Osteochondritis dissecans (OCD) is a developmental joint condition in which abnormal cartilage can crack or form a flap. It can cause pain and lameness, most often in growing dogs, and needs veterinary diagnosis. The shoulder is a common site, but the elbow, stifle (knee), and hock can also be affected. Treatment depends on the dog and the lesion; many clinically significant cases are treated surgically.

What happens in a joint with OCD?

OCD develops from osteochondrosis, a disorder of cartilage maturation and bone formation at a joint. Affected cartilage may become abnormally thick and weak, then crack or form a flap that extends into the joint. Debris and an uneven joint surface can irritate the joint lining and contribute to later degenerative joint disease. Merck Veterinary Manual and the American College of Veterinary Surgeons describe this process.

The condition is seen most often in immature medium-, large-, and giant-breed dogs. Its exact cause is not known. Rapid growth, excessive nutrition, trauma, and hereditary factors are suspected contributors, but no single factor is established as the cause. OCD should not be taken as proof that an owner caused a dog’s condition through feeding.

What signs might a dog show?

Lameness is common. A dog may favor a leg, bear less weight on it, or show pain when the affected joint is moved; swelling can also occur. Shoulder lesions often involve forelimb lameness, which may be more noticeable after exercise. These signs are not specific to OCD, and other orthopedic problems can look similar. A young dog with persistent limping or joint pain should be assessed by a veterinarian rather than diagnosed at home. ACVS and VCA Animal Hospitals outline signs and care considerations.

How do veterinarians diagnose OCD?

The veterinarian considers the dog’s history and performs an orthopedic examination, then chooses imaging based on the joint and the clinical picture. Radiographs are commonly used. CT can provide detailed cross-sectional views and help characterize a defect, particularly when radiographs do not show enough detail. Arthroscopy may help confirm the diagnosis and can also be part of treatment. No single imaging plan is appropriate for every dog. See the Merck Veterinary Manual and VCA guidance.

What treatment may be recommended?

The decision depends on the dog’s age and symptoms, lesion size and stability, joint location, imaging findings, and whether a flap or loose fragment is present. Veterinary management options are not interchangeable: rest may suit only a limited group of very young dogs with a crack or very small cartilage flap, while persistent lameness, a large defect, a folded flap, or a detached fragment may call for surgery. Merck describes osteochondrosis as usually treated surgically. The treating veterinarian can explain why a particular approach fits the dog’s lesion. VCA Animal Hospitals, Merck Veterinary Manual, and Veterinary Partner (VIN) discuss treatment considerations.

What does recovery involve?

After treatment, follow the veterinarian’s or surgeon’s activity restrictions and increase exercise only as directed. VCA describes short leash walks and range-of-motion exercises in the early weeks after surgery, followed by a gradual increase in controlled activity; this is an example of a source-specific plan, not a schedule for every dog. If a clinician prescribes confinement, a crate can help support that plan, but it does not treat OCD. The clinician should specify the crate size, confinement duration, and when activity can progress. Maintaining a healthy weight can also reduce unnecessary joint stress. VCA Animal Hospitals describes postoperative care.

How does the affected joint shape the outlook?

Prognosis varies with the joint, lesion, and other orthopedic problems. Merck describes shoulder outcomes as generally excellent, stifle outcomes as variable to guarded, elbow outcomes as guarded, and tarsal (hock) outcomes as poorest. These are broad descriptions, not guarantees for an individual dog; joint instability or other disease can worsen recovery. Your veterinarian can interpret the imaging and clinical findings for your dog. Merck Veterinary Manual summarizes prognosis by joint.
