What Health Problems Are Common in Flat-Faced Cats?

Flat-faced cats may face breathing, eye, dental and facial-fold problems, with risk varying by facial shape. Here is what owners should watch for and when to seek veterinary advice.

Flat-faced cats can be more prone to breathing difficulty, eye problems, dental crowding and disease, and irritation or infection in facial skin folds. The risk depends partly on how pronounced the cat’s facial flattening is; a breed name alone does not predict an individual cat’s health. Persian cats may also have coat and kidney concerns associated with the breed, but those are not established as effects of a flat face.

What flat-faced means—and why severity matters

Brachycephalic means short-headed or flat-faced. It describes a range of facial shapes rather than a single anatomy: cats can have a mildly shortened muzzle or a much more extreme one. Persians, Burmese, Himalayans, British Shorthairs and Exotic Shorthairs are among the breeds in which flat faces may occur, but individual cats do not all have identical features or risks.

The concern is not appearance by itself. A shortened skull can occur alongside changes to internal structures, including those of the upper airway, eyes and mouth. The UK government’s opinion on feline breeding practices explains: “Brachycephalic skulls are characterised by a mildly to profoundly shortened muzzle and there are concomitant changes to internal upper airway structures, which together can restrict airflow and cause breathing difficulties.” Its welfare review particularly highlights serious risks where brachycephaly is severe.

No single prevalence figure establishes how often every listed problem occurs across all flat-faced cats. The strongest numerical evidence available here concerns Persian cats seen in UK primary-care veterinary practices, not every breed or every cat with a shortened muzzle. Clinical records also may not capture every condition.

Breathing problems and heat tolerance

Changes accompanying a shortened muzzle can narrow the route air takes through the nose and throat. The UK government review identifies narrow nostrils, narrow nasal passages and/or a relatively long soft palate among abnormalities associated with brachycephalic obstructive airway syndrome (BOAS) in flat-faced cats. Restricted airflow can make breathing and exertion harder.

Cats Protection lists snorting or snoring, unusual sleeping positions and struggling during activity as signs owners may notice. Noisy breathing should not automatically be written off as harmless or simply normal for a breed: it can be a reason to discuss the cat’s breathing with a veterinarian, particularly if it is new, worsening or accompanied by difficulty coping with activity.

Heat adds another reason for care. Cats Protection advises providing access to water and a cooler place to rest in hot weather. If breathing is a concern, keep play short and gentle, and stop activity that makes the cat pant or struggle. A veterinarian can advise how to manage exercise and heat for the individual cat; these precautions are not a treatment for an airway disorder.

When describing a breathing concern to the veterinarian, note what you have observed and when it happens—for example, during activity, while sleeping or in hot weather. Snoring or snorting is a sign to pay attention to, not a diagnosis by itself. Conversely, a cat that looks flat-faced should not be assumed to have BOAS without veterinary assessment. Facial appearance alone cannot establish whether airflow is restricted or how serious a problem may be.

Eye irritation, exposure and tear drainage

Prominent eyes and shallow eye sockets can make it harder for a cat to close its eyelids fully, leaving the cornea more exposed. Changes to tear drainage can also contribute to discharge that persists. These are practical health concerns, not merely cosmetic features of a flat face.

Cats Protection identifies squinting, sensitivity to light, pawing at the eyes, redness, cloudiness and watering as signs that merit veterinary advice. Persistent or worsening discharge should also be assessed rather than assumed to be an unavoidable breed trait. Severe corneal ulceration may require an operation, according to Cats Protection’s guidance; only a veterinarian can assess the cause and appropriate care.

For owners, the useful distinction is between noticing an eye feature and noticing a change or symptom. Prominent eyes may be part of a cat’s appearance, but squinting, cloudiness, redness, discomfort or ongoing watering are reasons to seek advice. The Persian study’s 5.8% figure for recorded ocular discharge is one finding from its clinical-record population; it does not measure the full range of possible eye conditions or describe every flat-faced cat.

Dental crowding and problems eating

A shortened facial structure may leave less room for the teeth or affect how they line up. Crowding or malocclusion can make biting and chewing difficult and contribute to dental disease. The shape of the face does not tell an owner whether a particular cat has painful disease, so regular dental assessment matters.

Signs Cats Protection associates with dental problems include swollen or bleeding gums, bad breath, difficulty eating or grooming, and weight loss. A veterinarian should assess suspected mouth pain or difficulty eating. Dental-care products may support routine care, but they cannot replace an examination or treatment when disease is present.

Changes in eating or grooming can be important observations even if the teeth are difficult to inspect at home. A cat may show a problem through difficulty eating, while gum swelling or bleeding and bad breath are additional signs listed by Cats Protection. These observations do not identify the exact cause; an examination is needed to determine whether dental disease or another issue is present. In the Persian study, periodontal disease was recorded in 11.3% of the study population, but that figure is not a prevalence estimate for all brachycephalic cats.

Facial skin folds: irritation and infection

In cats with more severe brachycephaly, excessive facial folds can trap moisture or become irritated, inflamed or infected. Cats Protection advises checking the skin and keeping it clean and dry, and seeking veterinary advice if discomfort is suspected.

There is no single non-prescription cream or cleanser established as suitable for every cat in the guidance discussed here. If a fold looks inflamed or the cat seems uncomfortable, ask a veterinarian what care is appropriate instead of applying a product without advice. Checking the folds helps owners notice a change, but it does not replace veterinary assessment when the skin is sore or appears infected.

Persian-specific findings: coat and kidney concerns

Some health concerns reported in Persian cats should be kept separate from problems more directly linked to facial conformation. A UK study of Persian cats in primary-care veterinary records found haircoat disorders to be the most commonly recorded specific disorder in its population. Its discussion describes the breed’s long, dense coat and associated dermatological concerns. This is evidence about Persians, not proof that coat disorders are caused by a flat face.

The same study discusses Persian predisposition to renal and urinary disorders, including autosomal dominant polycystic kidney disease (ADPKD). Those breed-associated concerns are distinct from airway restriction, eye exposure, dental crowding and facial-fold problems. The study’s recorded figures are useful context, but they do not predict what will happen to an individual cat.

Keeping these categories separate matters: a health concern may be associated with a breed without being caused by that breed’s facial shape. The study also recorded overgrown nails among its findings, but that does not make nail overgrowth a specific consequence of brachycephaly. Its results are a picture of one Persian primary-care population, not a checklist of conditions every Persian—or every flat-faced cat—will develop.

What the Persian study’s numbers do—and do not—show

The figures below come from VetCompass researchers’ 2019 study of Persian cats under primary-care veterinary care in the UK. They are proportions of conditions recorded in that study population, not estimates for all flat-faced cats. The researchers caution that routine-care records may not capture every condition.

Finding in the Persian study Recorded figure How to interpret it
Haircoat disorders 12.7% Most commonly recorded specific disorder in the study population.
Periodontal disease 11.3% A recorded dental finding in the Persian study; not a rate for every flat-faced breed.
Overgrown nails 7.2% A recorded condition in that primary-care population.
Ocular discharge 5.8% A recorded eye-related finding; it does not capture every possible eye problem.
Median longevity 13.5 years; interquartile range 9.9–16.0 years A Persian study result, not a universal lifespan prediction for flat-faced cats.
Renal disease among grouped recorded causes of death 23.4% The share of grouped causes of death recorded in the study—not the share of all flat-faced cats that die of renal disease.

The study reported coat, ocular and dental disorders as prominent in its Persian population. It did not identify some previously reported respiratory, reproductive and neurological concerns among its top recorded disorders. That absence in one retrospective clinical dataset does not establish that those concerns never occur.

Each number answers a limited question: what was recorded in that study population. For example, 23.4% refers to renal disease as a share of grouped causes of death recorded in the study; it does not mean that nearly one in four flat-faced cats will die of renal disease. Likewise, the median longevity of 13.5 years is a statistic for the Persian study population, not a promised lifespan or a comparison with every other breed. Differences in breed, country, veterinary attendance and record-keeping limit wider conclusions.

How owners can monitor a flat-faced cat

Routine veterinary care is useful because appearance alone cannot diagnose an airway, eye, mouth or skin condition. Cats Protection recommends regular veterinary checks, monitoring breathing and weight, arranging dental checks, and checking the eyes and skin. A veterinarian can tailor the frequency and type of monitoring to the cat’s health and facial shape.

  • Notice changes in breathing, sleep position or ability to manage activity, and describe when they occur to the veterinarian.
  • In hot weather, make sure the cat can reach water and a cooler resting place; avoid activity that prompts panting or struggling.
  • Check for eye changes and examine facial folds for discomfort or inflammation as part of ordinary handling, without treating a persistent problem as cosmetic.
  • Keep up with veterinary dental checks, especially if eating or grooming changes.

Seek veterinary advice for breathing difficulty, persistent or worsening eye discharge, squinting or a cloudy eye, mouth pain or trouble eating, or inflamed or infected facial folds. These signs may have different causes, so there is no single home treatment that fits them all. A veterinarian can assess the individual cat and advise on appropriate care.

Frequently Asked Questions

Do flat-faced cats have breathing issues?

They can. A shortened muzzle may occur alongside narrow nostrils, narrow nasal passages or a relatively long soft palate, which can restrict airflow. Snorting or snoring, unusual sleeping positions and struggling during activity are signs to discuss with a veterinarian; noisy breathing should not automatically be treated as normal for the breed.

Do flat-faced cats have eye conditions?

They may be more prone to eye irritation or injury because prominent eyes and shallow sockets can make full eyelid closure harder, and altered tear drainage can cause persistent discharge. Squinting, light sensitivity, pawing, redness, cloudiness or ongoing watering merit veterinary advice.

Do flat-faced cats have dental issues?

A shortened facial structure may leave less room for teeth or affect their alignment, contributing to crowding, malocclusion or dental disease. Swollen or bleeding gums, bad breath, trouble eating or grooming, and weight loss are signs to discuss with a veterinarian.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Do flat-faced cats have skin issues?

Cats with more severe brachycephaly may have excessive facial folds that become irritated, inflamed or infected. Check the skin and keep it clean and dry; seek veterinary advice if the cat seems uncomfortable or the folds look inflamed. Do not assume a particular cream or cleanser is suitable for every cat.

Does every flat-faced cat develop these problems?

No. Facial shape and severity vary, and a breed name or appearance alone cannot diagnose a condition or predict an individual cat’s health. The UK Persian study’s figures describe recorded findings in that population and should not be generalized to every flat-faced cat.

The Bottom Line

Flat-faced cats may face breathing, eye, dental and facial-fold problems, especially when facial flattening is more pronounced. Watch for changes rather than dismissing symptoms as breed traits, and seek veterinary advice for breathing difficulty, persistent eye symptoms, mouth pain or inflamed skin. Persian coat and kidney findings are breed-associated concerns, not established effects of a flat face.
