Most cats use litter boxes instinctively, but they are more likely to keep using one when it is clean, easy to reach, and comfortable for them. A cat that stops using the box may be reacting to its litter, location, or access—or may have a medical problem. Straining to urinate or passing little or no urine is an emergency.
Why do cats use litter boxes?
Cats are naturally fastidious, and most will instinctively use a litter box. The box offers a consistent place to eliminate, but instinct does not mean every cat accepts every box or litter. Cleanliness, texture, scent, design, location, and ease of access can all affect an individual cat’s choice.

Cornell University’s Feline Health Center notes that many cats prefer unscented, fine-textured litter and a simple box without a hood. These are common preferences, not rules for every cat; observe what your own cat uses comfortably.
Why might a cat stop using the litter box?
House-soiling can result from a medical problem, an aversion to the box or its surroundings, or a preference for a different place or surface. More than one cause can be involved: for example, a painful episode may lead a cat to avoid the box even after the original problem has changed.

Medical discomfort or urgency
Pain, urgency, increased frequency, reduced control, or difficulty moving can make reaching or using the box uncomfortable. Urinary tract inflammation, kidney disease, diabetes, digestive problems, pain, mobility limitations, and age-related changes are among the issues Cornell identifies as potential contributors to house-soiling.
Box, litter, or location aversion
A box that is dirty, difficult to enter, too small, covered when the cat dislikes covers, or placed somewhere noisy or hard to reach may be avoided. Litter scent and texture can matter too. In a home with multiple pets, another animal may block access, startle the cat, or make the area feel unsafe.

A preferred alternative
A cat may begin choosing a particular room, spot, or surface instead. Check for repeated patterns: the same location, a soft surface, or a route that avoids another pet can help identify what is influencing the choice.
How to make the litter-box setup easier to accept
- Choose a quiet, accessible location. Keep boxes away from feeding areas and places where a cat could be cornered or surprised.
- Provide enough boxes. Cornell’s general guidance is one box per cat, plus one extra. In multi-cat homes, place boxes in separate areas so one animal cannot control access to all of them. The AVMA also recommends separating key resources, including litter-box areas, to help minimize conflict.
- Keep boxes clean. Remove clumps and feces daily when using clumping litter, and clean the box regularly. Replace a cracked box or one that retains odor.
- Match the box to the cat. A larger cat may need more room. Kittens and older cats with arthritis or mobility difficulty may benefit from a lower entry. Many cats accept an uncovered box, but preferences vary.
- Make litter changes gradually. If a change is needed, Cornell advises mixing small amounts of the new litter into the old over time rather than switching abruptly. Unscented, fine-textured litter suits many cats, but the cat’s response should guide the choice.
When is litter-box avoidance a veterinary concern?
A new or persistent change in elimination habits deserves veterinary attention. Cornell recommends an examination and appropriate tests when a cat is house-soiling, since behavior changes can be an early sign of illness or pain.

Watch for difficult or painful urination, frequent attempts, crying while urinating, blood in the urine, or urinating outside the box. These can be signs of lower urinary tract disease. A cat that strains and produces little or no urine may have a urethral obstruction: Cornell describes this as an absolute emergency requiring immediate veterinary treatment. Do not treat straining or inability to pass urine as a litter preference or training issue.
