Insulinoma is a rare pancreatic tumor that releases too much insulin, lowering a cat’s blood sugar. Weakness, collapse, seizures, or other sudden neurologic changes need prompt veterinary assessment; diagnosis and treatment typically require blood testing, imaging, and specialist care.
What is insulinoma in cats, and what causes it?
An insulinoma is a functional tumor of the pancreas’s insulin-producing beta cells. Because insulin lowers blood glucose, uncontrolled insulin release can cause hypoglycemia—blood sugar that is abnormally low—and, in turn, neurologic and other signs.
Insulinoma is extremely rare in cats, and feline evidence is limited. The available clinical sources explain how the tumor affects blood sugar but do not establish why a particular cat develops one or identify a preventable cause. The term describes what the tumor does; it does not mean that every pancreatic mass is an insulinoma.
What are the symptoms of insulinoma in cats?

- Dullness, confusion, or other changes in mentation or behavior
In a 2023 multi-institutional retrospective study of 20 cats that underwent insulinoma surgery, 18 had neurologic signs at presentation. That finding describes this selected surgical group; it does not mean that neurologic signs in a cat are usually caused by insulinoma.
Seizures, collapse, or suspected low blood sugar warrant prompt veterinary evaluation. Severe or prolonged hypoglycemia can be dangerous: a feline case report described irreversible neurologic complications after chronic hypoglycemia. These signs have other possible causes, so they cannot identify insulinoma on their own.

How is insulinoma diagnosed in cats?
Veterinarians look for a combination of biochemical and imaging findings, interpreted in the context of the cat’s condition. A pancreatic mass by itself does not confirm insulinoma.
- Document low blood glucose. Bloodwork must establish hypoglycemia. The timing and clinical context matter, so a single result needs veterinary interpretation.
- Interpret insulin alongside glucose. When blood glucose is low, an insulin concentration that is inappropriately normal or elevated supports a presumptive diagnosis. Insulin is normally expected to fall when glucose is low.
- Look for and assess a pancreatic lesion. Abdominal ultrasound or CT may help locate a mass and assess the extent of disease. The American College of Veterinary Surgeons (ACVS) also lists decreased fructosamine as a supportive finding.
- Confirm with tissue when surgery is pursued. ACVS describes biopsy at surgery as the definitive way to diagnose and stage insulinoma. Histopathology, sometimes with immunohistochemistry, can confirm an insulin-secreting pancreatic tumor.
Testing and stabilization may require intensive monitoring or referral to a veterinary specialty hospital. The veterinarian may recommend a specialist based on the cat’s condition and the imaging and laboratory findings.

What is the treatment for insulinoma in cats?
Care may combine stabilization, veterinarian-directed diet and medication, and surgery. The choice depends on the cat’s overall health, whether surgery is suitable, and whether the disease appears localized or has spread. Feline evidence is limited, especially for medical treatment, so there is no single plan that fits every cat.

What limited feline evidence says about medical treatment
A 2024 case report described one cat with severe chronic kidney disease that was not considered a surgical candidate. Frequent feeding and prednisolone were tried, followed by diazoxide. The report described improved glucose during the initial two months and glucose remaining above 50 mg/dL until the cat died of renal failure 161 days after treatment began. Vomiting and non-regenerative anemia were observed and might have been partly related to diazoxide. This is an observation in one cat, not proof of general efficacy, a dosing recommendation, or evidence that the treatment is safe for other cats.

Monitoring after surgery
After surgery, veterinary monitoring is important because glucose may become high, pancreatitis can occur, and persistent low glucose may point to residual disease. Prolonged hypoglycemia before treatment may also leave neurologic injury. ACVS describes hospitalization and rest after surgery; the 2023 feline surgical cohort recorded immediate postoperative complications.

Outcomes vary, and available survival figures come from a small, selected group of cats that underwent surgery. In the 2023 multi-institutional retrospective study of 20 surgically treated cats, median overall survival was 863 days. Reported survival in that same cohort was 75% at one year, 51% at two years, and 10% at three years. These population figures cannot predict an individual cat’s outcome or be applied to cats that were not surgical candidates.

In that cohort, excision was followed by immediate normal or high blood glucose in 18 of 20 cats, and 18 cats survived to discharge. Five of the 20 had immediate postoperative complications. Among the 18 discharge survivors, seven later had recurrent hypoglycemia-related signs; the median time to recurrence was 214 days. Metastasis at surgery and tumor invasion were among factors associated with poorer outcomes in the study.
These findings are useful for discussing possible outcomes with a veterinarian, not for estimating one cat’s life expectancy. The study was retrospective and small, and its participants were selected for surgery; a cat’s overall health, disease extent, response to treatment, and complications all matter.
