Ringworm is a fungal infection, not a worm. For a small, ordinary-looking patch of skin, an over-the-counter antifungal used exactly as the label directs is the usual first step. Scalp infection, nail changes, widespread or severe disease, uncertainty about the diagnosis, and any rash that does not improve need assessment by a healthcare provider. This guide follows the U.S. Centers for Disease Control and Prevention (CDC) and American Academy of Dermatology (AAD) guidance written for people. If the concern is a dog, cat, or other pet, read the final section first.
What ringworm is and what it is not
Ringworm is caused by dermatophytes, a group of fungi that live on the outer layer of skin, hair, and nails. The name refers to the ring-shaped rash the infection often produces, not to a worm. The infection can affect the body, scalp, feet, groin, and nails, and the medical names differ by site. Athlete’s foot and jock itch are skin infections of the same type.
A rash that looks like ringworm is not the same as a confirmed ringworm diagnosis. Other skin and nail conditions can look similar. A clinician may examine the area and take a skin, hair, or nail sample, and the result helps decide which treatment is appropriate (CDC, Ringworm Basics; AAD, Ringworm: Diagnosis and treatment).
Find your situation first
The right route depends mostly on where the infection is and how much of the body it covers. The table below summarizes the CDC and AAD guidance.

Treating limited skin ringworm with an over-the-counter antifungal
CDC states that skin ringworm and related skin infections are usually treated with non-prescription antifungals, and it lists clotrimazole, miconazole, terbinafine, and ketoconazole among the options (CDC, Treatment of Ringworm). The guidance does not rank these ingredients against one another, so choose by form, comfort, and the product’s label. Terbinafine 1% cream is one commonly sold example.
- Confirm the area is ordinary skin, not the scalp, a nail, or the face in an area you cannot easily assess. If you are unsure, stop here and see a provider.
- Wash and dry the area gently before applying the product.
- Apply the antifungal in the amount and frequency printed on the label. Extend a little past the visible edge of the rash, as the label directs.
- Wash your hands after each application.
- Continue for the full period on the label, which the CDC describes as usually 2 to 4 weeks, even after the rash looks better.
- If the rash has not clearly improved by the end of the course, or it spreads, see a healthcare provider rather than extending treatment indefinitely.
Scalp ringworm needs prescription treatment
Scalp infection is the main case where home over-the-counter cream is the wrong tool. CDC says it usually requires prescription oral antifungal medicine for 1 to 3 months, and topical creams, lotions, and powders do not work for it. AAD’s scalp guidance mentions antifungal shampoo as something that can be used alongside treatment, and it also addresses household members. Shampoo supports the prescribed course; it does not replace it.

Nail changes are a provider question
Thickened, discolored, or crumbling nails have several possible causes, including previous injury. CDC advises contacting a provider and discussing testing before starting treatment for a suspected fungal nail infection. Prescription oral antifungal treatment is usually the most effective option, and treatment is measured in months, not weeks. An over-the-counter ringworm cream is not a substitute for that course.
Common mistakes that prolong ringworm
Using steroid creams on a suspected ringworm rash
Do not apply an over-the-counter corticosteroid cream to a rash that might be ringworm. CDC’s patient guidance is direct: “Do not use over-the-counter creams containing steroids, or corticosteroids, for ringworm or for a rash that has not been diagnosed.” CDC says steroids can make the infection worse, let it spread, intensify some symptoms, and change how the rash looks, which can make diagnosis harder.

Stopping treatment when the rash fades
A rash that looks better is not always cleared. The CDC and AAD both direct people to finish the full course. Stopping early can allow the infection to return or make it harder to clear.
Treating only the visible patch
If the infection is on more than one body area, treat each area under the applicable guidance at the same time. Keep the hand hygiene step in mind, because touching one site and then another can move the fungus.

Reducing spread at home
Ringworm spreads through contact with an infected person, an infected animal, and contaminated surfaces. The practical steps supported by CDC and AAD are simple:
- Wash hands with soap and water after touching or treating the affected area.
- Avoid sharing towels, clothing, combs, and other personal items with someone who has active infection.
- Keep the affected person’s bedding and clothing separate until the infection is controlled.

When to contact a healthcare provider
CDC recommends contacting a provider in these situations:
- The infection is on the scalp or a nail.
- The rash is unusually severe or covers a wide area.
- Symptoms do not get better after a full course of treatment.
- You are unsure whether the rash is ringworm.
- You had close contact with someone who has a severe infection.
- You have recently travelled. CDC advises sharing travel history because resistant ringworm has been reported emerging.
Ringworm in pets
The guidance above is written for people. Ringworm is also seen in dogs, cats, and other animals, and CDC notes that infected animals can pass the infection to people. Treatment of pets should be decided by a veterinarian, who can confirm the diagnosis and choose the product, dose, and duration for the species. Do not apply a human over-the-counter cream to an animal unless a veterinarian has told you to do so. If a household member develops a ringworm-like rash after contact with a pet, the person should see a healthcare provider and the pet should see a veterinarian.
