Why Do Some Cats Have Folded or Crumpled Ears? Causes, Warning Signs and Care

Folded ears may be an inherited Scottish Fold cartilage trait, while a new crumpled ear often follows hematoma scarring or another acquired problem. Learn the differences, warning signs and safe next steps.

Some cats have folded ears because they inherited a cartilage-development mutation, as in Scottish Folds. A cat’s ear can also become crumpled later after an aural hematoma heals with scar tissue, or after other acquired problems such as long-term topical glucocorticoid exposure or sun damage. The appearance alone cannot tell you whether a particular cat is painful.

If the shape changed recently, or the ear is swollen, warm, painful, itchy, smelly, or producing discharge, arrange a veterinary examination. A new change should not be assumed to be a breed trait.

What a folded or crumpled cat ear can mean

“Folded” describes a forward bend that is present as part of a cat’s inherited appearance. “Crumpled” often describes an ear flap (the pinna) that has thickened, shrunk, or wrinkled after injury and healing. Those appearances can overlap, so ear shape is a clue rather than a diagnosis.

Feature Inherited fold Acquired crumpling or tip fold
Typical onset Present from kittenhood or develops as the breed-associated cartilage trait becomes apparent New or progressive change after swelling, irritation, medication exposure, trauma, or sun damage
Common appearance Both ear flaps fold forward in a relatively stable pattern One or both ears may look thickened, shrunken, wrinkled, uneven, or locally bent
Other clues Possible stiffness, altered gait, tail or limb changes, or reduced activity Swelling, tenderness, scratching, head shaking, odor, discharge, or a visible sore may occur
Likely mechanism Inherited cartilage-development variant Hematoma and scarring, chronic medication effect, solar damage, or another outer-ear disorder

These differences help you decide how urgently to seek care; they do not let you identify the cause reliably from a photograph or ear shape alone.

Why Scottish Fold ears fold

An inherited cartilage mutation

Scottish Folds are the best-known cats with ears that bend forward. The fold results from an inherited difference in cartilage development, not from a cosmetic change confined to the ear. The UC Davis Veterinary Genetics Laboratory identifies a dominant mutation in the TRPV4 gene. A single copy can affect cartilage formation and produce the Scottish Fold osteochondrodysplasia phenotype.

Because cartilage is also important in bones and joints, the same biological change can affect the distal limbs and tail. Possible consequences include abnormal bone development, degenerative osteoarthritis, pain, stiffness, and reduced ability to move normally. The ear fold is therefore a visible sign of a wider skeletal condition, although the severity and timing vary between individual cats.

What the mutation does and does not tell you

Having folded ears does not establish how much pain a cat feels today. Some cats show obvious mobility problems early, while others appear comfortable for longer. Cats also tend to conceal pain. A veterinarian must assess gait, joints, tail movement, muscle condition, and behavior rather than using the fold as a severity scale.

Welfare guidance from Cats Protection notes that radiographic signs of osteochondrodysplasia may be visible from about seven weeks of age, and that limping can begin as young as four to six months. These are observations reported in that guidance, not a prediction that every Scottish Fold will develop signs at those ages. No reliable prevalence percentage for each possible outcome is established in the cited material.

Why the inherited disorder matters for welfare

The UK government review describes Scottish Fold osteochondrodysplasia as an inherited disorder linked to the folded-ear trait. Reported effects include skeletal deformity and painful degenerative osteoarthritis that can restrict activity and normal behaviors such as jumping, climbing, grooming, and exploring.

The review states that breeding Scottish Folds with one another, or outcrossing to another breed, cannot eliminate the welfare problem associated with the folded-ear mutation. Several cat registries, including the Governing Council of the Cat Fancy, do not permit Scottish Fold registration because of inherent health and welfare concerns. A registry policy is not the same as a universal legal ban; breeding rules differ by jurisdiction and should be checked with the relevant current authority.

For people who already live with a Scottish Fold

Existing cats need practical monitoring rather than blame. Cats Protection advises regular veterinary checks and attention to changes in mobility or pain. Keep a simple record of jumping, stair use, play, grooming, litter-box access, and resting posture so gradual changes are easier to describe at appointments.

Environmental adjustments can reduce the effort required for everyday activities. Ramps, cat walks, or stable cat trees may help a mobility-limited cat reach preferred elevated areas. A ramp is an access aid, not a treatment for the inherited cartilage disorder. Place resources such as food, water, beds, and litter boxes where the cat can reach them without repeated high jumps, and ask your veterinarian how to adapt the home safely.

How an aural hematoma can leave a crumpled ear

Blood trapped between skin and cartilage

An aural hematoma forms when blood accumulates between the skin and cartilage of the ear flap. Trauma, vigorous head shaking, or repeated scratching can rupture small vessels. The trigger may be an irritating ear condition, including infection or parasites, so the hematoma is often a consequence of another problem rather than an isolated event.

The ear may suddenly look puffy, heavy, warm, or fluid-filled and can be uncomfortable. If the swelling is left to heal or recurs, scar tissue can cause the pinna to thicken, shrink, and wrinkle. The MSD Veterinary Manual describes the result as a crinkled “cauliflower ear” appearance. A healed, misshapen flap can remain even after the original swelling has gone.

Why treating only the swelling may not be enough

Successful care requires an examination of the ear canal and the cat’s skin, because infection, mites, allergy, foreign material, or another source of irritation may be driving scratching and head shaking. Treatment choices depend on the size and age of the hematoma, the underlying cause, and the cat’s condition. A veterinarian may discuss drainage, medication, a procedure to prevent re-filling, or monitoring in selected cases.

Do not squeeze, lance, or puncture an ear flap at home. Improvised drainage can introduce infection, increase pain, and leave a worse scar. Do not put human medicines or leftover ear drops into an ear unless a veterinarian has prescribed them for that cat.

Other causes of a folded ear tip

Long-term topical glucocorticoids

The MSD Veterinary Manual reports that folded ear tips are most often associated with long-term glucocorticoid therapy, including daily eye or ear preparations. The fold may not reverse. This is distinct from the inherited Scottish Fold trait and does not mean that every cat receiving a steroid preparation will develop a folded tip.

Medication effects depend on the drug, concentration, route, duration, and the individual cat. If a change appeared during treatment, tell the prescribing veterinarian exactly which product was used and how often. Do not stop an essential medicine abruptly without advice, and do not assume the medicine caused the change without an examination.

Solar damage

Sun exposure can also be associated with folding of the ear tip. Light-colored or sparsely haired ear margins are especially vulnerable to chronic ultraviolet injury, which may cause redness, crusting, sores, or progressive changes. A folded tip that is scabbed, bleeding, ulcerated, or repeatedly irritated needs prompt veterinary assessment because damaged skin can develop serious disease.

When a folded or crumpled ear needs veterinary attention

Book an appointment promptly for any newly swollen, painful, itchy, or changing ear. Seek advice especially when you notice:

  • Persistent scratching, rubbing, or head shaking
  • Heat, tenderness, or a soft or tense swelling in the ear flap
  • Discharge, foul odor, redness, crusts, or an open sore
  • A sudden change in the ear’s position or thickness
  • Reduced jumping, limping, stiffness, difficulty using stairs, or reluctance to play
  • Changes in grooming, sleeping posture, appetite, litter-box access, or sociability that could indicate discomfort

Urgency rises when the cat is in severe pain, the ear is rapidly enlarging, bleeding will not stop, the cat is weak, or balance and facial movement change. Contact a veterinary service for triage rather than trying to correct the shape yourself.

What a veterinarian may check

  1. History and timeline: The veterinarian will ask whether the fold has always been present, when it changed, whether one or both ears are affected, and whether scratching, head shaking, medication, sun exposure, or trauma occurred.
  2. Physical and orthopedic examination: The ear flap, ear canal, skin, joints, tail, gait, and muscle condition are assessed. The cat’s behavior at home is important because pain may be subtle in the clinic.
  3. Ear evaluation: An otoscope examination and, when indicated, samples of debris help identify infection, parasites, inflammation, or other irritation behind a hematoma.
  4. Imaging or further tests: If a Scottish Fold has stiffness, lameness, or suspected skeletal disease, radiographs and other tests may be recommended. The UC Davis laboratory information describes the genetic variant and phenotype; a genetic result does not replace an examination of current health.
  5. A treatment and monitoring plan: The plan may address infection or parasites, pain, a hematoma, skin disease, or mobility support. Follow-up is important when the ear has been drained or when a chronic joint problem is suspected.

How to observe your cat safely at home

Check the ears in good light without forcing them open or repeatedly handling a painful flap. Compare left and right sides, note whether the change is stable or progressing, and take dated photographs from the same angle for your veterinarian. Record episodes of scratching and head shaking, activity levels, and any difficulty reaching food, water, litter, beds, or favorite resting places.

Keep the cat’s environment calm and avoid unnecessary jumping while an acute ear problem is being assessed. Use stable, non-slip access aids if your veterinarian recommends them. Never apply oils, disinfectants, ear cleaners, steroid creams, or antibiotics on the basis of an internet photograph.

Questions owners commonly ask

Are all cats with folded ears Scottish Folds?

No. Scottish Folds are the classic inherited example, but an acquired hematoma, scar, medication-associated tip fold, or solar damage can produce a similar appearance. Breed identification and medical diagnosis require more than ear shape.

Does a crumpled ear prove that my cat was injured?

No. Scarring after an aural hematoma is one possibility, but long-term topical glucocorticoids, sun damage, and other outer-ear disorders can also alter the pinna. A veterinarian can distinguish these possibilities by examining the timeline, skin, canal, and the rest of the cat.

Can a Scottish Fold look comfortable while having joint disease?

Yes. Cats may hide pain and reduce activity gradually. Regular examinations and attention to subtle changes in jumping, gait, grooming, and posture are more informative than waiting for an obvious limp.

Will a healed hematoma return the ear to its original shape?

Not necessarily. Once scar tissue has caused the pinna to shrink and wrinkle, the “cauliflower ear” shape may persist. Early veterinary care focuses on the swelling and its underlying trigger, but no article can predict the final appearance for an individual cat.

The practical answer

A stable forward fold present from kittenhood, especially in a Scottish Fold, points toward an inherited cartilage mutation that can affect the entire skeleton. A new crinkle, swelling, or uneven fold points toward an acquired ear problem until a veterinarian rules one out. Because the same appearance can have very different implications, seek an examination for any recent change, pain, irritation, or mobility concern.
