changes that may not be apparent during an appointment. Ask which changes might be treatable, what comfort-focused options could fit your pet’s diagnosis and needs, and how you will tell whether a care plan is helping.
