A 4-month-old kitten is still growing, learning and building habits—not ready for adult-cat care. Plan on kitten-formulated food, daily play, a safe home setup and a veterinary review of vaccines, parasite prevention and spay or neuter timing. Your kitten’s records, health and local requirements determine the exact plan.
What is normal at four months?
Expect plenty of activity and curiosity. Kittens this age commonly explore, stalk, pounce and play as they practise coordination and hunting-like behaviors. Temperament and development vary with health, genetics, early experiences and surroundings, so one kitten’s behavior or size is not a reliable standard for all.

Four months is also a useful time to make gentle handling and everyday routines familiar. The 2021 AAHA/AAFP Feline Life Stage Guidelines recommend positive reinforcement and gradual practice with handling, grooming, nail care and carrier visits. Keep sessions brief and reward calm cooperation rather than forcing it.

Scratching is normal feline behavior. Offer appropriate scratching surfaces in places your kitten spends time; redirect to those surfaces rather than punishing scratching.

What should I feed my kitten?
Choose a nutritionally complete food formulated for kittens and follow its feeding guidance as a starting point. The Merck Veterinary Manual says growth-formulated food should generally continue until adulthood, around 9–12 months. The University of Sydney Veterinary Teaching Hospital likewise emphasizes a complete kitten diet during the first year.
The Royal Veterinary College’s kitten feeding guide places kittens aged 3–6 months at 2–3 meals a day. This is a general range, not a requirement that every kitten eat exactly three meals. Portion size depends on the food’s energy density and your kitten’s body condition, growth, activity and neuter status. Feed in a quiet place, keep fresh water available and ask your veterinarian to assess body condition and portions, particularly after neutering. Avoid treating a generic calorie estimate as an individualized prescription.

What veterinary care should I plan now?
Gather records and book a visit
If your kitten has just come home, the AAHA new-kitten guide advises scheduling a first veterinary visit within the first week. Bring adoption, shelter, breeder and previous veterinary records. If your kitten is already established with a clinic, arrange a visit to review what is due and ask the practice how soon to come in.

Ask the veterinarian to review vaccine doses already given, any boosters still needed, whether a fecal test or parasite treatment is appropriate, and how your kitten’s health and lifestyle affect prevention. Merck describes kitten visits recurring every 3–4 weeks until about six months for vaccines and parasite treatment, but that is broad guidance rather than a universal schedule. The University of Sydney says vaccines commonly start at 6–8 weeks and are repeated every 2–4 weeks until at least 16 weeks. The right plan depends on prior doses, vaccine product, health, exposure risks and local rules, including rabies requirements. Use your veterinarian’s plan rather than trying to reconstruct a schedule from age alone.

Does my indoor cat need parasite prevention?
Indoor living does not automatically eliminate parasite risk. Ask the veterinarian which prevention or testing is appropriate for your location and household. Any product must be approved for the kitten’s age and weight and suited to local risks; do not choose a medication, dose or schedule without veterinary direction.
When should my kitten be spayed or neutered?
Bring up timing at the next veterinary appointment rather than waiting until the kitten is older by default. The AAHA owner guide says many veterinarians recommend completing the procedure by five months, and its new-kitten checklist advises scheduling it before five months. The Royal Veterinary College advises early neutering from four months for suitable kittens. These recommendations make this a good time to discuss options; the veterinarian must assess your individual kitten’s suitability and follow local practice.

Should I ask about a microchip?
AAHA recommends considering microchipping at the first visit or soon after. A microchip is permanent identification, not a GPS tracker. If your kitten is chipped, keep the registry’s contact details current.

For veterinary trips, choose a secure, ventilated carrier that is easy to clean and appropriately sized, with room for the kitten to grow. Make it familiar by leaving it open at home with soft bedding, treats or toys, as AAHA recommends. Reward voluntary exploration and short, calm stays inside before a trip; the carrier should not appear only when it is time to leave.
Which changes need veterinary attention?
Contact a veterinarian if your kitten stops eating, develops diarrhea or sneezing, becomes unusually quiet, or has a sudden behavior change. Cats can conceal pain, so a noticeable shift from your kitten’s normal routine is worth taking seriously.
Repeated attempts to urinate, straining or vocalizing in the litter box require immediate veterinary care, especially in a male kitten. AAHA warns that a urinary blockage can become life-threatening quickly. Do not wait to see whether these signs resolve on their own.
