The safest path is veterinary assessment, a feeding plan directed by the veterinarian, and a specialist’s evaluation of whether repair is feasible—not trying to feed or treat the puppy by guesswork.
1. What should you do first for a newborn puppy with a cleft palate?
Call your veterinarian as soon as you notice nursing difficulty, milk draining from the nostrils, or other signs of a problem. If the puppy has difficulty breathing, is markedly weak, or is not taking in nutrition, contact an emergency veterinarian immediately. These signs need prompt assessment; they are not a reason to wait and see whether the puppy improves.

Recognize the warning signs
A cleft palate is an opening between the mouth and nose. It occurs when the tissues separating those spaces do not join properly. Depending on its location, a cleft may affect the lip or nostril (a primary defect), the hard or soft palate, or both (a secondary defect). A puppy may have trouble swallowing or nursing, milk may come from its nostrils after feeding, or breathing may be affected. These signs can indicate a cleft or another serious problem; a veterinarian must examine the puppy to determine the cause. The American College of Veterinary Surgeons (ACVS) explains cleft types and signs, and the Merck Veterinary Manual describes how congenital mouth defects may be identified.
What the veterinary examination can establish
The veterinarian can examine the mouth, assess breathing and nutrition, and check for concerns such as aspiration pneumonia. Milk entering the airway can cause a respiratory infection, which needs prompt veterinary attention. The Merck Veterinary Manual says aspiration pneumonia should be identified and treated quickly. A photo cannot establish the defect’s extent or whether the puppy has aspirated, and there is no home method in these sources for closing the opening.

2. How can you feed a puppy that cannot nurse?
Ask the veterinarian to choose the feeding method and prescribe the amount for this puppy. Do not force milk or formula into the puppy’s mouth, and do not place a feeding tube unless a veterinarian has instructed and trained you to do so. A tube placed or used incorrectly can send liquid into the lungs; VCA’s general orphaned-puppy guidance warns that improper tube or gavage feeding can cause aspiration pneumonia or death. That guidance is not a cleft-palate-specific feeding protocol.
Why a tube may be part of the plan
For some puppies, a veterinarian may recommend feeding by an orogastric tube as a bridge while the puppy grows and surgical options are considered. ACVS says puppies and kittens are often tube-fed until roughly 3–4 months of age because repair is difficult in very young pets; a hard-palate cleft may also become smaller as the animal grows. These are descriptions of clinical management, not instructions for an owner to place a tube. The veterinarian should demonstrate any technique you are expected to use and explain how much and how often to feed.

Ask the veterinary team to explain what to do if the puppy cannot take the prescribed amount, develops new breathing signs, or becomes weaker. The plan should address both nutrition and the risk of milk entering the airway.

3. Can a cleft palate in a puppy be repaired?
Some clefts can be repaired surgically, but feasibility and timing depend on the defect and the puppy’s condition. ACVS recommends referral of secondary palate defects to a board-certified veterinary surgeon. Repair can be difficult in very young or underweight pets, and recovery carries risks, so the treating surgeon must decide whether and when this puppy is a candidate. ACVS outlines referral, surgery, and postoperative care.

Timing is decided case by case
The sources do not establish one age that applies to every puppy. The Merck Veterinary Manual says patients should be at least 12 weeks old for surgical correction and notes that some studies found higher success after 20 weeks or in adults. ACVS describes tube feeding until roughly 3–4 months as common while young pets grow. These are not contradictory guarantees or a universal schedule: the surgeon weighs the cleft, growth, body condition, and risks when recommending timing. Merck identifies continued growth, small patient size, and irritation from the tongue or food as factors that can contribute to repair failure. The Merck Veterinary Manual’s discussion of congenital mouth anomalies provides the age and risk context.
Plan for recovery and follow-up
After surgery, care may include soft or blenderized food, or tube feeding, for 2–4 weeks; an Elizabethan collar; and avoiding hard food or toys for at least a month. The surgeon sets the actual instructions for the puppy. Possible complications include the repair opening, nasal discharge or sneezing, and continued coughing or gagging. Report concerning signs to the surgical team rather than changing the feeding plan on your own. ACVS describes these postoperative measures and complications.

What affects a puppy’s outlook?
The outlook depends on the cleft’s size and location, the puppy’s overall condition, and whether aspiration or infection is present. ACVS describes the prognosis as excellent for small clefts, while repair is more difficult when more than half of the hard palate is involved; very large defects may need special dental appliances and tissue techniques. Merck notes that extensive soft-palate involvement carries a poor prognosis even with surgery. A veterinarian or veterinary surgeon must assess the individual puppy, and these sources do not provide a canine survival percentage that can predict its outcome. The Merck page’s figure of up to 67% survival applies to horses with congenital cleft palate, not puppies, so it should not be used to estimate a puppy’s chances. ACVS discusses prognosis by defect; the Merck Veterinary Manual distinguishes the prognosis for extensive soft-palate defects.
