Swim Bladder Treatment for Aquarium Fish

Abnormal buoyancy in aquarium fish can have many causes, so test water quality and provide supportive care before considering medication. Learn how to feed safely, avoid common remedies that do not address the cause, and recognize when a veterinarian is needed.

Swim bladder disease is not one specific illness; it describes abnormal buoyancy, such as floating, sinking, rolling, or swimming sideways. Treatment depends on the cause, so test the aquarium water and observe the fish before changing its diet or adding medication.

What causes buoyancy problems in aquarium fish?

The swim bladder can be affected by several problems, and apparent swim bladder cases may have another cause. Possible causes include:

  • Ammonia or nitrite exposure
  • Poor nutrition, overeating, or gastrointestinal gas
  • Air gulped while surface-feeding
  • Infection or inflammation
  • Injury or spinal deformity
  • Neurological disease
  • A tumor, organ enlargement, or other internal condition
  • Permanent anatomical problems, especially in some fancy goldfish

A fish that is weak, breathing heavily, or resting on the bottom may have a serious water-quality or systemic illness rather than an isolated buoyancy disorder. Treating the wrong cause can delay effective care.

1. Test the aquarium before treating the fish

Clear water is not necessarily safe water. Test the aquarium, preferably with reliable liquid test kits, for:

Parameter General freshwater reference Why it matters
Total ammonia nitrogen 0 mg/L Ammonia damages gills and can cause weakness and abnormal behavior.
Nitrite 0 mg/L Nitrite interferes with oxygen transport in the blood.
Nitrate Below 20 mg/L Long-term accumulation stresses fish.
Dissolved oxygen Above 5 mg/L Low oxygen can look like lethargy or loss of swimming control.
pH 6.5–9.0 as a broad reference Species-specific stability matters more than chasing a particular number.
Temperature Species-specific Sudden changes can worsen stress and illness.

These are general freshwater reference ranges from the Merck Veterinary Manual, not substitutes for the particular species’ needs. For marine or brackish fish, also check salinity. If pH is unstable, test alkalinity.

If ammonia or nitrite is detectable, test daily and address the aquarium problem. Treat water for both chlorine and chloramine before changes. Check the filter, remove decaying material, reduce feeding temporarily, and make appropriate partial water changes.

Important exception: if the tank has very low pH and high ammonia, do not abruptly raise the pH or make a drastic correction without guidance. A rapid pH increase can convert less-toxic ammonia into more-toxic un-ionized ammonia. The Merck Veterinary Manual recommends careful correction with small daily changes in this situation.

2. Provide supportive care based on the fish’s position

If the fish floats at the surface

A fish that remains partly out of the water can develop drying, irritation, loss of protective mucus, and secondary infection. Keep the water clean and oxygenated, reduce strong currents, and make food easy to reach.

Do not cover the aquarium to force the fish underwater; this can reduce oxygen exchange and does not solve the underlying problem.

Do not attach homemade floats, weights, “life jackets,” or wheelchairs. These devices can rub away mucus, injure the skin, and create wounds that become infected.

If the fish sinks or lies on the bottom

Constant contact with the tank bottom can cause abrasions, particularly on the belly and sides. A smooth, non-abrasive resting area can help. Depending on the aquarium, that may mean a bare-bottom hospital tank or smooth glass beads. Avoid rough gravel or abrasive sand where the fish rests.

Keep the bottom especially clean. Waste trapped beneath a stationary fish can irritate damaged skin and worsen water quality.

3. Adjust feeding carefully

Some fish, including goldfish and koi, are physostomous: their swim bladder is connected to the digestive tract. Gulping air at the surface can contribute to floating or gastrointestinal gas in some individuals. A sinking or neutrally buoyant diet may help reduce air intake.

  1. Offer small portions rather than a large meal.
  2. Consider sinking or neutrally buoyant food for fish that worsen after surface feeding.
  3. Remove uneaten food promptly.
  4. Target-feed a fish that cannot compete with tankmates.
  5. Bring food to the fish rather than grabbing or restraining it.

A sinking food can reduce air intake, but it is not a cure for every buoyancy problem. Fish that cannot reach food may need supervised or hand-feeding; bring food to them rather than restraining them.

Do peas treat swim bladder disease?

Peas are often presented as a universal remedy, but they are not. A pea may be useful mainly because it sinks and can be a suitable food for some fish; it is not a proven swim-bladder treatment. Peas cannot correct a deformity, tumor, spinal problem, neurological disease, traumatic injury, or confirmed infection.

If abnormal buoyancy persists after sensible feeding and water-quality corrections, stop repeating home remedies and investigate the cause.

4. Do not medicate blindly

There is no single “swim bladder medicine.” Antibiotics are appropriate only when bacterial disease is supported by examination and diagnostic findings. Unnecessary antibiotics can stress the fish and interfere with biological filtration.

Medication choice also depends on where the problem is located. Bath treatments are generally more useful for external infections, while medicated food or, under veterinary direction, injection may be appropriate for internal infections. Accurate bath dosing requires the actual water volume, not the aquarium’s advertised capacity.

Do not assume aquarium salt, Epsom salt, antifungal products, antiparasitic drugs, or products marketed for swim bladder problems will help. Salt can be unsuitable for some species and plants, while Epsom salt is not a cure for an unknown buoyancy disorder. Use any treatment only when the suspected cause and the fish’s species tolerance justify it.

When to contact an aquatic veterinarian

Arrange veterinary advice when the problem is severe, persistent, recurrent, or accompanied by other signs of illness. Seek help promptly if the fish is:

  • Breathing rapidly or struggling to breathe
  • Not eating or unable to reach food
  • Bloated, bleeding, ulcerated, or covered in abnormal growths
  • Showing abnormal coloration or severe lethargy
  • Unable to remain submerged or upright
  • Affected at the same time as several other fish

An aquatic veterinarian may review the feeding routine and aquarium history, examine the fish, assess how many fish are affected, and recommend radiographs or other imaging. X-rays can reveal an enlarged, displaced, misshapen, or fluid-filled swim bladder, as well as spinal abnormalities, masses, or other internal disease. Surgery to repair a gas bladder or remove a compressive mass is a veterinary procedure, not a home treatment.

Can a fish live with permanent buoyancy problems?

Yes. Some fish do well despite never returning to a normal swimming posture. Structural problems are particularly common in certain fancy goldfish because their rounded body shape and spinal curvature leave less room for normal swim-bladder function. Permanent changes can also follow spinal or neurological problems.

Focus on quality of life rather than forcing the fish into a normal position:

  • Maintain stable, excellent water quality.
  • Provide a smooth place to rest.
  • Remove sharp ornaments and abrasive substrate.
  • Position food where the fish can reach it.
  • Use supervised or targeted feeding when necessary.
  • Reduce competition and strong currents.

The practical goals are preventing skin injury, ensuring adequate nutrition, and minimizing stress.

Common mistakes to avoid

Claim What is more accurate
“Swim bladder disease is one disease.” It is a symptom or syndrome with many possible causes.
“Every floating fish needs antibiotics.” Floating can result from air intake, gas, anatomy, water quality, injury, or nonbacterial disease.
“Peas cure it.” Peas may be a suitable sinking food for some fish, but they do not repair structural or infectious problems.
“A homemade float is harmless.” Attachments can damage skin and mucus and should not be improvised.
“Covering the tank will keep a floating fish safe.” Covering the tank to force submersion can reduce oxygen availability and is discouraged.
“The water looks clean, so it is safe.” Water quality must be measured; visual clarity does not establish safety.

How do I treat swim bladder disease in a fish?

First test and correct the aquarium water, then provide clean, well-oxygenated water, a low-stress setup, safe access to food, and a smooth resting surface if needed. Adjust feeding according to the species and buoyancy pattern. Medication is appropriate only when a specific cause is identified or supported by a veterinarian.

Should I feed peas to a fish with swim bladder problems?

Peas are not a universal cure or a proven swim-bladder treatment. They may provide a sinking food for some fish, but they will not correct deformity, injury, infection, neurological disease, or an internal mass. Persistent symptoms need investigation rather than repeated pea-feeding.

Should I use aquarium salt or Epsom salt?

Not automatically. Salt products do not treat every cause of abnormal buoyancy and may be unsafe for some fish, plants, or invertebrates. Use them only when the suspected condition, dose, and species tolerance justify the treatment.

Can a fish recover from a swim bladder problem?
