What demodectic mange is
Demodectic mange (canine demodicosis) is a skin disease caused by Demodex mites living in hair follicles. Dogs commonly carry small numbers of these mites without illness. Merck Veterinary Manual explains that mites usually pass from a mother to her puppies during nursing in the first 72 hours after birth. That normal transfer is why canine demodicosis is generally not considered contagious between dogs.

Disease develops when mite numbers increase and the puppy’s skin or immune response cannot keep them controlled. Juvenile cases are usually seen before one year of age. A hereditary and immunologic component is suspected in generalized juvenile disease, which is why the 2020 World Association for Veterinary Dermatology consensus guideline advises against breeding affected dogs and their parents.

The word “mange” is not a diagnosis by itself. Sarcoptic mange and other mite conditions can be contagious and can look different or overlap with demodicosis. Ringworm, bacterial folliculitis, allergies, fleas, endocrine disease, nutritional problems and self-trauma can also cause bald or inflamed areas. A visual checklist can tell you when to arrange care; it cannot identify the mite species.

How localized demodicosis may look
Typical pattern
Localized juvenile demodicosis usually produces one to five small, well-demarcated patches of hair loss. Merck describes redness and scaling in these spots, most often around the lips, eyes and forelegs. The skin may look mildly pink or flaky rather than dramatically inflamed. Itching is commonly absent or mild, although a puppy may rub a sore area.
- Distribution: face, muzzle, eyelids and forelegs are common starting points.
- Edges: patches tend to be clearly outlined rather than diffuse thinning across the whole body.
- Surface: fine scale or redness may be visible; heavy pus, thick crusts or deep wounds suggest complications or another disease.
- Behavior: many puppies remain bright, hungry and playful.
Most limited cases settle without mite-specific medication. That does not make every bald patch harmless: a minority of localized cases progress to generalized disease, and similar-looking conditions need different treatment. Arrange a veterinary examination rather than assuming a patch will resolve.
How generalized demodicosis may look
More extensive skin disease
Generalized juvenile demodicosis affects multiple body regions or a large proportion of the skin. Hair loss may become widespread, with redness, papules (small raised bumps), crusts, oily or greasy skin, swelling and increased pigmentation. The coat can thin in irregular islands or in broad areas. Feet may become inflamed, swollen or painful; inflammation between the toes is an important reason to seek prompt care.

Bacterial infection can make demodicosis substantially more severe. Fever, unusual tiredness, poor appetite, enlarged lymph nodes, pus or deep skin inflammation indicate possible complications and warrant prompt veterinary assessment rather than watchful waiting.
Localized versus generalized: a practical comparison
| Feature | Localized pattern | Generalized pattern |
|---|---|---|
| Extent | Often one to five small, sharply defined patches | Many areas, large regions or widespread hair loss |
| Common sites | Near lips, eyes and forelegs | Trunk, limbs, face and feet can all be involved |
| Itching | Usually absent or mild | May increase, especially when bacteria are also present |
| Skin changes | Light redness and scale are common | Redness, papules, swelling, crusts, oiliness and darkening are possible |
| Feet | Usually not a major feature | Inflamed or swollen feet raise concern for extensive disease |
| Whole-body illness | Puppy often acts normal | Fever, lethargy, enlarged nodes or poor appetite suggest complications |
| What confirms it | Microscopic examination of appropriate skin samples; appearance alone cannot classify the disease reliably | |
This comparison helps you describe what you see to a clinic. It is not a substitute for sampling: a few lesions can hide generalized disease, and another skin disorder can mimic either pattern.
When a puppy needs veterinary care
Contact your veterinarian for any unexplained hair loss, particularly in a puppy younger than one year. Request an earlier appointment if the patch is enlarging, new areas appear, several body regions are involved or a foot is affected. Same-day advice is appropriate when skin is painful, hot, swollen, bleeding or draining.
Seek urgent veterinary attention if the puppy is lethargic, feverish, refusing food, vomiting, developing rapidly spreading redness, or has extensive pus, a strong odor or deep wounds. These signs can reflect secondary infection or another serious illness, not simply a cosmetic mite problem.

How veterinarians confirm demodicosis
Deep skin scrapings
The usual test is a deep skin scraping examined under a microscope. The clinician gently squeezes the skin and scrapes deeply enough to collect material from hair follicles, then looks for Demodex mites and their life stages. The 2020 consensus guideline calls deep skin scrapings the diagnostic gold standard.
Additional sampling
Hair plucks (trichograms) and tape-squeeze preparations can be useful in selected circumstances, such as lesions that are difficult to scrape or sensitive areas around the eyes. A veterinarian may collect samples from several sites because one negative sample does not always exclude disease when suspicion remains high.
Checking for complications and look-alikes
The examination may include cytology to look for bacteria or yeast, evaluation for fleas and other parasites, and tests for ringworm or underlying illness when the history suggests them. Your veterinarian interprets all results together; a microscope finding, lesion pattern and clinical course matter more than any single photograph.
What to expect during the appointment
Bring a timeline of the first lesion, changes in its size or number, and any rubbing, licking or foot discomfort. The veterinarian will examine the entire skin surface rather than only the most obvious bald spot. Mention the puppy’s age, appetite, energy level, previous illnesses and every parasite product or cream used. These details help distinguish a localized problem from a broader process and identify possible medication risks.

Random freezes, missing sound and display glitches usually trace back to one bad driver. Find and replace yours safely.Free scan · under a minuteDeep scrapings may be taken from more than one lesion, including an active edge or an area with follicular changes. A trichogram or tape preparation can supplement scraping when a site is difficult to sample. If bacteria are suspected, cytology may be performed so infection is treated appropriately. Additional testing is guided by the examination; not every puppy needs the same panel of tests.
A negative sample is interpreted in context. If the lesions and history remain suspicious, the veterinarian may repeat sampling or choose another technique. Conversely, finding a mite does not by itself explain every skin change, so the clinician also considers bacterial infection and look-alike diseases.
What to do before the appointment
- Record the timeline. Note when the first spot appeared, how quickly it changed and whether new areas or foot lesions developed.
- Take clear photographs. Photograph the same sites in good light from a consistent distance. Images help show progression but do not diagnose mites.
- Prevent self-trauma. Keep nails short and discourage scratching or rubbing. Ask the clinic before using a cone or any topical product on irritated skin.
- Keep the puppy comfortable and clean. Follow normal bathing and feeding routines unless a veterinarian advises otherwise; avoid harsh shampoos, essential oils and household pesticides.
- Bring medication details. Tell the veterinarian about every flea, tick, deworming, supplement or cream used, including the product, dose and date.
Do not apply leftover antiparasitic drugs, ivermectin, prescription flea-and-tick medicine, medicated dips or steroid creams on your own. Some therapies are used off label, and safety depends on the drug, dose, local approval, health status and monitoring. Merck notes that ivermectin can be toxic and is contraindicated in Collies and Collie crosses, with unpredictable toxicity possible in any breed. Corticosteroids are contraindicated in diagnosed demodicosis. A clinic should select and supervise treatment.

Treatment depends on the extent of disease
Localized disease
Merck states that localized demodicosis can generally be left untreated because many cases resolve as the puppy matures. “Untreated” does not mean “never rechecked.” Your veterinarian may recommend observation, skin care and a planned review to ensure lesions are shrinking rather than multiplying. Treatment is reconsidered if patches spread, recur, become infected or fail to improve.
Generalized disease
Generalized demodicosis is serious and normally requires veterinarian-directed acaricidal (mite-killing) treatment. The exact medicine and schedule vary by country, regulatory approval, breed, age, concurrent illness and the veterinarian’s assessment. Do not assume a product approved for one parasite or one region is appropriate for this disease.
Secondary bacterial infection is assessed and treated when present. That may involve prescription antimicrobials, topical care or both, selected from examination and laboratory findings. Treating infection can improve pain and odor, but it does not replace mite control when mites are driving the disease.
Why treatment can take months
Mites occupy follicles, and skin needs time to heal after mite numbers fall. Generalized cases often require several months of therapy. Visible hair regrowth is encouraging but is not proof that mites are gone.
Monitoring: when is treatment finished?
Follow-up combines a clinical examination with repeat microscopic testing. Merck advises continuing treatment until there are at least two consecutive monthly negative skin scrapings. The 2020 consensus guideline recommends continuing miticidal treatment for four weeks beyond the second set of negative monthly scrapings to reduce the chance of relapse.
Keep every scheduled monthly recheck, even when the puppy looks normal. Ask the clinic what to do if a dose is vomited, missed or causes a reaction; do not double a dose. Contact the veterinarian promptly for weakness, tremors, vomiting, diarrhea, breathing changes or other unexpected signs after medication.
Recurrence during the first year is not uncommon. New bald spots after apparent recovery deserve another examination rather than an automatic restart of old medicine. A relapse can reflect residual mites, an underlying immune problem or a different skin disease; reinfection from another dog is not the usual explanation for canine demodicosis.

Is demodectic mange contagious?
Merck’s professional reference states: “The disease is not considered to be contagious.” Canine Demodex mites are normal residents in small numbers and are generally transferred from dam to nursing puppies early in life. Routine separation of a puppy from healthy household dogs is therefore not usually needed solely because demodicosis is suspected.
This does not apply to every condition called mange. Sarcoptic mange and Cheyletiella can spread between animals and, in some cases, to people. Until a veterinarian identifies the cause, avoid sharing brushes or bedding with unfamiliar animals and tell the clinic about itching in people or other pets.

Breeding and prevention considerations
There is no reliable home prevention program that eliminates the normal transfer of Demodex mites. Good nutrition, parasite prevention chosen by a veterinarian and timely treatment of illness support general health but cannot guarantee that demodicosis will not occur.
Juvenile generalized demodicosis has a hereditary or immune-related component. The 2020 consensus guideline recommends that dogs affected with generalized disease, and their parents, not be bred. If a puppy develops generalized disease, discuss the diagnosis with the breeder and provide veterinary records; this is a population-health decision, not a reason to blame the individual puppy.
How common is it?
A 2020 World Association for Veterinary Dermatology consensus review reports that as many as 0.58% of dogs in the United States suffer from generalized demodicosis. The review attributes that number to an earlier cited study; it is secondary reporting, not a current national prevalence survey and not an estimate of an individual puppy’s risk. The more useful point for an owner is that generalized disease is uncommon enough that other diagnoses remain possible, yet important enough to investigate when signs are extensive.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Common mistakes to avoid
- Diagnosing from a photo: baldness and scale have many causes, and lighting can hide pustules or swelling.
- Waiting because the puppy is not itchy: mild or absent itching is compatible with localized demodicosis; it does not rule out disease.
- Assuming every case is harmless: a localized patch can progress, and generalized disease can involve painful infection.
- Using a household or leftover remedy: incorrect products can delay diagnosis, worsen inflammation or cause toxicity.
- Stopping when hair returns: microscopic tests, not coat appearance alone, determine whether treatment can end.
- Isolating every dog automatically: canine demodicosis is generally not contagious, although other mite diseases are.
A simple decision checklist
- One to five small facial or foreleg patches, puppy otherwise well: book a routine veterinary appointment and monitor for spread.
- Several patches, expanding hair loss, redness, crusts or oily skin: arrange a prompt examination and ask about deep skin scrapings.
- Inflamed feet, pain, pus, odor, swelling or draining lesions: seek prompt care because secondary infection or generalized disease is possible.
- Fever, lethargy, poor appetite or enlarged lymph nodes: contact a veterinarian urgently.
- After diagnosis: use only the prescribed plan, attend monthly checks and continue treatment for the full period advised after negative scrapings.
What a good outcome looks like
Most puppies with limited localized disease recover as their immune system matures. Generalized disease can also be controlled, but success depends on an accurate diagnosis, an appropriate veterinary-selected mite treatment, management of bacterial infection when present and disciplined follow-up. Expect improvement to be measured over weeks to months, not overnight. If lesions return, recheck rather than guessing, because recurrence and look-alike diseases require different responses.
Frequently Asked Questions
Can I tell from a photograph whether my puppy has demodectic mange?
No. Small bald patches, scale and redness can occur with Demodex, ringworm, bacterial disease, allergies, fleas and other problems. Photos are useful for documenting change, but a veterinarian usually needs deep skin scrapings or another appropriate sample to identify mites.

If my puppy has only one bald spot, can I wait?
Arrange a veterinary appointment even when the puppy feels well. One to five sharply defined facial or foreleg patches can fit localized disease, which often resolves, but some cases progress and other conditions look similar. Monitor the spot for enlargement, new lesions, pain, discharge or foot involvement.
Will demodectic mange spread to my other dog or to my family?
Canine demodicosis is generally not considered contagious. Demodex mites commonly live on dogs in small numbers and pass from mother to puppies during early nursing. Other conditions called mange, including sarcoptic mange, can spread, so confirmation of the cause still matters.
Why does my puppy need repeat scrapings if the hair is growing back?
Coat regrowth shows healing but does not prove that mites are cleared. Merck advises at least two consecutive monthly negative scrapings, and the 2020 consensus guideline advises continuing miticidal treatment four weeks beyond the second negative monthly scraping. Your veterinarian sets the schedule.

Can I use ivermectin or a leftover flea-and-tick product?
Do not self-medicate. Drug choice, dose, approval and monitoring depend on the individual puppy. Merck warns that ivermectin can be toxic, is contraindicated in Collies and Collie crosses, and can cause unpredictable toxicity in any breed. Corticosteroids are contraindicated in diagnosed demodicosis.
How long will generalized demodicosis treatment take?
Generalized cases commonly need several months of veterinarian-directed therapy, with treatment continuing through clinical improvement and negative microscopic checks. Secondary bacterial infection may require separate treatment. Missed doses, reactions or new lesions should be discussed with the clinic rather than managed by changing the plan yourself.
Should a dog that had generalized juvenile demodicosis be bred?
The 2020 World Association for Veterinary Dermatology consensus guideline recommends that affected dogs and their parents not be bred because generalized juvenile disease has hereditary or immunologic components. Discuss records and breeding decisions with your veterinarian and breeder.
The Bottom Line
Small, lightly itchy bald patches may be localized demodicosis, while widespread loss, inflamed feet, pus or illness require prompt veterinary care. Only appropriate microscopic sampling can confirm the diagnosis. Do not self-medicate; follow the prescribed plan and monthly rechecks until testing shows the mites are cleared.
