How Vets Diagnose Alabama Rot and Assess Other Causes of Skin Sores

Vets assess suspected Alabama Rot through the skin lesion, history and kidney tests. Learn what bloodwork and biopsies can—and cannot—confirm.

Vets do not diagnose Alabama Rot from a skin sore alone. They assess the lesion and the dog’s history, check for kidney injury and other blood abnormalities, and may repeat tests if concern remains. There is no single non-invasive test that confirms the disease: confirmation requires examination of kidney tissue, and is commonly made after death because kidney biopsy is rarely appropriate in an acutely ill dog.

What vets look for first

Alabama Rot is the common name for cutaneous and renal glomerular vasculopathy (CRGV). Vets consider it when a dog has compatible erosive or ulcerative skin lesions, but a sore is a reason to investigate—not proof of CRGV. Some dogs have skin lesions without detectable acute kidney injury.

The vet examines the sores and asks when they appeared, how they have changed, and where they are. The dog’s general health and relevant history also matter. A clear description of each lesion, its location and duration, and photographs where useful can help guide the assessment. This information helps the vet consider other explanations rather than treating a particular-looking sore as diagnostic.

The sources do not establish a complete list of causes for canine skin sores. The vet’s differential diagnosis—the set of possible explanations considered—depends on the individual dog and findings from examination, laboratory tests and, when appropriate, tissue analysis.

How vets check for kidney injury

For a dog with a compatible lesion, specialist CRGV guidance recommends baseline blood testing that includes serum creatinine and a platelet count, with urinalysis ideally included. Depending on the case, the vet may assess additional measures of kidney function. These tests help identify kidney injury and monitor change; they do not confirm CRGV by themselves.

If concern remains, the vet may reassess bloodwork and urinalysis within 24 to 48 hours and ask the owner to monitor the dog’s urine output. This follow-up matters because kidney injury can appear after the skin lesions: Royal Veterinary College guidance reports an interval of one to nine days in the cases it discusses. That range is not a prediction for an individual dog, and monitoring should be directed by a veterinarian.

What abnormal results mean

In the Alabama Rot veterinary professional resource’s clinical case material, among dogs presented with acute kidney injury, nearly 84% had thrombocytopenia (a low platelet count), 29% had anaemia, 91% had elevated serum urea, 86% had elevated serum creatinine and 72% had elevated liver enzyme activities. These figures describe that case material; they are not test accuracy estimates or population-wide prevalence. No single result establishes CRGV.

Why blood and urine tests cannot confirm Alabama Rot

Suspicious skin lesions together with acute kidney injury may strengthen the suspicion of CRGV. Platelet abnormalities or elevated bilirubin can also be part of the clinical picture, but none of these findings provides a stand-alone yes-or-no diagnosis. The Alabama Rot veterinary professional guidance explains that antemortem diagnosis is challenging because there is no single non-invasive diagnostic test available (Diagnosing CRGV).

Definitive confirmation depends on renal histopathology: examination of kidney tissue for thrombotic microangiopathy, the characteristic damage to small blood vessels. Kidney biopsy is rarely recommended for dogs with acute kidney injury, and low platelets can make tissue sampling a concern. As a result, confirmation is commonly post-mortem. The review by Holm and colleagues describes this diagnostic challenge and the role of renal histopathology (2019 review of CRGV).

What a skin biopsy can—and cannot—show

A skin biopsy may help investigate a skin disease when a lesion is unusual, severe, persistent or otherwise warrants tissue examination. It is not the same as a kidney biopsy and cannot, by itself, confirm or rule out CRGV: skin changes associated with the disease may be non-specific.

For an ulcerated lesion, Cornell’s dermatopathology guidance recommends sampling the intact skin margin and extending the sample into the ulcer, rather than taking tissue only from the ulcer’s center. The vet should send the clinical history, a description and location of the lesions, and relevant possible diagnoses; photographs can also help. See Cornell’s dermatopathology guidance.

How season fits into the assessment

Season can provide context, but it is not a diagnostic test. A study of 178 dogs reported that 91% of its cases occurred between November and May. That describes the study cohort, not an individual dog’s likelihood of having CRGV or current annual case levels. A vet still needs to assess the lesion, the dog’s health and test results. The cohort is described in the 2021 study of 178 dogs.

When to contact a vet

Contact a veterinarian promptly about unexplained or worsening skin sores, especially if your dog seems unwell or has changes in drinking, urination, appetite or energy, or is vomiting. Do not try to diagnose Alabama Rot at home or delay veterinary advice while watching a lesion; the vet can decide whether kidney monitoring or further investigation is needed.
