The sound alone cannot tell you which: compare it with your cat’s usual pattern, the situation, and any changes in behavior.
Why cats purr in different situations
Purring is not a reliable test of happiness or health. A cat may purr in a comfortable moment, during interaction, or when unsettled. Cats Protection’s 2025 behaviour guide notes that purring associated with contentment and pain can sound very similar; body language and context matter.

1. Comfort and rest
A cat may purr while curled up, resting, or kneading. If this fits your cat’s normal routine and their activity, appetite, and behavior are otherwise typical, it may simply be part of a comfortable resting moment.

Kittens purr in communication with their mother, and cats may purr during positive interactions with people or other cats. The context helps: notice whether your cat approaches, settles near you, or otherwise seems at ease.

3. Seeking attention or food
Some cats use purring when soliciting food or attention. Cats Protection describes this kind of solicitation purr, but frequent purring does not automatically mean your cat wants something. Look for the pattern around meals, interaction, and your response.
4. Stimulation during petting or play
Purring can accompany petting or play. Watch your cat’s response rather than assuming that purring means they want more. If they show signs of overstimulation, such as aggression, stop and give them space.

How to judge a change in purring
Compare the purring with your cat’s normal behavior and the circumstances in which it happens. A familiar purr during a familiar routine is different from a new or noticeably more frequent pattern paired with other changes.

Cats can conceal pain, and a dramatic cry or limp is not required for something to be wrong. VCA’s guide to recognizing pain in cats suggests paying attention to changes in activity, grooming, social behavior, eating, drinking, and sleep. Share your observations with the veterinary team rather than trying to diagnose the cause from purring alone. Do not give human pain medicine unless a veterinarian specifically directs you to do so.

If your cat strains to urinate but produces little or no urine, seek immediate veterinary treatment. Cornell Feline Health Center identifies difficult or painful urination, repeated attempts, vocalizing while urinating, blood in the urine, and urinating outside the usual place as possible signs of lower urinary tract disease. A urethral obstruction can be life-threatening; passing little or no urine requires urgent care, not watchful waiting.
For further signs to discuss with a veterinarian, see VCA’s overview of how pain can affect behavior. Its examples include kitten behavior, so apply that guidance cautiously to adult cats.
