Polymyositis is inflammation of multiple skeletal muscles. In dogs it is often considered an immune-mediated disease, and it can cause weakness, muscle pain, lameness, and muscle loss. Veterinarians assess the signs and may use blood tests, electromyography, and muscle biopsy to investigate it. Treatment usually involves prescription immunosuppressive medication, with dosing and tapering tailored to the individual dog. The outlook is generally favorable, but relapses can occur.

What polymyositis is and which dogs can be affected

Polymyositis is an inflammatory muscle disease affecting skeletal muscles, including muscles of the limbs. The Merck Veterinary Manual describes it as immune-mediated. It primarily affects adult dogs, but any breed may be affected. Newfoundlands, Boxers, and Vizslas are among breeds reported in association with the disease; that does not mean an individual dog of one of these breeds will develop it. The disease may begin suddenly or progress over time. Merck Veterinary Manual, reviewed February 2026.

Signs of polymyositis in dogs

Signs vary, and none alone confirms polymyositis. Weakness may be more noticeable after exercise. Other signs described by the veterinary reference include:

  • Lethargy or fever
  • Generalized weakness, sometimes with reduced stamina
  • Weight loss
  • Lameness that shifts between legs
  • Muscle pain or swelling
  • Muscle wasting, particularly in chronic disease

Regurgitation can occur when the esophagus becomes enlarged and loses normal function (megaesophagus). Involvement of muscles of the throat or larynx may lead to voice changes, difficulty swallowing, or difficulty breathing. These symptoms warrant prompt veterinary attention; breathing difficulty should be treated as urgent.

Polymyositis has been reported alongside myasthenia gravis, systemic lupus erythematosus, polyarthritis, and use of trimethoprim-sulfa antimicrobials. The Merck reference also notes that a subset of Boxers with autoimmune polymyositis developed round-cell tumors months later. This is a reported association, not evidence that polymyositis causes tumors or a prediction for a particular dog.

How veterinarians investigate polymyositis

Diagnosis starts with the dog’s history, examination, and signs. Because other conditions can also cause weakness or abnormal muscle enzymes, a veterinarian interprets test results in context. The available tests contribute different kinds of evidence:

Test What it can show
Blood tests Creatine kinase (CK) and aspartate aminotransferase (AST) activity may be increased, supporting suspicion of muscle injury or disease; these results alone do not confirm polymyositis.
Electromyography Can reveal abnormal spontaneous electrical activity in muscle.
Muscle biopsy The Merck Veterinary Manual describes biopsy as ideal for confirmation. Microscopic findings may include muscle-fiber damage, inflammation, phagocytosis, and regeneration.
MRI and immunohistochemistry These were used as part of the investigation in a published case report; they are not established as routine requirements for every dog.

Clear veterinary diagnostic criteria have not been established. A 2024 case report describes a five-year-old Poodle whose evaluation included elevated CK and AST, hind-leg muscle loss, MRI, tissue examination, and immunohistochemistry. It illustrates possible diagnostic tools, not a mandatory test sequence. Han et al., BMC Veterinary Research, 2024.

How polymyositis is treated

Treatment is directed by a veterinarian. The professional reference describes glucocorticoids such as prednisone as the usual treatment of choice, given at immunosuppressive levels until clinical remission and then tapered. The appropriate drug, dose, monitoring plan, and pace of taper depend on the dog and must be managed by its treating veterinarian; owners should not start, stop, or adjust these medicines without veterinary direction.

For severe or relapsing disease, or to maintain control while reducing steroid use, a veterinarian may consider another immunosuppressive medicine, such as azathioprine, cyclosporine, or cyclophosphamide. These prescription drugs require individualized oversight.

One 2024 case report described a dog treated with prednisolone and azathioprine. The dog’s clinical signs resolved, CK returned to normal, and hind-leg muscle thickness increased during four months of continued immunosuppressive treatment. A single case cannot establish that the same combination will work for every dog or define a universal protocol.

What to know about swallowing problems and supportive care

If regurgitation is linked to confirmed megaesophagus, the veterinarian may advise feeding adjustments, including elevated feeding, and—in severe cases—feeding-tube support. The right approach depends on the dog’s condition. An elevated feeding station or Bailey chair may be relevant for some dogs with megaesophagus, but it does not treat polymyositis itself. Tell the veterinarian about regurgitation, swallowing changes, or a changed voice. Seek urgent care for breathing difficulty, and follow the veterinarian’s guidance on feeding and aspiration risks.

Can polymyositis come back?

The Merck Veterinary Manual describes the prognosis as favorable, while noting that relapses can occur. The sources cited here do not establish a general relapse rate or a population-level recovery statistic, so an individual dog’s outlook depends on its condition and response to veterinary care. Merck Veterinary Manual, “Muscle Disorders in Dogs,” reviewed March 2018.
