How to File a Pet Insurance Claim and Get Reimbursed

A practical guide to submitting a pet insurance claim, gathering veterinary records, understanding reimbursement, and appealing a denial.

To file a pet insurance claim, follow your insurer’s instructions, send the required claim form and veterinary documents, then track the decision and payment. Before you submit, check your policy for covered expenses, filing deadlines, deductibles, reimbursement rules, and limits. These details vary by insurer and policy, so there is no single process or payout that applies to every pet.

1. Check your policy and filing deadline

Start with your policy documents and the insurer’s current claims instructions. Confirm that the treatment happened during the covered period and falls within a benefit your policy includes. Check waiting periods and exclusions, too: an expense may be ineligible even if it appears on a veterinary bill.

Look for the deadline to file. It is set by the insurer and policy, not by a universal pet-insurance rule. MetLife, for example, publishes a 90-day claim deadline for its process; do not assume that deadline applies to another insurer or policy. MetLife claims guidance

Also find out how the insurer calculates reimbursement. The National Association of Insurance Commissioners (NAIC) says policies may base benefits on the actual bill, a benefit schedule, or usual-and-customary fees. 2. Gather the invoice, records, and any required forms

Ask the veterinary practice for an itemized invoice showing the services and charges. Gather the medical records related to the visit, since they help document the diagnosis and treatment. Depending on the insurer, you may also need proof of payment and a completed, signed claim form. Check first-claim requirements

A first claim may require records beyond those for the current visit. MetLife says its first claim requires the prior 12 months of veterinary records, or adoption records where applicable. That is a MetLife-specific requirement, not a standard that applies to all insurers. MetLife claims guidance

Before submitting, compare your documents with the insurer’s checklist. If something is missing, ask the veterinary practice or insurer how to obtain it rather than sending an incomplete packet.

3. Submit through the insurer’s accepted channel

Use the submission method allowed for your policy. Some insurers accept claims through an online portal, but available channels vary.   1. Complete the insurer’s claim form, if required.
  2. Attach the itemized invoice and relevant veterinary records, plus proof of payment or other requested documents.
  3. Submit through the specified portal or other accepted channel.
  4. Save a copy of everything you sent and keep the confirmation or submission reference.

A folder—digital or paper—for invoices, records, claim forms, and insurer correspondence can make it easier to find documents if the insurer asks for more information.

4. Track the decision and understand the payment

Monitor the claim using the insurer’s portal or the contact method it provides. Respond promptly if the insurer asks for additional records or clarification. Keep a note of the submission date and any follow-up.

Do not assume reimbursement will equal a fixed percentage of the total bill. The insurer first determines which charges are eligible under the policy, then applies its reimbursement method, deductible, coinsurance, and applicable limits. Ask for an itemized explanation if the payment differs from what you expected. Payment methods, such as direct deposit, also vary by insurer.

5. Challenge a denial or lower-than-expected payment

Read the denial or payment explanation and identify the specific reason. If it is unclear, ask the insurer which policy provision it applied and whether it needs additional documents. Then follow the appeal instructions and deadline in your policy or denial notice; do not assume another insurer’s appeal timetable applies.

  1. Note the stated reason for the denial or reduced payment.
  2. Check the policy language relevant to that reason.
  3. Gather supporting records, invoices, or other documents that address the issue.
  4. Send a clear written appeal through the insurer’s required channel before its deadline.
  5. Keep copies of the appeal and all supporting documents, along with confirmation of receipt.

When appealing, connect each piece of evidence to the disputed policy term or the insurer’s stated reason. NAIC’s 2019 guide notes that some complaints may stem from consumers misunderstanding coverage and policy terms; checking the specific contract can help clarify what the insurer must consider. What to check in a policy before future care

If you are comparing policies, look beyond the advertised reimbursement rate. Review the contract and any state-specific terms for:

  • How reimbursement is calculated: actual bill, benefit schedule, or usual-and-customary fees.
  • Whether the deductible applies annually or per incident, and how coinsurance works.
  • Per-incident, annual, or lifetime benefit limits.
  • Waiting periods, exclusions, and the claim-filing deadline.
  • Which medical records are required and how claims must be submitted.
  • Whether payment can go directly to a veterinarian and what appeal process applies.

These features differ by policy and are not interchangeable guarantees. A new policy also does not establish coverage for a condition or treatment that existed before the policy began; check the current contract for its terms.
