How to Recognize and Manage Cats at Risk for Diabetes

A high blood sugar result does not automatically mean a cat has diabetes. Learn how veterinarians assess stress-related elevations, follow-up tests, risk factors, and monitoring.

A cat with mildly high blood glucose does not necessarily have diabetes: stress at the veterinary clinic can temporarily raise glucose, and a single result is not enough to diagnose the disease. A veterinarian distinguishes a transient rise from persistent hyperglycemia by reassessing the cat after the stressful event, checking for glucose and ketones in urine, and considering clinical signs and other test results. For an otherwise well cat, the next steps are to look for treatable contributors, support a healthy weight and activity, and set an individualized monitoring plan—not to start diabetes medication.

What does “at risk” or subclinical diabetes mean?

In the American Animal Hospital Association’s (AAHA) 2026 guidance, an at-risk or subclinical cat has intermittent or sustained mild to moderate high blood glucose that is not explained by stress, but does not have the clinical signs of diabetes mellitus (DM). This status is not the same as a confirmed diagnosis. AAHA says it is difficult to predict which at-risk cats will progress to clinical diabetes; the guideline gives no quantified progression rate.

Diabetes is assessed from the whole clinical picture, not from one laboratory value. A veterinarian looks for sustained hyperglycemia and may use repeated results collected when the cat is not stressed, samples obtained at home, or increased glycation markers. High blood glucose or glucose in the urine can occur for reasons other than diabetes, so results need veterinary interpretation.

Can stress cause high blood sugar in cats?

Yes. Stress, including the trip to a clinic, can cause temporary hyperglycemia in cats. AAHA notes that stress-associated hyperglycemia is generally below 250 mg/dL, although it can be severe. Glucose may also spill into the urine during severe stress; ketones in the urine are not expected from stress hyperglycemia.

Because of this, a high clinic reading in a cat without symptoms may need follow-up rather than an immediate diabetes label. The approximate 250–300 mg/dL kidney glucose-reabsorption threshold cited in AAHA’s recognition guidance is diagnostic context, not a stand-alone cutoff for diagnosing diabetes.

How veterinarians distinguish stress from persistent hyperglycemia

For an asymptomatic cat, the veterinarian may recommend checking blood glucose and urine glucose and ketones again several days after the stressful event has passed. When practical, samples collected at home can reduce the stress associated with a clinic visit. The pattern over time, the urine findings, and whether clinical signs appear help guide the assessment.

Depending on the cat and the results, the veterinarian may consider continuous glucose monitoring or a serum fructosamine test, which reflects blood glucose over a longer period than a single measurement. No one follow-up test or schedule is right for every cat; the veterinarian chooses methods and timing based on the individual case.

Signs that warrant a veterinary assessment

Contact a veterinarian if you notice changes that could fit diabetes, including:

  • Drinking more water or urinating more often
  • Increased appetite alongside weight loss
  • Lethargy or reduced energy
  • Hindlimb weakness or walking with the hocks lowered in a plantigrade stance
  • An unkempt coat

These signs do not confirm diabetes on their own. A veterinarian must interpret them alongside examination findings and test results. Seek veterinary advice promptly if they develop or worsen, rather than waiting for a routine monitoring visit.

What can contribute to insulin resistance?

In an at-risk cat, the veterinarian may review conditions and exposures that can contribute to insulin resistance or complicate glucose regulation, including:

  • Obesity or excess body weight
  • Medications, including corticosteroids or progestins

AAHA advises avoiding medications such as corticosteroids or progestins when possible, and managing concurrent conditions with veterinary oversight. Do not stop or change a prescribed medication without discussing the risks and alternatives with the prescribing veterinarian.

Practical steps at home

Track weight and body condition

AAHA recommends that caregivers weigh cats at home monthly. A consistent routine and a scale suitable for pets can make it easier to notice gradual changes; weight tracking is not a test for diabetes. Your veterinarian can interpret the trend alongside body condition and advise whether weight reduction is appropriate.

Review food and calorie intake

Work with the veterinarian to assess the cat’s diet, portions, treats, and calorie intake. A plan should fit the cat’s health, body condition, and household routine rather than rely on a particular food or feeding device as a universal solution.

Encourage movement and enrichment

Offer regular opportunities for play and activity, and use environmental enrichment that suits your cat. One practical option is to replace some food treats with play, where appropriate. These lifestyle measures may support weight management, but they have not been shown to prevent diabetes in cats.

How often should an at-risk cat be monitored?

There is no universal blood glucose or urinalysis schedule for every at-risk cat. The veterinarian will set the frequency and type of checks according to the cat’s test results, health, weight-management plan, and any changes in symptoms. Home weight records and observations of drinking and urination can help inform those appointments.

Follow-up can show whether hyperglycemia resolves as stress passes or weight changes, remains present, or progresses. Contact the veterinary team if thirst, urine output, appetite, energy, or weight changes between scheduled checks; they can decide whether the cat should be assessed sooner.

Should a cat with high blood sugar start insulin?

Not solely because of an elevated result or an at-risk status. AAHA’s 2026 guideline states that there is no evidence supporting SGLT2 inhibitors, insulin, or other anti-diabetic medication for at-risk or subclinical cats. Diabetes treatment is not indicated until clinical DM is confirmed. Starting, stopping, or adjusting diabetes medication should be a veterinarian’s decision based on diagnosis and the cat’s clinical condition.

Can diabetes be prevented?

AAHA states, “There is currently no known strategy to prevent DM.” Weight optimization, thoughtful calorie management, activity, and enrichment are reasonable health measures, but they should not be presented as a proven way to prevent diabetes. Their value for an individual cat is supporting overall care and helping the veterinary team monitor changes.
