A heartworm-positive dog can sometimes be moved when relocation cannot wait, but only after a veterinarian has assessed that individual dog. The American Heartworm Society (AHS) and the Association of Shelter Veterinarians (ASV) say dogs with clinical signs attributed to heartworm infection should not be transported. They also say the preferred sequence is treatment and recovery first, travel second. If the move can’t be delayed, the plan, the timing and the paperwork all need to be settled with a vet before departure.
The short version
- Symptomatic dogs stay put. AHS/ASV: “Dogs with clinical signs attributed to heartworm infection should not be transported.”
- Test close to departure. Dogs older than six months should be tested for both heartworm antigen and microfilariae no more than 30 days before relocation.
- Wait after melarsomine. The AHS/ASV relocation document calls for at least four weeks of recovery after melarsomine before transport.
- Treatment continues after arrival. Heartworm treatment should be completed according to AHS guidelines as soon as possible after safe transport.
- Destination rules apply. In the U.S., the receiving state or territory sets entry requirements.
Is it advisable to move a dog mid-treatment?
Ideally, no. Brian DiGangi, DVM, DABVP, Senior Director of Shelter Medicine at the ASPCA, writes in the AHS FAQ “Safe Transport of Heartworm-Positive Dogs”: “In an ideal world, a dog like this would first undergo heartworm treatment and post-treatment recovery before being transported, but ‘ideal’ is not how many such cases are described.”
That is the practical position. Postponement is best. When a move is truly unavoidable, such as an evacuation, a shelter transfer or a household relocation, the decision should weigh the individual dog’s welfare and status and the risk of spreading infection along the way. No transport-specific outcome rate or quantified transmission risk is available in the AHS/ASV materials, so the guidance rests on expert recommendation rather than measured numbers.

These are recommendations, not population statistics. The two-negative-test step is not proof that a dog is cured, and it applies within a specific branch of the algorithm. Follow the whole algorithm with your vet rather than lifting a single step.

Planning the move, step by step
1. Establish current health and infection status
Ask the vet for an exam and for antigen and microfilaria tests dated within the 30-day window. The guidance does not support treating an untested dog as negative. When ownership changes, test results and any preventive or treatment history should go with the dog.
2. Decide whether to postpone
Clinical signs attributed to heartworm disease rule out transport. For a dog that has had melarsomine, the four-week recovery interval exists because stress and exertion during recovery may worsen complications. If delay is possible, treating and recovering first is the preferred route.

3. Build the plan with a veterinarian
The AHS/ASV algorithm branches by antigen and microfilaria findings. Depending on the branch, it discusses approved macrocyclic lactone preventives, topical moxidectin, mosquito-control products and doxycycline. Choosing products, timing, contraindications and monitoring is the treating veterinarian’s job, and this article is not a dosing guide.
One practical point from the guidance: avoid giving the first dose of doxycycline on the day of transport. If the dog becomes nauseated, it is then easier to tell a drug reaction from travel stress. The AHS FAQ notes that doxycycline timing can be tailored to the situation.

4. Continue care at the destination
After safe arrival, complete treatment per AHS guidelines as soon as possible. Send test results and treatment records to the receiving veterinarian or organization so nothing is lost in the handoff, including any treatment already started and the date of any melarsomine injections.
Legal requirements for U.S. travel
USDA APHIS (page “Take a Pet From One U.S. State or Territory to Another (Interstate)”, last modified June 10, 2026) states that the receiving state or territory sets domestic entry requirements. Depending on the destination, these may include a health certificate, updated vaccinations, diagnostic testing or treatment. APHIS advises seeing a local veterinarian early and contacting the destination’s state animal health official for clarification. APHIS also regulates businesses that transport pets on behalf of owners. AHS/ASV separately tells veterinarians to follow state and federal transport regulations.

No single universal certificate rule applies. Check the actual origin, destination, travel mode, carrier and travel date. Airlines and ground transporters may have their own conditions on top of state rules.

- The receiving jurisdiction’s and carrier’s requirements
No product makes transport of a positive dog safe. A crate or carrier is ordinary travel equipment. It doesn’t replace an exam, a veterinary plan or a check of destination rules.
