Pyometra is a potentially life-threatening bacterial infection of the uterus and a veterinary emergency. Contact a veterinarian promptly if an intact female dog or cat seems ill, particularly after a heat, or has discharge, increased thirst or urination, vomiting, or a swollen or painful abdomen. Discharge may be absent when the cervix is closed, so its absence does not rule out pyometra.
What is pyometra?
Pyometra is an infection in the uterus, usually discussed in intact female dogs and cats. Bacteria infect the uterus and can cause severe illness. The American College of Veterinary Surgeons (ACVS) identifies E. coli as the organism most commonly cultured from affected uteri. Hormonal changes after heat can alter the uterine lining and make infection more likely. In dogs, Merck Veterinary Manual describes the progesterone-dominant period after heat as a factor that supports infection. Bacteria may ascend from the vagina, accompany a urinary tract infection, or enter through fecal contamination, according to ACVS.
What are the signs of pyometra?
Signs vary. Some animals show discharge as their most obvious sign; others become systemically ill without it. Possible signs include:

- Lethargy, depression, or reduced appetite
- Drinking more water and urinating more often
- Vomiting or diarrhea
- A swollen abdomen, abdominal discomfort, or weight loss
- Pale mucous membranes
- Pus or bloody vaginal discharge, sometimes visible beneath the tail or on bedding
Increased thirst and urination can occur when bacterial toxins affect the kidneys’ ability to conserve fluid. These signs are not specific to pyometra, but in an intact female pet they warrant prompt veterinary advice.
Open- and closed-cervix pyometra
With an open cervix, infected material may drain and appear as vaginal discharge. With a closed cervix, there may be no visible discharge: infected material remains in the uterus, which can enlarge, and the animal can worsen quickly. You cannot safely determine which form is present by watching at home. A lack of discharge is not a reason to wait.

When does pyometra occur?
Pyometra can affect intact dogs of different ages, although it is more common in older dogs. Dog-focused guidance from Merck says it is reported primarily in dogs over five and tends to occur four to six weeks after estrus. VCA gives a possible range of two to sixteen weeks after the last heat, with four to six weeks common. These are general dog-specific timing observations, not diagnostic cutoffs; do not wait for a particular interval before calling a veterinarian. Those timing estimates should not be treated as feline-specific. The available guidance here does not establish comparable cat-specific incidence figures or timing.

How do veterinarians diagnose pyometra?
A veterinarian considers reproductive history and examination findings alongside tests. Depending on the patient, evaluation may include blood work, urinalysis, abdominal imaging, and vaginal cytology. Blood and urine results help assess systemic illness and possible kidney effects, but do not diagnose pyometra by themselves.
- Ultrasound: Can show an enlarged, fluid-filled uterus and may reveal thickened uterine walls.
- Abdominal X-rays: May suggest uterine enlargement, but are not always definitive.
- Clinical interpretation: An enlarged uterus can have other causes, including pregnancy, so the veterinarian interprets imaging together with the history and examination.
Diagnostic approaches described by ACVS and Merck include blood chemistry, a complete blood count, urinalysis, radiographs, ultrasound, and, in some cases, vaginal cytology. ACVS: Pyometra Merck Veterinary Manual: Pyometra in Small Animals

How is pyometra treated?
For most affected pets, the usual recommended treatment is ovariohysterectomy: surgery to remove the ovaries and uterus. Because pyometra can cause severe systemic illness, the veterinary team may first stabilize the animal and treat infection, then proceed with surgery. ACVS and VCA describe intravenous fluids and antibiotics as part of care; the attending veterinarian determines the specific medicines, timing, and monitoring for each patient. VCA: Pyometra in Dogs

Medical management is not home care. Merck advises emergency surgery if there is no major improvement within 48 hours or the condition worsens. A veterinarian must decide whether this approach is appropriate and monitor the patient closely.

What is the prognosis?
Outcome depends on the animal’s condition and response to treatment. ACVS gives a possible survival prognosis of 80–100% after ovariohysterectomy when abdominal contamination is avoided and systemic infection is treated and responds appropriately. What should owners expect after surgery?
Follow the veterinary team’s discharge instructions, including prescribed medication and any follow-up plan. ACVS says antibiotics are generally prescribed for at least 10 days, but the individual veterinarian sets the plan. Protect the incision from licking or chewing; use a protective collar or other measure if the veterinarian recommends it.
ACVS advises avoiding normal play, stairs, and off-leash activity for the first two weeks after surgery. Follow your pet’s specific activity restrictions, since recovery instructions can vary with the patient and procedure. ACVS: Pyometra
Can pyometra be prevented?
Early spaying generally prevents pyometra by removing the ovaries and uterus. A less common exception is stump pyometra, which can occur if uterine tissue remains after an incomplete ovariohysterectomy and ovarian tissue or progestational hormones are involved. Discuss the timing and implications of spaying with your veterinarian, especially if your pet is intact or has a relevant medical history.
