Straining, frequent litter-box trips, or blood in a cat’s urine do not by themselves mean a urinary tract infection. These signs have several possible causes, and bacterial infection is only one of them. A veterinarian usually sorts them out with a urinalysis and, when bacteria are suspected, a urine culture. The one pattern that cannot wait is a cat that strains and passes little or no urine, which can signal a blocked urethra.
What “UTI” Means in Cats
Owners often use “UTI” for any painful, frequent, or bloody urination. Veterinarians usually describe this group of symptoms as feline lower urinary tract disease (FLUTD) or lower urinary tract signs (LUTS). A bacterial urinary tract infection is one possible cause within that group, not a synonym for it.
The most common alternative explanation for many cats is feline idiopathic cystitis (FIC), an inflammation of the bladder with no bacterial cause found. Cornell University College of Veterinary Medicine, the 2019 ISCAID guidelines, and the Merck Veterinary Manual all describe idiopathic cystitis as a frequent diagnosis among cats with lower urinary signs, particularly younger cats.
Signs That Need a Veterinary Visit
Cornell lists the following as typical lower urinary tract signs. None of them, on its own, tells you whether bacteria are involved:

- Painful or difficult urination, including straining
- Urinating more often than usual
- Crying or vocalizing in the litter box
- Blood in the urine
- Urinating outside the litter box
- Frequent licking of the genital area
Any of these warrants a call to your veterinarian, who can decide whether examination, urinalysis, or further testing is needed.
Urethral Obstruction: The Emergency Pattern
A cat that makes repeated trips to the litter box, passes little or no urine, becomes increasingly distressed, or vocalizes while straining may have a urethral obstruction. Cornell University College of Veterinary Medicine states: “A urethral obstruction is a true medical emergency, and any cat suspected of suffering from this condition must receive immediate veterinary attention.”

Male cats, including neutered males, are at higher risk because their urethra is longer and narrower. Treatment usually involves catheterization, often under sedation or anesthesia. Depending on the case, follow-up may include fluid therapy and hospitalization. Relieving a blockage at home is not safe, and waiting to see whether a suspected obstruction clears on its own is not an option.

Who Is More Prone to Bacterial Cystitis
Healthy adult cats are relatively resistant to bacterial urinary infections. The Merck Veterinary Manual identifies older age, compromised immunity, long-term steroid therapy, and systemic illnesses including diabetes and thyroid disease as factors associated with bacterial cystitis. Having one of these risk factors raises the likelihood of infection, but it does not prove that a cat’s symptoms are caused by one.
How the Diagnosis Is Made
A typical workup follows these steps:
- History and physical examination. The veterinarian asks about symptoms, litter-box habits, and any illnesses or medications, then examines the cat.
- Urinalysis. This is the usual initial test.
- Further testing if the cause is unclear. Options include urine culture, blood tests, radiographs, or abdominal ultrasound.
- Urine collection for culture. Cystocentesis, a needle collection directly from the bladder, is the preferred method when feasible, according to the 2019 ISCAID guidelines and the Merck Veterinary Manual.
The ISCAID guidelines say bacterial cystitis should be confirmed by aerobic bacterial urine culture in cats, because it is relatively unlikely among cats presenting with lower urinary tract signs. Bacteria found in urine without clinical signs do not automatically require antibiotic treatment, which is why culture results need to be interpreted alongside the cat’s symptoms.

Age and the Bacterial Share of Cases
Cornell Feline Health Center’s frequent-UTI explainer, “Are These Frequent Urinary Tract Infections?”, reports that bacterial causes account for 1–3% of lower urinary tract disease cases in young to middle-aged cats and about 10% in cats over 10 years old. The page does not give a publication date. These figures describe the populations Cornell cites, and they are not universal prevalence estimates for all cats or all settings.
Treatment Depends on the Cause
There is no single treatment for “a UTI in a cat.” The approach follows the diagnosis.

Bacterial cystitis
Antibiotics are for infections confirmed by the veterinarian, and the drug is selected by the veterinarian. Lower urinary symptoms alone do not justify assuming infection. ISCAID distinguishes bacterial cystitis from nonbacterial causes and advises against treating bacteriuria without clinical signs as if it always needs antimicrobial treatment, a point that matters for antibiotic stewardship.
Feline idiopathic cystitis
Merck describes the management goals for FIC as reducing severity and recurrence. Measures it lists include lowering environmental stress, improving litter-box conditions, paying attention to interactions between cats in the household, dietary changes including canned food, pheromones, and analgesia for pain. Canned food is a supportive, veterinarian-guided measure for FIC. It is not a treatment for bacterial infection.
Stones and urethral plugs
Bladder stones and urethral plugs need a different veterinary treatment plan than bacterial cystitis or FIC, and the sources reviewed do not describe the specific approach. Your veterinarian will decide on the plan based on the findings from the examination and testing.
Urethral obstruction
Obstruction is treated as an emergency, as described in the section above. Catheterization, sedation or anesthesia, and hospital care are the usual components.

- Do not give leftover antibiotics from a previous illness or another animal.
- Do not treat an undiagnosed urinary problem with a food product or supplement and assume that will resolve it.
- Do not attempt to unblock a cat’s urethra at home.
- Do not wait to see whether a suspected obstruction clears.
If your cat has any of the signs described above, book a veterinary visit so the cause can be identified before treatment starts.
