Lactulose for Cats: Uses, Reference Dosage and Side Effects

Lactulose may soften stool or help reduce ammonia in cats, but dosing must follow the veterinarian’s prescription. Learn about administration, side effects and constipation risks.

Lactulose is a veterinarian-directed medicine that can soften stool in cats with constipation and, in some cases, help reduce blood ammonia associated with liver dysfunction. The dose depends on the cat and the reason for treatment: published reference doses are not a substitute for your cat’s prescription. Watch for digestive side effects, and contact your veterinarian about dose concerns, worsening symptoms or difficulty giving the medicine.

What lactulose does for cats

Lactulose is a synthetic sugar that acts as an osmotic laxative. It helps retain water in the bowel; acids produced when intestinal bacteria ferment it can also stimulate fluid secretion and movement in the colon. In cats, veterinarians may prescribe it to soften stool and manage constipation. It may also be used to lower blood ammonia in hepatic encephalopathy, a complication of liver dysfunction. VCA Animal Hospitals describes its use in cats as off-label, meaning the medication is used in a way that is not specifically FDA-approved for cats.

The Merck Veterinary Manual calls lactulose the most effective agent among the hyperosmotic laxatives it discusses. That describes its place within that drug group; it does not mean lactulose is right for every constipated cat or that it treats the underlying cause.

How much lactulose can you give a cat?

Use the dose on your cat’s prescription label. As a clinical reference—not an individualized prescription—Merck lists lactulose liquid at 0.5 mL/kg by mouth every 8–12 hours, or crystal granules at ¼–½ teaspoon by mouth or mixed into food every 8–12 hours. The formulation, condition being treated, response and veterinarian’s plan all matter. Do not use these reference figures to start, stop or change your cat’s treatment; ask the prescribing veterinarian if the label is unclear or you have a dosing question.

How to give lactulose

  • Measure liquid carefully with an oral medication syringe or another suitable measuring device. Do not estimate the amount by eye.
  • If your veterinarian says the liquid may be mixed with food, make sure your cat eats the entire portion so it receives the full dose.
  • Keep clean drinking water available.
  • Cats may dislike the taste. If giving the medicine is difficult, contact your veterinarian for advice rather than changing the dose or method on your own.

VCA says the medication usually begins to work within 1–2 days. Follow your veterinarian’s instructions for judging response; that timeframe does not guarantee when a particular cat will have a bowel movement or improve.

Side effects and safety cautions

Possible side effects include diarrhea, bloating, gas and abdominal cramps. At higher doses, lactulose may contribute to low blood potassium or high blood sodium. Call your veterinary office promptly if you suspect an adverse reaction or overdose.

Lactulose should not be used in pets with an intestinal obstruction or an allergy to the medication. It should be used cautiously in pets with diabetes, fluid or electrolyte imbalances, or pregnancy or lactation. For long-term treatment, the veterinarian may monitor electrolytes; blood glucose monitoring is advised for diabetic patients.

Tell the veterinarian about all medicines, supplements and herbal products your cat takes. VCA identifies antacids, gentamicin, other laxatives, neomycin and warfarin as medicines that require caution when used with lactulose. Do not add or stop a medicine without veterinary guidance.

Why constipation may need more than a laxative

Constipation can have different causes, including an intestinal foreign body, tumors or strictures, electrolyte disturbances, neuromuscular disease and medication effects. A laxative may help stool pass, but it does not identify or resolve every cause. Cornell’s Feline Health Center describes treatment as cause-directed; depending on the cat, veterinary care may involve hydration, laxatives, enemas, motility medicines, dietary changes or, in severe unresponsive cases, surgery.

Severe or recurring constipation and obstipation—an inability to pass stool—may require veterinary enemas or manual stool removal under anesthesia. Never give a cat a phosphate-containing enema: the Merck Veterinary Manual warns these can cause severe, potentially life-threatening electrolyte imbalances. Do not attempt an at-home enema unless your veterinarian has specifically directed and instructed you.

When to contact the veterinarian

  • Ask the prescribing veterinarian before changing the dose, stopping treatment or giving a missed dose if you are unsure what to do.
  • Contact the veterinary office if your cat is not responding as expected, constipation recurs, or giving the medicine is difficult.
  • Call promptly if your cat develops significant diarrhea or other suspected side effects, or if you think it received too much.
  • Seek veterinary advice for constipation rather than assuming it can be managed with a laxative alone; a blockage or another underlying problem may need a different treatment.
