Undigested food coming back up may be regurgitation—a usually passive return from the esophagus—or forceful vomiting. The appearance alone cannot identify the cause. Note what happens and when, and contact a veterinarian if episodes recur or your cat seems unwell.
Is it vomiting or regurgitation?
These terms describe different processes, and the distinction can help your veterinarian decide what to investigate. Merck Veterinary Manual describes vomiting as the forceful ejection of stomach or upper-intestinal contents. It is commonly preceded by signs such as salivation, repeated swallowing, retching, or abdominal contractions. Regurgitation is usually passive and brings food or fluid back from the esophagus; it often happens soon after eating or drinking.

These clues are not a diagnosis. If you can, note how soon the episode follows a meal and whether your cat retches or contracts its abdomen. A short video can help your veterinarian tell the events apart, as VCA Animal Hospitals advises.
What can cause undigested food to come up?
If it is regurgitation
A problem with swallowing or the esophagus may be involved. Esophageal disorders can cause difficulty swallowing and regurgitation. Merck Veterinary Manual notes that esophagitis may be associated with certain drugs, foreign objects, or acid reflux, among other causes. Drooling, repeated or difficult swallowing, pain, appetite loss, or extending the head and neck can also occur with esophagitis.

If it is vomiting
Vomiting can have many digestive and non-digestive causes. Possibilities include inflammation, infection, parasites, constipation, inflammatory bowel disease, pancreatitis, kidney or liver failure, metabolic or nervous-system disorders, food allergy, medications, toxins, foreign objects, and tumors. Hairballs and dietary indiscretion are also possible, but recurring vomiting should not automatically be attributed to hairballs.
When should you contact a veterinarian?
Cornell University College of Veterinary Medicine’s Feline Health Center advises prompt veterinary evaluation when a cat vomits more frequently than once per week. Its guidance also flags vomiting accompanied by lethargy, weakness, reduced appetite, blood, increased thirst, increased or decreased urination, or diarrhea.

Seek veterinary advice promptly if your cat has vomiting with fever, abdominal pain, dehydration, or other concerning signs. Contact a veterinarian immediately if you suspect your cat swallowed a foreign object or toxic substance. Merck Veterinary Manual also identifies long-term vomiting, vomiting more than once or twice daily, or vomiting with weakness, dehydration, fever, or weight loss as reasons for detailed examination. Follow veterinary advice for your cat’s circumstances, especially if signs are severe or worsening.
What may happen during a veterinary assessment?
The veterinarian will use your cat’s history and physical examination to guide next steps. Depending on the signs and suspected cause, evaluation may include blood, fecal, or urine tests and abdominal X-rays or ultrasound. Endoscopy or biopsy may be appropriate in some persistent or unexplained cases. The work-up depends on the examination findings; not every cat needs every test.

Can brushing or home care help?
Regular brushing removes loose fur and can help prevent hairballs, according to Merck Veterinary Manual. It may be useful if swallowed fur is contributing to hairballs, but it does not treat regurgitation, esophageal disease, obstruction, poisoning, or other causes of vomiting. Do not assume that undigested food means your cat ate too fast, has a hairball, or is reacting to a food; recurring episodes or warning signs call for veterinary advice.
