Is Your Cat Choking or Coughing? How to Tell and What to Do

Coughing can resemble choking or a hairball attempt, but breathing difficulty, gasping, open-mouth breathing, or abnormal gum color means call an emergency veterinary hospital now.

If your cat is struggling to breathe, gasping, breathing with its mouth open, or has blue, gray, or very pale gums, contact an emergency veterinary hospital now. Do not wait to decide whether the episode is choking, coughing, or a hairball attempt: breathing difficulty is an emergency either way.

Coughing can look like gagging, but an airway blockage can also cause noisy breathing and frantic distress. The appearance or sound alone cannot reliably distinguish them. Keep your cat calm, call ahead, and follow the veterinary team’s instructions while arranging prompt transport.

What to look for right now

Notice whether your cat can move air, whether breathing returns to normal between episodes, the gum color, and whether there is severe distress or collapse. These signs help you describe the situation to a veterinarian; they do not establish the cause.

Observation What it can mean for urgency
Open-mouth breathing, difficulty breathing, gasping, or breathing that stops Emergency veterinary help is needed immediately, whether the cause is choking or a respiratory problem.
Blue, gray, or white/pale gums Emergency sign. Call a veterinary hospital immediately.
Frantic behavior or pawing at the mouth Can occur with choking. Treat it as urgent, especially if breathing is affected.
Crouching, neck extended, followed by a dry or harsh cough or gag Can fit coughing, including cough mistaken for a hairball attempt; it does not rule out a serious breathing problem.
Visible object in the mouth Tell the veterinary team. If safe, you may try to remove only an object you can grasp without pushing it farther in or risking a bite.

Do not use “I can hear a sound, so it cannot be choking” as a rule. Choking may involve noisy breathing, and coughing or asthma can resemble obstruction. Gasping, abnormal gum color, collapse, or difficulty breathing calls for urgent help regardless of the sound.

How coughing and choking can look alike

Coughing and hairball-like retching

A cough is a forceful expiration and may sound dry or harsh. A cat may crouch and extend its neck, then gag. That posture can look like vomiting or an attempt to bring up a hairball. Asthma and bronchitis are among the conditions that can cause coughing and respiratory distress, so a cough-like episode is not automatically harmless. VCA’s guide to coughing in cats explains this presentation.

Possible airway obstruction

Choking is more concerning when airflow is compromised or the cat is in severe distress. Possible signs include frantic behavior, pawing at the mouth, noisy or difficult breathing, gasping, stopped breathing, and blue or white gums. A cat owner cannot safely confirm the cause from a sound or brief description alone.

What to do if choking or breathing distress is suspected

  1. Call an emergency veterinary hospital or your veterinarian immediately. Describe what you see and follow their directions. The American Veterinary Medical Association advises calling ahead so the hospital can prepare: AVMA, “First Aid for Your Pet” (2023).
  2. Keep your cat as calm as possible. Avoid unnecessary handling. A frightened or injured cat may bite or scratch, so do not put yourself at risk.
  3. If it is safe, look for an object you can see and grasp. Remove only an object that can be taken out safely. Do not probe blindly or push an object deeper into the throat.
  4. Arrange prompt transport to the veterinary hospital. Call ahead if possible and use a secure carrier for the trip. A carrier helps with transport; it does not treat choking or breathing distress.

Do not delay the call or trip to try home treatment. The American Red Cross describes cat-choking first-aid maneuvers, including back blows and abdominal thrusts, but these are for an apparent obstruction and require appropriate first-aid instruction. Do not practice them on a conscious, breathing cat. If an obstruction appears severe and you have been instructed in first aid, follow professional guidance while arranging veterinary care: American Red Cross cat and dog first aid.

Why a veterinary check matters even if the episode stops

Get veterinary assessment even if an object comes out or your cat seems normal again. The Red Cross notes that choking can lead to fluid in the lungs and recommends a veterinary visit even when the cat appears okay. The AVMA likewise cautions that first aid is not a substitute for veterinary care.
